Is the tail of the pancreas always tumor-infiltrated when macroscopically affected during cytoreductive surgery? A clinicopathological study and experience from a high-volume center.

BACKGROUND: Distal pancreatic resection during cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) is rare, with limited knowledge available. Therefore, a retrospective observational study was conducted using the data registry of a single institution to identify patients that underwent distal pancreatic resection during CRS + HIPEC. METHODS: All resected pancreatic specimens were examined for invasive parenchymal tumor infiltration. Pre-, peri-, and postoperative variables and their associations were analyzed. RESULTS: Over a period of more than a decade, 31 of

Peritoneal disseminations from appendiceal vs. colorectal mucinous adenocarcinoma. An Italian multicentric comparative study.

BACKGROUND: We compared baseline characteristics, outcomes and patterns of failure between patients undergoing cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) for peritoneal disseminations from appendiceal (MACA) or colorectal mucinous adenocarcinoma (MCRC), in order to assess clinical and prognostical differences. METHODS: Prospectively collected data for 402 patients treated for MACA (n = 218) or MCRC (n = 184) in 17 Italian centers were retrospectively analyzed. RESULTS: Colorectal origin was associated with lower peritoneal cancer index (PCI) (P < 0.001),

Re-cytoreduction predictive Index(RECPI)-based Immune-Ascites(IMA) scoring system for peritoneal metastatic disease: A paired-analysis of surgical and oncological outcomes.

PURPOSE: There is no reliable scoring system to carefully select the recurrent peritoneal metastatic patient for potential re-cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (re-CRS&HIPEC) to optimize patient outcomes. In this cohort, it was aimed (1) to compare the postoperative surgical and oncological outcomes of patients who underwent re-cytoreduction with primary cytoreduction, (2) to assess the prognostic value of modified Glasgow Prognostic Score (mGPS) and CRP-albumin ratio (CAR), and (3) to be able to predict patients who may be a potential candidate for safe re-cy

Mesenteric Peritonectomy as the Seventh Peritonectomy Technique in Cytoreductive Surgery for Peritoneal Surface Malignancies: A Propensity Score-Matched Analysis.

BACKGROUND: Mesenteric involvement represents a major challenge in cytoreductive surgery (CRS) for peritoneal surface malignancies, historically regarded as a relative contraindication to complete cytoreduction. Mesenteric peritonectomy (MP), first described by Deraco et al. in 2009, selectively removes the peritoneal layer at the bowel-mesentery interface. Despite growing interest in its clinical application, robust outcome data from comparative studies remain lacking. METHODS: This retrospective, single-center study included 391 consecutive patients undergoing CRS-hyperthermic intraperitone

Robotic cytoreductive surgery and hyperthermic intra-peritoneal chemotherapy for selected appendiceal neoplasms: Early experience from a national referral centre.

BACKGROUND: Selected high-risk appendiceal neoplasms with tumour perforation or T4 disease may be considered for risk-reducing cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS-HIPEC), even without overt radiological peritoneal disease. Since 2012, our centre has offered laparoscopic CRS-HIPEC, and a robotic approach was recently introduced to enhance visualisation, dexterity, and precision while minimising surgical trauma. This study evaluates the feasibility, safety, and early outcomes of risk-reducing robotic CRS-HIPEC (RCRS-HIPEC) in selected patients with high-risk

Predictive Factors for Peritoneal Metastases in Colorectal Cancer: A Retrospective Study Conducted at a Tertiary Care Center in Riyadh, Saudi Arabia.

Background The incidence of colorectal cancer (CRC) is increasing nationally, emphasizing the importance of early detection. Late-stage diagnosis, particularly with peritoneal metastasis (PM), is associated with poor outcomes despite advancements in treatment. PM poses a challenge owing to the lack of standardized treatment, although cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) shows promise in improving survival rates. This study aimed to document the incidence and risk factors for patients presenting with PM following CRC. Materials and methods This retr

Impact of preoperative inflammatory and nutritional markers on the prognosis of patients with peritoneal metastasis of colorectal cancer.

BACKGROUND: Identifying patients with peritoneal metastasis (PMs) of colorectal cancer (CRC) who will benefit from cytoreductive surgery and hyperthermic intraperitoneal chemotherapy is crucial before surgery. Inflammatory and nutritional indicators play essential roles in cancer development and metastasis. AIM: To investigate the association of preoperative inflammatory and nutritional markers with prognosis in patients with CRC-PM. METHODS: We included 133 patients diagnosed with CRC-PM between July 2012 and July 2018. Patients' demographics, overall survival (OS), and preoperative inflamm

Diagnostic difficulties in the differentiation between an ovarian metastatic low‑grade appendiceal mucinous neoplasm and primary ovarian mucinous cancer: A case report and literature review.

Low-grade appendiceal mucinous neoplasm (LAMN) is a tumor that primarily originates from the appendix and belongs to the family of appendiceal mucinous neoplasms (AMNs). In 50% of female patients, AMNs (particularly LAMNs) have a tendency to metastasize to organs in the genital tract, where the neoplasm can mimic the features of primary ovarian mucinous cancer (POMC). The present case report reviewed the difficulties in differentiating between these two types of tumors. In the present case report, a 61-year-old female patient was admitted to the Second Department of Gynecological Surgery and G

Effect of BRAF mutation on the prognosis for patients with colorectal cancer undergoing cytoreductive surgery for synchronous peritoneal metastasis.

BACKGROUND: KRAS/BRAF mutations (mutKRAS/mutBRAF) are unfavorable prognostic factors for colorectal cancer (CRC) metastases to the liver and lungs. However, their effects on the prognosis for patients with synchronous peritoneal metastasis (S-PM) of CRC after cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) are controversial. In the study, we aimed to determine the effects of mutKRAS/mutBRAF on the prognosis for patients with S-PM who received CRS. METHODS: A total of 142 patients diagnosed with S-PM between July 2007 and July 2019 were included in this study.

Outcomes after curatively intended treatment of limited peritoneal metastases and thermal ablation for liver metastases from colorectal cancer.

OBJECTIVES: Peritoneal metastases (PM) and liver metastases (LM) are present simultaneously in up to 2 % of patients at the time of their colorectal cancer (CRC) diagnosis. Curatively intended treatment includes cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) combined with LM resection. A less invasive treatment for LM is ablation. We aimed to estimate overall survival (OS), disease-free survival (DFS) and postoperative data in patients managed simultaneously with CRS, HIPEC and radiofrequency ablation (RFA) as first choice. METHODS: This was a retrospective

A proposal for changing nomenclature from pseudomyxoma peritonei (PMP) to abdomino-peritoneal mucinous carcinoma (APM) based on its long journey and experience from tertiary oncology center in India.

INTRODUCTION: Pseudomyxoma peritonei (PMP) is a generalized term, usually known as "jelly belly" since 1884. Incidence is very low, 1-3 per million people per year. Because of its indolent nature, it is usually diagnosed at an advanced stage, thereby impacting the quality of life. The 5-year survival rate varies from 23 to 86% in world literature. Even 10 years and 20 years of survival have been described. With our experience, we like to propose rename of PMP as abdomino-peritoneal mucinous carcinoma (APM) as we strongly feel the time has come to specify the term and standardize the management

Thirty-three long-term survivors after cytoreductive surgery in patients with peritoneal metastases from colorectal cancer: a retrospective descriptive study.

BACKGROUND: Cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) improves survival in selected patients with peritoneal metastasis (PM) from colorectal cancer (CRC). However, little has been reported on characteristics and clinical course of long-term survivors with CRC-PM beyond 5 years. The objective of this study was to identify the clinical and oncological features affecting long-term survival of CRC-PM after comprehensive treatment. METHODS: Between January 1990 and April 2015, CRC-PM patients who underwent CRS with or without HIPEC in two Japanese

Laparoscopic total pelvic peritonectomy for colorectal cancer pelvic carcinomatosis: a retrospective case series and photographic/videographic step-by-step guide.

BACKGROUND: Cytoreductive surgery (CRS) for colorectal cancer peritoneal carcinomatosis has been shown to prolong survival with acceptable morbidity rates. Total pelvic peritonectomy (TPP), or complete removal of all pelvic peritoneum, constitutes an important and technically challenging component of CRS. Here we report our experience and describe our technique of laparoscopic total pelvic peritonectomy (LTPP), using a photographic/videographic step-by-step guide. METHODS: All patients who underwent LTPP for pelvic carcinomatosis from a colorectal origin were included in the study. Only patie

Impact of Previous Gynecologic Surgical Procedures on Outcomes of Non-Gynecologic Peritoneal Malignancies Mimicking Ovarian Cancer: Less Is More?

BACKGROUND: Non-gynecologic rare peritoneal surface malignancies (PSMs) often are misdiagnosed as disseminated ovarian cancer and initially treated by gynecologic surgeons. This study aimed to assess whether these previous maneuvers (i.e., full surgical staging and/or cytoreductive attempts) affect outcomes after the definitive surgery performed in a tertiary center. METHODS: The study reviewed 298 women affected by non-gynecologic PSM who underwent cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) after previous gynecologic surgery. Prior surgery was categoriz

Malignant peritoneal mesothelioma: a review.

Malignant peritoneal mesothelioma (MPM) is a very rare malignancy of the peritoneum and has a poor prognosis. Of all mesotheliomas, pleural mesothelioma is more common than MPM. In comparison to pleural mesothelioma, the link with asbestos exposure is weaker (33-50% vs. >80%), but it is still the best-defined risk factor. MPM spreads predominantly expansive rather than infiltrative and symptoms are related to tumor spread within the abdominal cavity. Often, MPM is encountered incidentally by diagnostic imaging or by surgery. Computed tomography scan is widely accepted as a first line modality

Biological mesh is a safe and effective method of abdominal wall reconstruction in cytoreductive surgery for peritoneal malignancy.

BACKGROUND: Patients with peritoneal malignancy often have multiple laparotomies before referral for cytoreductive surgery (CRS). Some have substantial abdominal wall herniation and tumour infiltration of abdominal incisions. CRS involves complete macroscopic tumour removal and hyperthermic intraperitoneal chemotherapy (HIPEC). Abdominal wall reconstruction is problematic in these patients. The aim of this study was to establish immediate and long-term outcomes of abdominal wall reconstruction with biological mesh in a single centre. METHODS: A dedicated peritoneal malignancy database was sea
