We report the case of a pigeon keeper who presented with a post-traumatic lesion of his third right finger. Cryptococci were found in the lesion in the absence of any other underlying disorder. Treatment with oral itraconazole achieved clinical resolution of the lesion.

Chronic radiodermatitis was already diagnosed a few years after the discovery of X-rays. Despite the fact that protection should be known to be necessary to avoid radiodermatitis, cases of chronic radiodermatitis still occur. The present case report describes a nurse who endured years of direct hand exposure, leading to chronic radiodermatitis ten years after cessation of exposure.

Therapy with khellin and UVA irradiation (KUVA) is a therapeutic approach to vitiligo. Little is known about the photobiological properties of khellin and its long-term side-effects after prolonged administration. A 47-year-old woman suffering from acral vitiligo since the age of 3 is reported, who developed blisters on hands and fingers during KUVA-therapy. Pityriasis rubra pilaris (PRP) is a rare papulosquamous disorder of unknown aetiology. There are only three cases reported occurring coincident with internal neoplasia. We report a case of this disorder presenting as the initial manifestation of a previously undiagnosed malignancy with metastases in the liver. The primary tumour was not detected. The patient had a complete remission of his PRP with topical steroids alone, but died of the malignancy.

A 58-year-old man with mild glucose intolerance noted a subcutaneous cystic nodule on the ventral aspect of his ring finger. He had experienced trauma to the finger while gardening five weeks before the lesion occurred. Exophiala jeanselmei was isolated and identified on culturing of aspirate from the cystic nodule. The nodule was then removed surgically, and a wooden splinter was found close to the subcutaneous fibrous cyst, suggesting that the causal organism had entered the subcutaneous tissue with the wooden splinter.

The first reported case of bilateral pyoderma gangrenosum of the hands is presented. One lesion was treated with wound care only, with spontaneous healing after two months. The contralateral lesion which occurred one month later was treated with wound care and oral dapsone. Healing occurred between four and six weeks after the start of dapsone. It is suggested that this acts by limiting the necrotizing process, thus allowing earlier epithelialization. Pyoderma gangrenosum is rare, and treatment is non-surgical. It is important that it is recognized to avoid extension of the necrotizing process

In conclusion, oil of mustard, contained in many plants and recognized mainly as a skin irritant, is also capable of causing an allergic contact dermatitis. Nasturtium, which contains mustard oil, should be added to the list of plants capable of causing this dermatitis and must be suspected in any patient who handles plants and presents with hand dermatitis.

Pustulotic arthro-osteitis is a rare syndrome manifested by palmo-plantar pustulosis and abnormalities involving the anterior chest wall, the spine, the sacroiliac and peripheral joints. The treatment results in 133 patients who underwent bilateral upper dorsal sympathectomy (BUDS) for palmar hyperhidrosis (HH) are reported. Group 1 (67 patients) was followed for 5 to 10 yr (mean 7) and Group 2 (66 patients) was followed for 1 to 4 yr (mean 2). Early results and complications, early and late sequelae, late recurrence and patient satisfaction with the results of the operation were analyzed.
