Epidermolysis bullosa associated with congenital localized absence of skin, fetal abdominal mass, and pyloric atresia.

A 2320-g male infant was delivered at 35 weeks gestation to a mother who had polyhydramnios. He had a combination of congenital localized absence of skin, unilateral hydronephrosis, and hydroureter due to ureterovesical obstruction, and nonbilious vomiting due to pyloric atresia. Blistering of the skin developed after birth. Epidermolysis bullosa simplex was confirmed by electron microscopy of a skin biopsy specimen. We describe this patient, who had three unusual manifestations of epidermolysis bullosa.

Esophageal atresia.

Esophageal anomalies present a challenge to the pediatric surgeon and demand close attention and care from the nursing staff as well as the surgical staff. We have encountered a survival rate of 96 per cent for all infants undergoing operation for esophageal atresia. An emergency gastrostomy is recommended as an immediate procedure when the diagnosis is established. In many instances, it can be performed with local anesthesia if the infant is particularly small or has developed aspiration pneumonia. The definitive operation is delayed until the infant is in optimum condition to permit a genera

Development and Preliminary Validation of a Rabbit Model of Duodenal Atresia for Training in Pediatric Surgical Skills.

Duodenal atresia is a congenital defect that requires advanced surgical skills. The objective of this study is to present an anatomical defect of duodenal atresia using a rabbit model and evaluate the preliminary experience for the training of surgical skills with pediatric surgeons. Adult white New Zealand male rabbits weighing 3.0 to 4.5 kg were used to create the defect. To simulate the bottom of the dilated blind pouch, the gastric antrum of the rabbit was obliterated using a 2-0 Prolene suture, and the cecal appendix was dissected to simulate the continuation of the duodenum. Participants

In utero diagnosis of congenital pyloric atresia in a single twin using MRI and ultrasound.

Congenital pyloric atresia (CPA) is a rare disorder that has traditionally been diagnosed in the postnatal period. With improvements in ultrasound technology and increasing use of MRI, CPA is now diagnosed with increasing accuracy in utero. This also allows for identification of concomitant anomalies, which greatly affects infant survival. In addition, antenatal diagnosis of CPA and associated anomalies allow for family counseling and planning of treatment for the perinatal period including early referral to a center with pediatric surgical services. Here, we present a case of CPA with preoper

Laparoscopic treatment of duodenal obstruction: report on first experiences in Latin America.

BACKGROUND: There are few reports in the literature on the use of a laparoscopic approach for duodenal obstruction, particularly for duodenal atresia. We report here the results of 4 cases treated laparoscopically, and discuss the safety, feasibility and long-term results of this approach. PATIENTS AND METHODS: Four pediatric patients presented with duodenal obstruction, one with duodenal atresia, one with annular pancreas, and two with duodenal obstruction due to Ladd's bands but without malrotation. Diagnosis was made by clinical evaluation, simple X-ray film in the Ladd's patients, and con

Evidence of a common pathogenesis for foregut duplications and esophageal atresia with tracheo-esophageal fistula.

The pathogenesis of the alimentary tract duplications, including foregut duplications (FgD) remains speculative. The accidental finding of FgD in fetal rats with esophageal atresia and tracheoesophageal fistula (EA-TEF) induced by Adriamycin provided an animal model to investigate a possible relationship between these two entities. Timed-pregnant rats were intraperitoneally injected with Adriamycin (1.75 mg/kg) on gestational Days 6 to 9. Their embryos were harvested by Caesarean section from gestational Days 14 to 21. Forty-six of embryos were processed and serially sectioned in the transvers

[Duodenal atresia. Experiences with 145 patients].

145 children with duodenal atresia were operated on between 1971 and 1988. 57 were premature, 48 newborn, 11 toddlers and 5 older children. Once the immediate perioperative phase is overcome, the postoperative prognosis is determined only by the severity of associated anomalies. Most atresias were found to be located prepapillary. Trisomy 21, malrotations and vitium cordis were the most frequent associated anomalies. Relaparotomies were required in 21%, mostly because of adhesions.

Clinical Utility of Indocyanine Green Fluorescence in Neonatal Surgery: A Single-Center Study and Systematic Review.

The use of indocyanine green (ICG) fluorescence in neonatal procedures is limited to specific pathologies, with variability in its application, highlighting the need to expand its indications and standardize administration protocols. We present our experience and review of the literature.Prospective assessment (2019-2023) of ICG-navigated neonatal surgeries. Administration routes included intravenous, endoluminal, and intracatheter. ICG dosages were variable according to the indication. The results were prospectively registered, focusing on its ability to achieve the desired goal. A systematic

Duodenal stenosis in adult malrotation: When Ladd procedure is not enough: A case report.

INTRODUCTION: Congenital causes of duodenal obstruction can be grouped into intrinsic and extrinsic causes. The degree of obstruction caused by such etiologies determines the severity and timing of symptom presentation. Early neonatal diagnosis is common in patients with etiologies that present with high degrees of obstruction such as atresia whereas etiologies that cause lesser degrees of obstruction such as malrotation and duodenal stenosis can go undiagnosed into adulthood. PRESENTATION OF CASE: We report a case of a 24-year-old female who presented with acute on chronic abdominal pain wit

Liver-Spleen Volume Ratio as a Predictor of Native Liver Survival in Patients with Biliary Atresia.

BACKGROUND: The appropriate timing of liver transplantation (LT) in patients with biliary atresia (BA) who survived with their native livers until adolescence remains controversial. The liver-spleen volume ratio (LSR) has been reported to be efficacious in predicting the prognosis of chronic liver disease. We investigated whether LSR could predict long-term native liver prognosis and serve as an indication for LT in patients with BA. METHODS: Patients with BA who survived with their native liver until the age of 15 years were included. These patients were classified into 2 groups. The unfavor

Prenatal Dilated Rectum: Do We Need to Worry?

BACKGROUND: Advances in prenatal imaging is increasing detection of abnormally dilated bowel. There is no literature to date defining the criteria for a dilated rectum or its association with postnatal pathology. The aim of this study is to investigate the clinical significance of a prenatally identified dilated rectum. METHODS: A retrospective review was performed of all cases of "dilated bowel" on prenatal ultrasound between January 2000 and December 2017 at a single institution. We excluded ventral wall defects from review and sought to include only cases of a prominent or dilated rectum.

Prevalence and pathogenesis of congenital anomalies in cerebral palsy.

BACKGROUND: It has been hypothesised that cerebral palsy (CP) and other congenital anomalies are attributable to feto-fetal transfusion problems in a monochorionic multiple gestation. Thus more than one organ could be compromised leading to the coexistence of two or more anomalies in a fetus. Such anomalies in a singleton birth may be attributable to early demise of the co-conceptus as a vanishing twin. AIM: To determine whether the coexistence of congenital anomalies and CP is greater than a chance finding by comparing the prevalence of congenital anomalies in children with CP with that in t

Living-related liver transplantation for biliary atresia associated with polysplenia syndrome.

This report describes a 1-yr-old boy with biliary atresia (BA) and polysplenia syndrome (PS) who underwent successful living-related liver transplantation (LTx). At the time of initial hepatic portoenterostomy, he was noticed to have a preduodenal portal vein (PV), non-rotation of the intestine, and polysplenia. Because he did not achieve good bile excretion, he underwent a living-related LTx (using a left lateral segment from his mother) at the age of 14 months. Evaluation of the vascular anatomy was made by angiography, magnetic resonance imaging (MRI), computerized tomography (CT), and Dopp

Study of Down syndrome in 238,942 consecutive births.

The genetics and the epidemiology of Down syndrome (DS) was studied in the area which is covered by our registry of congenital malformations … on in 7 cases. The most common types of associated malformations were cardiac anomalies (46.2%) and intestinal atresias (6.0%). Seasonality or time/space clusters were not observed in spite of the Chernobyl nuclear accident. No paternal age effect was demonstrated; 5.3% of the mothers of DS had 2 previous spontaneous abortions (controls 3.7% p < 0.05). At birth, the …
