Forensic pathology and the miscarriage of justice.

A case of conviction for rape-murder that was eventually overturned based on a post-conviction re-evaluation of the findings at autopsy is d … erpretation of postmortem artefacts. The evolving role of the evidence-based framework for forensic pathology is explored as a systemic solution to enhance the administration of justice.

[Breast lesion diagnosis: integrating pathology and radiology].

Preoperative breast biopsies do not always provide a complete picture of a breast abnormality, especially in the subgroup of breast lesions classified as BI-RADS category 4 ('suspicious'). In such cases correlation with imaging results is mandatory. Sometimes ultrasound is the best modality, as demonstrated here in a 77-year-old patient with an intracystic papillary carcinoma with invasion. At other times mammography is preferred, as we demonstrate in a 50-year-old patient with screen-detected microcalcifications, initially diagnosed as ductal carcinoma in situ of the clinging type, later diag

Calcimimetics inhibit renal pathology in rodent nephronophthisis.

The development and progression of renal cysts appears to be driven by reduced cellular calcium and increased cyclic adenosine monophosphate (cAMP) from G-protein-coupled receptors. To test whether treatment with a calcimimetic that stimulates the G-protein-coupled calcium-sensing receptor might normalize cystic epithelial cell intracellular calcium and cAMP, thereby inhibiting cyst progression, we used pcy mice. These animals develop cysts principally in the collecting duct, as do humans with nephronophthisis (NPHP). We administered the calcimimetic R-568 mixed in their food at early or late

[Joint endoprosthesis pathology. Histopathological diagnostics and classification].

Prosthesis durability has steadily increased with high 10-year rates of 88-95%. However, four pathogenetic groups of diseases can decrease prosthesis durability: (1) periprosthetic wear particle disease (aseptic loosening) (2) bacterial infection (septic loosening) (3) periprosthetic ossification, and (4) arthrofibrosis. The histopathological "extended consensus classification of periprosthetic membranes" includes four types of membranes, arthrofibrosis, and osseous diseases of endoprosthetics: The four types of neosynovia are: wear particle-induced type (type I), mean prosthesis durability (M

Lymphatic involvement in vertebral and disc pathology.

STUDY DESIGN: Analysis of lymphatic vessels in childhood and adult normal and pathological vertebral bone and intervertebral disc tissue. OBJECTIVE: To determine whether lymphatic vessels are present in spinal vertebrae and intervertebral discs in normal children and adults (4-30 years) as well as in pathological lesions of the spine. SUMMARY OF BACKGROUND DATA: There is uncertainty regarding the presence or absence of lymphatic vessels in normal intervertebral discs and the role of lymphatics in the pathobiology of disc degeneration and infective, neoplastic, and other spinal pathology. ME…

Imaging correlates of pathology in corticobasal syndrome.

BACKGROUND: Corticobasal syndrome (CBS) can be associated with different underlying pathologies that are difficult to predict based on clinical presentation. The aim of this study was to determine whether patterns of atrophy on imaging could be useful to help predict underlying pathology in CBS. METHODS: This was a case-control study of 24 patients with CBS who had undergone MRI during life and came to autopsy. Pathologic diagnoses included frontotemporal lobar degeneration (FTLD) with TDP-43 immunoreactivity in 5 (CBS-TDP), Alzheimer disease (AD) in 6 (CBS-AD), corticobasal degeneration in 7…

Pathology and current treatment of neurodegenerative sphingolipidoses.

Sphingolipidoses constitute a large subgroup of lysosomal storage disorders (LSDs). Many of them are associated with a progressive neurodegeneration. As is the case for LSDs in general, most sphingolipidoses are caused by deficiencies in lysosomal hydrolases. However, accumulation of sphingolipids can also result from deficiencies in proteins involved in the transport or posttranslational modification of lysosomal enzymes, transport of lipids, or lysosomal membrane proteins required for transport of lysosomal degradation end products. The accumulation of sphingolipids in the lysosome together

Advances in hepatobiliary pathology: update for 2010.

Recent publications on hepatology and hepatic pathology provide a wealth of new information on wideranging topics. Morphologic aspects of liver disease associated with hepatitis B and C viruses, autoimmune hepatitis, and HIV infection were addressed, as was the prevalent problem of nonalcoholic fatty liver disease. Advances in diagnosis and pathogenesis of primary biliary cirrhosis, primary sclerosing cholangitis, and the increasingly complex spectrum of IgG4 hepatobiliary diseases were also reported. The histologic and immunohistochemical features of the rare "calcifying nested stromal-epithe…

Incidental bony pathology when reporting trauma orthopantomograms.

Radiologists frequently report orthopantomograms (OPTs) and other views of the mandible, most often in patients who have suffered facial trauma. These examinations may reveal incidental pathology. It is important that radiologists are aware of the radiological appearances and the clinical significance of these lesions. In this review we will present examples of the more common odontogenic lesions including: radicular cyst, odontogenic keratocyst, dentigerous cyst, ameloblastoma, and also examples of non-odontogenic pathology: bisphosphonate-related osteonecrosis of the jaw (BRONJ) and chronic …

Endometriosis - morphology, clinical presentations and molecular pathology.

Endometriosis is found predominantly in women of childbearing age. The prevalence of endometriosis is difficult to determine accurately. Laparoscopy or surgery is required for the definitive diagnosis. The most common symptoms are dysmenorrhea, dyspareunia, and low back pain that worsen during menses. Endometriosis occurring shortly after menarche has been frequently reported. Endometriosis has been described in a few cases at the umbilicus, even without prior history of abdominal surgery. It has been described in various atypical sites such as the fallopian tubes, bowel, liver, thorax, and ev

Physiology and pathology of ovarian hyperstimulation syndrome.

Ovarian hyperstimulation syndrome (OHSS) occurs when ovaries primed with follicle-stimulating hormone/leuteinizing hormone (LH) are subsequently exposed to human chorionic gonadotropin (hCG). The ultimate pathophysiological step underlying this clinical picture is increased vascular permeability (VP). With the administration of hCG, the expression vascular endothelial growth factor (VEGF) and VEGF receptor 2 (VEGFR-2) mRNA increases significantly rising to a maximum coinciding with peaked VP. Immunohistochemistry shows the presence of VEGF and VEGFR-2 proteins in the granulosa-lutein and endot

Vascular pathology in the aged human brain.

Cerebral atherosclerosis (AS), small vessel disease (SVD), and cerebral amyloid angiopathy (CAA) are the most prevalent arterial disorders in the aged brain. Pathogenetically, AS and SVD share similar mechanisms: plasma protein leakage into the vessel wall, accumulation of lipid-containing macrophages, and fibrosis of the vessel wall. CAA, on the other hand, is characterized by the deposition of the amyloid beta-protein in the vessel wall. Despite these differences between CAA, AS and SVD, apolipoprotein E (apoE) is involved in all three disorders. Such a pathogenetic link may explain the corr

[X-ray diagnosis of frontal sinus pathology].

A new variant of X-ray diagnosis for the evaluation of frontal sinus conditions is proposed. The method implies examination of the head positioned so as to have its sagittal plane perpendicular to the table surface, with the central ray directed tangentially to the nasal dorsum and vertically toward the centre of the cassette. Such an arrangement ensures high quality of the image due to the absence of overlapping projections of the adjacent and surrounding facial structures of the skull. Moreover, it eliminates the dependence of visualization results on the age-specific and anatomical cranial
