[Mandibular desmoplastic fibroma: diagnosis and therapeutics difficulties].

INTRODUCTION: The desmoplastic fibroma is a rare bone tumor, characterized by aggressive local infiltration, with frequent recurrence. The most common site is the mandible. Radioclinical signs are not specific and the histological diagnosis may be difficult. CASE: A 16 year-old male patient consulted for a painless and hard left mandibular swelling, without inferior alveolar nerve disorders. The tumor extended from tooth 31 to tooth 35, the mucosa was healthy. The panoramic view showed a multilocular osteolytic lesion with dental root resorption. CT scan showed expansion of bony cortex with r

Thalidomide-related vessel thrombosis in microsurgical free tissue transfer.

This article describes a 49-year-old man with lower gum cancer who received tumor ablation by an otolaryngologist and immediate reconstruction with microsurgical free tissue transfer. The thrombosis occurred at the arterial anastomotic site at least twice during operation by an experienced microsurgeon and progressed to skin paddle necrosis following surgery. The thalidomide-induced recipient vessel thrombosis is highly suspected after excluding other hypercoagulation problems and technique errors. While thalidomide is increasingly used as an adjuvant therapy agent in head and neck cancer trea

Radiographic assessment of Gardner's syndrome.

The detection of osteomas in the maxillofacial region may be the initial clinical finding in Gardner's syndrome (GS). The most common location of osteomas is in the skull, but the lesion can also occur in the jaws. We present a case of a 47 year old male patient with GS who was referred for radiological evaluation. Extraoral examination revealed an epidermoid cyst and the patient had a history of intestinal polyps. A panoramic radiograph demonstrated numerous osteomas and diffuse sclerosis of the mandible, and compound odontomas with impacted teeth. CT scan allowed the localization and extensi

Eruption of a severely displaced second permanent molar following surgical removal of an odontoma.

The process of tooth eruption is very complex, and many of its factors remain unknown. Although radiographic features can provide clues into the eruptive potential of a tooth, underlying factors that affect tooth development and eruption are not as well defined, ranging from local disturbances to systemic disease. In addition, it is difficult to predict which teeth will require treatment and when the optimal time is to intervene. The purpose of this report is to illustrate the eruption potential of an impacted molar following the removal of a developing odontoma, despite its unfavorable positi

[Stability of dental implants in microvascular scapula and iliac crest transplants].

STUDY: In this study 24 patients with tumours of the mandible, mandibular resection, neck dissection and reconstruction by microvascular iliac crest (13) or scapula transplants (11) were examined following implantological treatment. RESULTS: In all patients the implantological examination was performed on average two years and five months after implant insertion. This allowed for observation of periotest values, periimplant probing depth and sulcus bleeding (SBI). Furthermore, the loss of periimplant bone was registered radiologically. In both groups periotest values were normal. In the group

Serratus anterior muscle in composite head and neck flaps.

The serratus muscle (SAM) can be raised as an isolated muscular or osteomuscular flap on its nutrient vessels or in combination with a latissimus dorsi (LD) myocutaneous flap on the thoracodorsal pedicle for various reconstructive purposes. The aim of the present is to report on the results achieved after the use of the SAM alone (n = 7) and in combination with an LD (n = 3) after tumor resection or osteoradionecrosis (ORN) in ten patients with defects of the head and neck. One flap was lost due to venous thrombosis. The donor site morbidity was minimal. Emphasis is given on a new indication,

Ameloblastoma in young persons: a clinicopathologic analysis and etiologic investigation.

Ameloblastoma, an odontogenic tumor of ectodermal origin, has been reported to arise, on rare occasions, in a primordial or dentigerous cyst of a young person. Numerous authors have suggested differing nomenclatures for these ameloblastomas (e.g., mural, unicystic, monocystic, intracystic, cystogenic, cystic, plexiform unicystic) and have sought to describe and classify the clinical and histopathologic features. These tumors have been characterized as a distinct variant exhibiting less aggressive behavior and a lower rate of recurrence than conventional ameloblastoma. Furthermore, various etio

Mandibulotomy: a surgical approach to oral and pharyngeal lesions.

The history of mandibulotomy is reviewed. The surgical steps involved and the variations of techniques are discussed. Two illustrative cases are reported and the clinical applications of the approach are discussed. It is concluded that mandibulotomy has a place in the management of malignancies as well as awkwardly situated benign lesions as well as malignancies.

Clinical application of the functional matrix: mandibular reconstruction and monobloc functional therapy.

A 13-year-old black girl with an extensive ameloblastoma of the mandible required subtotal mandibular resection and immediate reconstruction with use of an autogenous, ipsilateral, corticocancellous graft from the iliac crest and monobic functional therapy. Twenty-seven months later, clinical evaluation showed normal mandibular function and no facial asymmetry. Bony architectural changes after 27 months included reformation of the coronoid process, angular process, and external oblique ridge; preservation of mandibular length and thickness; and maintenance of the alveolar ridge height. The ref

Oral and maxillofacial tumours in children: a review.

This retrospective review presents our experience of oral and maxillofacial tumours in children. The subjects were 250 children under the age of 15 years (out of a total of 2747 patients with oral and maxillofacial tumours), who were treated after histopathological confirmation of their diagnoses during the 28 years 1965-92. Diagnosis, incidence, and age at presentation were the main outcome measures and the results showed that 232 patients (93%) had benign tumours and 18 (7%) were malignant. The most common benign tumour was haemangioma (n = 69) and the most common malignant tumour sarcoma (n

The deep circumflex iliac artery perforator flap (DCIAP)--a reconstructive option for the large composite oro-mandibular cutaneous defect.

The deep circumflex iliac artery (DCIA) flap is often used for mandibular reconstruction but it is bulky and causes additional donor-site morbidity because of the inclusion of an "obligatory internal oblique muscle". Large composite segmental mandibular resections that consist of floor of mouth, subtotal tongue, and adjacent facial skin are a challenge in terms of reconstruction. They often require 2 free flaps or a free scapular flap and both have disadvantages. The deep circumflex iliac artery perforator (DCIAP) flap with a cutaneous component overcomes the disadvantages. We describe reconst

Parotid gland squamous cell carcinoma invading the temporomandibular joint.

BACKGROUND: Tumor invasion of the temporomandibular joint from the parotid gland is rare. Practitioners should be able to differentiate tumor involvement from temporomandibular disorders (TMDs). CASE DESCRIPTION: . The authors present case reports of two patients with parotid gland masses accompanied by pain, dysfunction and other symptoms not consistent with musculoskeletal disorders. In both cases, clinicians initially reached an incorrect diagnosis and treated the masses as if they were TMDs, which delayed a definitive diagnosis and provision of appropriate treatment. CONCLUSIONS: Dentist

Neurofibromatosis type 1: a clinicopathological study of the orofacial manifestations in 6 pediatric patients.

Neurofibromatosis type 1 (NF1) is a relatively frequent mucocutaneous syndrome, which is transmitted as an autosomal dominant trait or which may represent neomutation. It is characterized by a variety of clinical manifestations, including multiple neurofibromas that are associated with a high risk of sarcomatous transformation. The aim of this report was to elucidate the orofacial manifestations observed in 6 pediatric patients (between 4 and 15 years of age) diagnosed with NF1. Physical, clinical, radiological, histological, and immunohistochemical studies were performed. Orofacial lesions we

Delayed Infection of Porous Polyethylene Implants After Oncologic Maxillectomy and Reconstruction: 2 Case Reports and Review of Literature.

Medpor porous polyethylene implants are commonly used for facial skeletal reconstruction due to reported biocompatibility, fibrovascularization, and durability. While uncommon, late implant infections are an important consideration. We report delayed infections in 2 patients after unilateral total oncologic maxillectomy and reconstruction using Medpor implants for an ossifying fibroma and squamous cell carcinoma, respectively. In the first patient, annual interval computed tomography (CT) scans showed no recurrence of tumor or inflammatory changes. The second was lost to follow-up after adjuva

Clinical study on mandibular fracture after marginal resection of the mandible.

OBJECTIVES: Postoperative mandibular fracture (PMF) after marginal resection (MR) of the mandible remains an unresolved issue, and it has been reported that at least 10 mm of postoperative mandibular body height (PMBH) is required to prevent PMF. This study evaluated the clinical, physical, and structural risk factors for PMF in MR patients and determined appropriate preventive measures for PMF. STUDY DESIGN: This retrospective study included 44 patients with lower gingival carcinoma who underwent MR. PMF occurred in four of these patients. Thirteen associated factors identified from medical

Reconstruction of lateral through and through oro-mandibular defects following oncological resections.

BACKGROUND: Resections of oromandibular squamous cell carcinoma involving lateral mandible, oral cavity, and the skin, lead to composite oromandibular defects that can be approached in several ways depending on the extension of the bone defect, of the soft tissue and cutaneous resection, the patient's general status and the prognosis. Purpose of the study is to evaluate retrospectively functional and esthetic outcome obtained with different reconstructive technique employed. METHODS: A retrospective evaluation of 42 patients has been performed. The study population consisted of 24 males (57.1
