[The manual lifting of patients: the results of an assessment of exposure to the specific risk and of a clinical study in a rehabilitative institute].

An investigation was carried out in a rehabilitation centre in the Province of Milan in order to quantify exposure to patient handling and assess impairment of the spine induced by this risk factor. Altogether 5 departments were analyzed, with different rehabilitation aims (cardiological, respiratory and neurological); 97 of the 104 exposed staff underwent physical examination by an occupational health physician. Both exposure assessment and identification of impairment were performed using the methods proposed by the EPM Research Group. The exposure indices (MAPO Index) were high in 4 departm

Bedside nuclear medicine investigations in the intensive care unit.

The functional nature of nuclear medicine procedures makes them especially valuable in the management of patients undergoing intensive care. However, the severe nature of the patient's condition invariably prevents him or her from attending the nuclear medicine department for diagnostic investigations. We have piloted a bedside nuclear medicine service using a four-probe detector system linked to an IBM computer with curve processing software. Protocols for a range of radionuclide probe investigations, including renal, hepatobiliary, gastric outflow and lung vascular permeability, using 99Tcm

[Hypertensive emergency and urgence].

DEFINITION, PATHOPHYSIOLOGY, THERAPY: The hypertensive crisis is characterized by a massive, acute rise in blood pressure. Patients with underlying hypertensive disease usually have an increase in systolic blood pressure values > 220 mmHg and diastolic values > 120 mmHg. The severity of the condition, however, is not determined by the absolute blood pressure level but by the magnitude of the acute increase in blood pressure. Thus, in the presence of primarily normotensive baseline values (such as those in eclampsia), even a systolic blood pressure > 170 mmHg may lead to a life-threatening cond

Effects of cover cropping on orchard soil microbiomes: Mechanisms and perspectives.

Orchards have long faced severe soil erosion, acidification of red soils, low nutrient-use efficiency, and frequent soil-borne diseases. Conventional clean tillage combined with intensive chemical inputs often fails to simultaneously improve fruit yield and quality while safeguarding orchard ecological security. Cover cropping (i.e., managed groundcover vegetation) introduces persistent surface plant cover and introduces continuous inputs of root exudates, litter, and residues, while simultaneously modifying soil moisture, temperature, aggregation, porosity, and nutrient availability. Conseque

Development of a multidisciplinary medication management program in nursing homes: protocol for a randomized controlled trial : Multidisciplinary medication management in nursing homes.

BACKGROUND: Polypharmacy and the use of potentially inappropriate medications are common among nursing home residents and are associated with negative outcomes. Although deprescribing has been proposed as a way to curtail these problems, the best way to implement multidisciplinary comprehensive medication review and deprescribing and its real impact in specific high-risk populations, such as nursing home residents, is still unclear. This multicenter randomized controlled clinical trial aims to assess the effects of a multidisciplinary mediation management program on medication use and health p

Exploring the barriers to the antiretroviral therapy adherence among people living with HIV in Bangladesh: A qualitative approach.

INTRODUCTION: Since the evolution of highly active antiretroviral therapy (ART), a near-perfect ART adherence level (>95%) is needed to control viral suppression. Non-adherence to treatment may lead to acquired immunodeficiency syndrome (AIDS) and drug resistance. Though the Bangladesh government provides free treatment and counselling services to people living with human immunodeficiency virus (PLHIV), only 22% of the identified PLHIV continue treatment. Therefore, this study aims to explore the barriers that obstruct the Bangladeshi PLHIV to ensure ART adherence. METHODS: We conducted a qua

Quality of maternal and newborn hospital care in Brazil: a quality improvement cycle using the WHO assessment and quality tool.

BACKGROUND: Increasing efforts toward quality improvement (QI) are necessary in low- and middle-income countries (LMICs) to reduce maternal and perinatal mortality and morbidity and to promote respectful care. In Brazil, perinatal health indicators are below targets in several states despite universal access to perinatal services and very high rates of institutional births, indicating poor quality of care (QoC) as a key issue to be addressed. However, research efforts to develop and test QoC improvement interventions are scarce. OBJECTIVES: We assessed the effects of a 1-year comprehensive QI

Examining the benefits and harms of Alzheimer's disease screening for family members of older adults: study protocol for a randomized controlled trial.

BACKGROUND: Multiple national expert panels have identified early detection of Alzheimer's disease and related dementias (ADRD) as a national priority. However, the United States Preventive Services Task Force (USPSTF) does not currently support screening for ADRD in primary care given that the risks and benefits are unknown. The USPSTF stresses the need for research examining the impact of ADRD screening on family caregiver outcomes. METHODS: The Caregiver Outcomes of Alzheimer's Disease Screening (COADS) is a randomized controlled trial that will examine the potential benefits or harms of A

Identifying common barriers and facilitators to linkage and retention in chronic disease care in western Kenya.

BACKGROUND: Sub-Saharan Africa is increasingly being challenged in providing care and treatment for chronic diseases, both communicable and non-communicable. In order to address the challenges of linkage to and retention in chronic disease management, there is the need to understand the factors that can influence engagement in care. We conducted a qualitative study to identify barriers and facilitators to linkage and retention in chronic care for HIV, tuberculosis (TB) and Hypertension (HTN) as part of the Academic Model Providing Access to Healthcare (AMPATH) program in western Kenya. METHOD

Medication Adherence in Comorbid Hypertension and Diabetes: An Exploratory Descriptive Qualitative Study of Healthcare Providers' Perspectives.

BACKGROUND AND AIMS: Medication adherence is a critical determinant of clinical outcomes among patients with comorbid hypertension and diabetes mellitus. Despite the availability of effective therapies, adherence remains suboptimal, particularly in low- and middle-income settings where multiple patient, treatment, and health system factors influence medication-taking behavior. Healthcare providers are uniquely positioned to identify barriers and facilitators to adherence. This study explored healthcare providers' perspectives on medication adherence among patients with comorbid hypertension an

Surgery of cerebral trauma and associated critical care.

The last 30 years have been both exciting and frustrating for those in the field of traumatic brain injury (TBI). Much has been learned, but no new treatment has been shown to improve patient outcomes despite the execution of many clinical trials. The overall incidence of TBI has decreased, probably because of intensive efforts toward prevention and education. Rigorous assessment of available research has produced several evidence-based guidelines for the management of neurotrauma patients. The creation of organized emergency medical services systems in many regions has improved prehospital ca
