Streamlining intrapartum glycemic control: Subcutaneous insulin for intrapartum diabetes management.

OBJECTIVE: Optimal strategies for intrapartum glycemic control among patients with gestational diabetes mellitus (GDM) and type 2 diabetes mellitus (T2DM) remain uncertain. In 2024, our labor and delivery unit implemented a subcutaneous (SC) insulin protocol to replace routine intravenous (IV) insulin initiation when glucose exceeded a treatment threshold. We hypothesized that this change would not worsen neonatal outcomes and would improve nursing (RN) satisfaction. METHODS: We conducted a retrospective cohort quality improvement study of patients with GDM or T2DM who delivered at ≥36 weeks

Continuous Subcutaneous Insulin Infusion Versus Multiple Daily Injections for Glycemic Management in Pregnant Women With Type 1 Diabetes: A Systematic Review and Meta-Analysis.

BACKGROUND/OBJECTIVE: To compare glycemic management outcomes between continuous subcutaneous insulin infusion (CSII) and multiple daily injections in pregnancy and assess maternal and neonatal outcomes. There is an ongoing increase in diabetes rates worldwide. The International Diabetes Federation estimated that there were 23.0 million cases of hyperglycemia (19.7%) before and during pregnancy in 2024 worldwide, causing 1.5 million deaths annually. Pregnant women with pregestational type 1 diabetes mellitus (T1DM) struggle with 2-4 times more maternal and fetal complications than pregnant wom

Programming of hypertonicity in neonatal lambs: resetting of the threshold for vasopressin secretion.

Lambs exposed in utero to maternal hypertonicity demonstrate plasma hypertonicity and arterial hypertension. To determine whether hypertonicity is due to an altered osmoregulatory set point, we examined arginine-vasopressin and cardiovascular responses to hypertonic saline infusion in these offspring. Study lambs [dehydrated (Dehy)] were exposed to maternal hypernatremia (8-10 mEq/liter increase; 110-150 d gestation) induced by water restriction. Control singleton and Control twins were born to ewes provided ad libitum water. We anticipated reduced birth weight due to maternal dehydration-indu

Healthcare workers' perceptions about the use of mobile health technologies in public health facilities in Lagos, Nigeria.

BACKGROUND: Mobile health has enormous potential in healthcare due to the increasing use of mobile phones in low- and middle-income countries; its effective deployment, uptake, and utilization may result in improved health outcomes, including a reduction in neonatal deaths. However, there is a suboptimal uptake of mobile health technologies among healthcare workers in low-resource settings like Nigeria, which are often context-specific. OBJECTIVE: To investigate healthcare workers' perceptions of mobile health technologies in public health facilities in Lagos, Nigeria. METHOD: A qualitative

A Quality Improvement Initiative: Improving First-hour Breastfeeding Initiation Rate among Healthy Newborns.

INTRODUCTION: "No matter where a newborn takes his or her first breath, the desire to give that baby the best start in life is universal." The best gift a mother can give her baby is the gift of health. The gift of health can be given to the baby through early and adequate breastfeeding. Globally, only 2 out of 5 newborns are put to the breast within the first hour of life. Therefore, initiating breastfeeding is an evidence-based intervention for improving neonatal survival. METHODS: We aimed to improve the first-hour breastfeeding initiation rate from the existing 12%-80% over 3 months throu

Medical-Device Related Pressure Injuries to Children in the Intensive Care Unit.

Medical devices that are attached to children put pressure on their skin or mucous membranes, and the devices can lead to injuries. The objective of this study was to determine the effectiveness of injury-prevention guidance about children who need to have medical devices attached to their bodies as part of their treatment. The research used a randomized controlled trial method, along with a cross-over design in 50 respondents with age ranges from 1 day to 18 years. The control group was treated in accordance to hospital routine, while the intervention group was given medical treatment based o

International Guideline on the Diagnosis, Treatment, and Monitoring of Long-Chain Fatty Acid Oxidation Disorders (LC-FAOD).

Long-chain fatty acid oxidation disorders (LC-FAOD) are rare inherited defects of mitochondrial β-oxidation that impair energy generation during fasting or metabolic stress. Clinical manifestations range from neonatal hypoketotic hypoglycemia and cardiomyopathy to hepatopathy, recurrent rhabdomyolysis, and chronic myopathy. Although newborn screening (NBS) enables early detection, management remains inconsistent due to limited evidence, broadening phenotypic spectrum, and regional practice variation. An international multidisciplinary workgroup of 34 members, including clinicians, nurses, diet

Mother's Lived Experience During Repair of Long-Gap Esophageal Atresia: A Phenomenological Inquiry.

BACKGROUND: Infants born with long-gap esophageal atresia (LGEA) pose unique physiologic risks in the newborn period. Anatomic and physiologic anomalies require an extended hospitalization with procedural analgesia and sedation that impact the mother's experience of birth, maternal response, and nurturing of her infant. PURPOSE: The aim of this study was to understand the meaning of experiences that mothers of infants born with LGEA encounter in the neonatal intensive care unit while their infant undergoes esophageal repair. METHODS: A hermeneutical phenomenological design was used to guide

A Simple and Safe Protocol for Intra-Testicular Gene Delivery in Neonatal Mice Using a Convenient Isoflurane-Based Anesthesia System.

Newborn mice (up to 6 d after birth) are suitable for genetic manipulations, such as facial vein-mediated injection, owing to their hairless and thin skin. Their small body volumes also facilitate the rapid dissemination of injected solutions, supporting gene engineering-related experiments. However, anesthesia in newborns is challenging because of the potential risks associated with anesthetic agents. Isoflurane inhalation anesthesia is an option, although its effects on brain development remain under investigation. In this study, we established a reproducible protocol for delivering nucleic

Midwifery practice in Mongolia: Policy implications for accelerating care delivery transformation.

Midwives can play a significant role in reducing maternal and neonatal mortality rates as well as in improving maternal and perinatal health outcomes in low- and middle-income countries such as Mongolia. However, the shortage of midwives in Mongolia is severe. Despite the evidence indicating numerous challenges associated with substandard midwifery education and practice in Mongolia, there is a need for policy recommendations to accelerate the improvement of midwifery care delivery in the country. Therefore, we identified three main topics as key issues in Mongolian midwifery care: 1) the curr
