Anatomical Characteristics Contributing to Patellar Dislocations Following MPFL Reconstruction: A Dynamic Simulation Study.

Pathologic anatomy is a primary factor contributing to redislocation of the patella following reconstruction of the medial patellofemoral ligament (MPFL). A pivot landing was simulated following MPFL reconstruction, with the hypothesis that position of the tibial tuberosity, depth of the trochlear groove, and height of the patella are correlated with lateral patellar maltracking. Thirteen dynamic simulation models represented subjects being treated for recurrent patellar instability. Simplified Hertzian contact governed patellofemoral and tibiofemoral joint reaction forces. Pivot landing was r

Editorial Commentary: Medial Patellofemoral Ligament Reconstruction for Recurrent Patellar Instability-Single or Double Bundle: Does It Really Matter?

Medial patellofemoral ligament reconstruction is considered by most surgeons to be the standard of care for patients with recurrent lateral patellar instability, although the choice of how and when to address concomitant bony pathology (trochlear dysplasia, patella alta, or coronal-plane malalignment-elevated tibial tubercle-trochlear groove distance) remains unclear. Medial patellofemoral ligament reconstruction works to re-establish the primary static restraint to lateral translation of the patella and reduce the risk of recurrent dislocation. Regardless of graft choice or construct, this op

Treatment of patella alta in patients with episodic patellar dislocation: a systematic review.

PURPOSE: While there are numerous anatomic contributors to patellar instability, the role of patella alta has been traditionally under-appreciated. The goal of this systematic review is to identify the described techniques for treating patella alta in skeletally mature patients with episodic patellar dislocation (EPD) and review their published results. METHODS: A comprehensive literature review was performed to identify published surgical techniques and their results. Tibial tubercle distalization is the primary described treatment for patellar alta in patients with EPD, and five studies rep

Patellofemoral disorders and instability.

Originally the main idea was to obtain a stable patella, i.e., to stabilize the "slipping patella". In the past many conditions like patella alta, ligamentous laxity, PF bone hypoplasia, weakness of the quadriceps muscle, genu valgum or genu recurvatum were thought to predispose to patellar instability. For a long period muscle exercises were instituted to strengthen the weak m.vastus medialis and to make vastus lateralis stronger. This pulls the patella laterally, especially during running or jumping, when lateral luxation of the patella occurs. Muscle imbalance as well as anatomical abnormal

Neurologic and vascular injuries associated with knee ligament injuries.

BACKGROUND: Multiple ligament knee injuries are commonly associated with neurovascular complications such as popliteal artery, common peroneal nerve, and tibial nerve injuries. HYPOTHESIS: While the importance of identifying these complications in association with bicruciate and triligamentous injuries has been well established, it is important to recognize that any biligamentous and some single ligament knee injuries present with neurological and vascular complications as well. STUDY DESIGN: Literature review. CONCLUSION: Popliteal artery injuries require immediate intervention to help pre

MPFL reconstruction for recurrent patella dislocation: a new surgical technique and review of the literature.

Recurrent dislocation, subluxation and functional instability due to patellofemoral pain might be present in 30 % to 60 % of patients managed non-operatively for posttraumatic patella instability. Disruption of the capsule, medial patella retinaculum and/or vastus medialis obliquus have been associated with recurrent patella instability but recently the medial patellofemoral ligament (MPFL) has been recognised as the most important ligamentous stabiliser preventing lateral dislocation of the patella. Many nonanatomical surgical techniques for the treatment of recurrent patellar dislocation hav

In vivo contact areas of the knee in patients with patellar subluxation.

OBJECTIVE: Ex vivo studies have suggested that cartilage contact areas and pressure are of high clinical relevance in the etiology of osteoarthritis in patients with patellar subluxation. The aims of this study were therefore to validate in vivo measurements of contact areas with 3D open magnetic resonance imaging (MRI), and to study knee joint contact areas in patients with patellar subluxation at different angles of knee flexion in comparison with healthy subjects. METHODS: 3D-MRI data sets of 12 healthy volunteers and eight patients with patellar subluxation were acquired using a standard

Current concepts of lateral patella dislocation.

Surgical treatment of patellar dislocations, acute and chronic, has evolved significantly over the past decade with the advance of biomechanical knowledge of patellofemoral restraints and injury patterns identified by physical examination and improved imaging techniques. There continues to be no consensus on treatment parameters. Despite the presence of predisposing factors, such as dysplasia or generalized hyperlaxity, medial retinacular injury associated with primary (first-time) patellar dislocations represents a ligament injury, which may result in residual laxity of the injured structure.

Combined congenital diastasis of the inferior tibiofibular joint with contralateral distal femoral bifurcation and hemimelia of the medial structures. A case report.

A case of congenital bilateral lower limb deformities is presented. Diastasis of the distal tibia and fibula has the appearance of a severe clubfoot. Recognition of this condition and early surgical treatment are important. Tibial aplasia is best treated by disarticulation and early mobilization.

Morphological Changes in Femoral Trochlea After Extensor Realignment Surgery in Children With Congenital Patellar Dislocation.

BACKGROUND: Femoral trochlear dysplasia (FTD) is a major factor affecting the stability of the knee joint. Congenital patellar dislocation (CPD) is usually accompanied by distal FTD. Few studies have investigated trochlear development following surgery in skeletally immature patients. PURPOSE: To observe trochlear remodeling following surgical extensor realignment and stabilization in children with CPD and to analyze the factors associated with this remodeling. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A retrospective analysis was conducted on surgical cases of CPD treated a

Ninety-day postoperative mortality and complications in continuous and unselected single-stage revisions for chronic periprosthetic joint infection.

PURPOSE: Single-stage revision has gained significant attention as a major surgical approach for periprosthetic joint infection (PJI). However, the 90-day mortality and complication profile of single-stage revision is poorly characterized. The purposes of this study were to determine the incidence rates of and identify the risk factors for 90-day postoperative mortality and complications of single-stage revision for chronic PJI. METHODS: A retrospective review was conducted on patients who underwent single-stage revision for PJI between August 2000 and May 2022. Patient demographics, 90-day m

Long-term functional outcome after lateral patellar retinacular release in adolescents: an observational cohort study with minimum 5-year follow-up.

BACKGROUND: Lateral patellar retinacular release has been recommended for patients with patellar tilt, tight lateral retinaculum, patellar subluxation, patellar dislocation, and patellofemoral pain. Studies of long-term outcomes after lateral release are limited, especially for differing indications. HYPOTHESIS: Adolescents do well after lateral retinacular release in the 5- to 22-year time frame. METHODS: Patients having undergone lateral retinacular release between the years of 1981 and 1999 were contacted. Evaluation was by the Cincinnati and Lysholm scales and by level of satisfaction an

Modified Dejour trochleoplasty for severe dysplasia: operative technique and early clinical results.

Instability of the patella occurs when it fails to track fully in the femoral groove during knee movement. The groove begins at the metaphysis of the distal femur and extends on its anterior surface inferiorly to the femoral notch. It is normally present at birth and the cartilaginous shape is similar to that of the adult. An abnormal trochlea may be shallow. In extreme cases it may be dome-shaped, where it is associated with recurrent subluxation or dislocation of the patella. In these cases it is logical to consider fashioning a groove as part of a realignment procedure. This study reports t

Ice-skating injuries.

The range of injuries sustained at an ice-rink and presented to an Accident Service department is described. A total of 203 patients with 222 injuries presented themselves during a 2-month period. There were 103 noteworthy injuries, including 61 fractures, 2 dislocations and 2 severed tendons, but the commonest injuries were wounds, sprains and bruises. Beginners appear to be more prone to injury than experienced skaters. In addition to using well-fitting skate-boots to protect the ankle, some injuries could be avoided by wearing elbow and knee pads, and a thick pair of gloves. The number of i
