Research on real-time detection and staging technology for pressure injuries in critically ill patients based on the YOLOv8 deep learning model.

BACKGROUND: Aiming to address the clinical challenges of high incidence rates of pressure injuries (PIs) in critically ill patients and the subjectivity and inefficiency of traditional assessment methods, this study developed a real-time detection and staging technology based on the YOLOv8 deep learning model. This technology enables rapid and objective staging of PIs, aiming to overcome the limitations of current manual assessments and provide support for precision nursing care. OBJECTIVE: To develop and validate a real-time detection and staging system for PIs using the YOLOv8 deep learning

Canine brain imaging with a new low-field portable (0.05 T) MRI scanner: a pilot in vivo comparison to conventional 1.5 T.

Prohibitive costs associated with high-field MRI systems have resulted in reduced access in resource-limited areas with consequential healthcare inequities. This has renewed interest in purposefully designed low-field (LF) systems that, despite inherently lower signal-to-noise ratios, have advantages in terms of cost, portability, and accessibility. This pilot study successfully translated a previously developed LF (0.05 T) MRI canine cadaver brain protocol for in vivo application and compared the acquired images with paired 1.5 T images from 21 different canine patients. The visibility of 15

Risk factors and mechanisms of stroke in young adults: The FUTURE study.

Incidence of ischemic stroke and transient ischemic attack in young adults is rising. However, etiology remains unknown in 30-40% of these patients when current classification systems designed for the elderly are used. Our aim was to identify risk factors according to a pediatric approach, which might lead to both better identification of risk factors and provide a stepping stone for the understanding of disease mechanism, particularly in patients currently classified as "unknown etiology". Risk factors of 656 young stroke patients (aged 18-50) of the FUTURE study were categorized according to

Barriers to hypertension management in a developing country: a qualitative study among patients with hypertension at the National Hospital of Sri Lanka.

BACKGROUND: A rapidly rising prevalence of hypertension (HTN) is observed in low-and-middle-income countries but few studies explore factors such as attitudes and beliefs, experiences and social and cultural conditions which affect its overall management. This study addresses this deficit through a qualitative assessment in adults with HTN from Sri Lanka. METHODS: A phenomenological approach was used to explore patient experiences and perceptions regarding HTN management. Thirty-five adults were purposively sampled from outpatient clinics at the National Hospital of Sri Lanka, Colombo, Sri Lan

Early detection of pediatric health risks using maternal and child health data.

Machine learning (ML)-driven diagnosis systems are particularly relevant in pediatrics given the well-documented impact of early-life health conditions on later-life outcomes. Yet, early identification of diseases and their subsequent impact on length of hospital stay for this age group has so far remained uncharacterized, likely because access to relevant health data is severely limited. Thanks to a confidential data use agreement with the California Department of Health Care Access and Information, we introduce Ped-BERT: a state-of-the-art deep learning model that accurately predicts the lik

Profile and quality of life of the adult population in good health according to the level of vitality: European NHWS cross sectional analysis.

BACKGROUND: The World Health Organization's definition of health highlights the importance of mental and physical wellbeing and not only disease state. However, lack of awareness on the burden of impaired vitality and its impact on the quality of life of the general healthy population prevents healthcare providers from delivering appropriate solutions and advice. This study aims to better characterize this population in Europe and identify the profile and the health reported outcomes associated with impaired vitality. METHODS: This retrospective observational study included National Health an

Approaches to optimising access to NICE-approved biologic anti-TNFs for patients with rheumatoid arthritis with moderately active disease.

BACKGROUND: Rheumatoid arthritis (RA) is a chronic inflammatory disease that is associated with joint pain and stiffness. Biologics represent some of the most effective treatments for RA, but previous guidance from the National Institute for Health and Care Excellence (NICE) has limited their use to patients with severely active disease. This has meant patients with moderately active RA have been treated as if they have an acceptable disease state, despite many cases where the inflammation has a major impact on joint damage, mobility, pain and quality of life. However, recent guideline changes

Living with ongoing whiplash associated disorders: a qualitative study of individual perceptions and experiences.

BACKGROUND: Whiplash associated disorders (WAD) are the most common non-hospitalised injury resulting from a motor vehicle crash. Approximately 50% of individuals with WAD experience on-going pain and disability. Results from intervention trials for individuals with chronic WAD are equivocal and optimal treatment continues to be a challenge. It may be that traditional quantitative measures included in treatment trials have not captured the full benefits patients experience through participation in an intervention. The aim of the present study was to explore participant subjective experiences a

Integrating Diagnostic Approaches in Infant Bacterial Meningitis Caused by a Non-K1 Escherichia coli: A Case Report.

BACKGROUND: Infant meningitis, particularly caused by Escherichia coli, remains a life-threatening condition, especially in premature and low-weight infants. Infections of the central nervous system can be fatal, necessitating prompt diagnosis and appropriate treatment. Acute infections caused by various pathogens, including E. coli, often present with similar clinical symptoms. The rapid identification of pathogens and their antimicrobial resistance mechanisms is critical for timely and effective treatment. We report the case of an 8-month-old patient who presented with fever, diarrhea, and c

A rapid molecular assay for the detection of hypervirulent Klebsiella pneumoniae in the context of antimicrobial resistance surveillance.

Hypervirulent Klebsiella pneumoniae (hvKP) represents an emerging clinical and public-health concern, particularly as hypervirulence increasingly converges with multidrug resistance. Current diagnostic approaches rely on phenotypic assays, such as the string test, or on whole-genome sequencing (WGS), both of which have limitations in specificity, turnaround time, standardization, and feasibility for routine surveillance. To address this gap, we developed a multiplex real-time PCR assay targeting key hvKP-associated virulence loci, including siderophore systems, hypermucoviscosity regulators, a

[Clear cell adenocarcinoma of the corpus uteri].

Specimens obtained from 15 patients suffering from a rare form of carcinoma of corpus uteri--a clear cell adenocarcinoma--were studied. The importance of identification of this form of cancer as a specific nosologic unit was shown, since the 5-year survival rate in patients with this lesion (57.2 +/- 16.4%) was lower than that in patients with endometrial adenocarcinoma, involvement in stage I tumor patients and scheme of treatment being identical. The clear cell adenocarcinoma was shown to occur in more elderly patients as compared with adenocarcinoma of the endometrium and its symptoms are n

Next generation sequencing identifies a pathogenic mutation of WFS1 gene in a Moroccan family with Wolfram syndrome: a case report.

BACKGROUND: Wolfram syndrome is a rare autosomal recessive neurodegenerative disorder that affects 1/200,000 to 1/1,000,000 children. It is characterized by juvenile onset diabetes, optic nerve atrophy and other systemic manifestations. Symptoms of the disease arise mostly in early childhood with a high mortality rate due to severe neurological complications. Two causative genes have been identifed in this syndrome; the classical form is caused by autosomal recessive mutations of the WFS1 gene, and a smaller portion of patients has mutations in the CIDS2 gene, which are responsible for autosom

Scanning Electron Microscope Examination as an Alternative to Classical Microbiology in the Diagnostics of Catheter-Related Sepsis?

Central venous catheters are essential elements enabling the treatment of intensive care unit patients. However, these catheters are sometimes colonised by both bacteria and fungi, and thus, they may become a potential source of systemic infections-catheter-related bloodstream infections (CRBSI). The identification of the pathogen responsible for CRBSI is a time-consuming process. At the same time, the relationship between the quick identification of the pathogen and the implementation of targeted antibiotic therapy is of key importance for controlling the clinical symptoms of sepsis and septi

Brain auditory evoked potentials in pediatric Intensive Care Unit: diagnostic role on encephalopathy and central respiratory failure on infants.

BACKGROUND: Encephalopathy of different etiologies in infants is often the reason for central respiratory insufficiency which eventually leads patients to the Pediatric Intensive Care Unit. Magnetic resonance imaging (MRI) and brainstem auditory evoked potentials (BAEPs) may be useful to identify brainstem alterations among patients with respiratory insufficiency of central origin. MRI is a compulsory technique to identify brain abnormalities, but may fail to detect brainstem lesions of small dimensions. BAEPs play a highly sensitive role on brainstem dysfunction identification because of the

Emerging role of myostatin and its inhibition in the setting of chronic kidney disease.

The past two decades have witnessed tremendous progress in our understanding of the mechanisms underlying wasting and cachexia in chronic kidney disease (CKD) and in other chronic illnesses, such as cancer and heart failure. In all these conditions wasting is an effect of the activation of protein degradation in muscle, a response that increases the risk of morbidity and mortality. Major recent advances in our knowledge on how CKD and inflammation affect cellular signaling include the identification of the myostatin (MSTN)/activin system, and its related transcriptional program that promotes p
