Brain switch for reflex micturition control detected by FMRI in rats.

The functions of the lower urinary tract are controlled by complex pathways in the brain that act like switching circuits to voluntarily or reflexly shift the activity of various pelvic organs (bladder, urethra, urethral sphincter, and pelvic floor muscles) from urine storage to micturition. In this study, functional magnetic resonance imaging (fMRI) was used to visualize the brain switching circuits controlling reflex micturition in anesthetized rats. The fMRI images confirmed the hypothesis based on previous neuroanatomical and neurophysiological studies that the brain stem switch for reflex…

The pupillary light reflex pathway: cytoarchitectonic probabilistic maps in hemianopic patients.

OBJECTIVE: The anatomy of the human pupillary light reflex (PLR) pathway is a matter of debate. The aim of this study was twofold: namely, to investigate the association of a relative afferent pupillary defect (RAPD) in acquired suprageniculate lesions with the location and extent of the cerebral lesions. Further, we suggest a new strategy of lesion analysis by combining established techniques with the stereotaxic probabilistic cytoarchitectonic atlas developed by the Jülich group. METHODS: Twenty-three patients with homonymous visual field defects participated in this study. The RAPD was qua…

Effect of tiotropium on cough reflex sensitivity in acute viral cough.

STUDY OBJECTIVES: Cough is the most common complaint for which patients in the United States seek medical attention. Few, if any, effective therapies exist for the most common form of acute cough, that due to viral upper respiratory tract infection (URI). The aim of this study was to evaluate the effect of the anticholinergic agent tiotropium bromide on cough reflex sensitivity in subjects with acute viral URI. PATIENTS: Otherwise healthy adult nonsmokers with acute viral URI were randomized to receive inhaled tiotropium, 18 microg once daily, or matched placebo, for 7 days. A control group o…

Muscle stiffness and spinal stretch reflex sensitivity in the triceps surae.

CONTEXT: Greater musculotendinous stiffness may enhance spinal stretch reflex sensitivity by improving mechanical coupling of the muscle spindle and the stretch stimulus. This heightened sensitivity would correspond with a shorter latency and higher-amplitude reflex response, potentially enhancing joint stability. OBJECTIVE: To compare spinal stretch reflex latency and amplitude across groups that differed in musculotendinous stiffness. DESIGN: Static group comparisons. SETTING: Research laboratory. PATIENTS OR OTHER PARTICIPANTS: Forty physically active individuals (20 men, 20 women). IN…

A stereotypic "elbowing" movement, a possible new primitive reflex in newborns.

The primitive reflexes are brainstem-mediated and play various roles in the child's psychomotor development. The objective of the current study is to describe a new pattern of primitive reflex, noticed in 52 of 81 randomly chosen newborns and young infants during pressing of the subcostal region. Some of them reacted by three-phase stereotypic movement as follows: phase 1: quick adduction of upper arm with flexion of the forearm, with elbow directed toward the site of stimuli, touching the stimulated area; phase 2: abduction and retroflexion of upper arm with the movement of removing the stimu…

[Photic sneeze reflex or autosomal dominant compelling helio-ophthalmic outburst syndrome].

Sneeze is an ubiquitous phenomenon that happens to everyone. In spite of this, little attention has been paid to it, among medical literature in general, and even less in neurologic texts. A curious entity, called photic sneeze reflex, solar sneeze reflex, light sneeze reflex or autosomal dominant compelling helio-ophthalmic outburst syndrome, known perhaps since ancient Greek, has been scarcely described in the scientific literature, mainly as clinical notes and letters to the editor, but in a detailed way, we can find just a few reports. This reflex appears when subjects are exposed suddenly…

Prediction of auditory brainstem reflex screening referrals in high-risk infants.

OBJECTIVES/HYPOTHESIS: The objectives of this study were to 1) quantify the relative importance of established risk factors for congenital hearing loss (HL), 2) identify other risk factors for congenital HL, and 3) create a prognostic system that can predict the chance of an infant having HL. METHODS: The authors conducted a retrospective cohort study with validation of a prognostic system. The authors used a medical record review on 1,863 infants admitted to level II and level III nurseries who underwent auditory brainstem reflex (ABR) hearing screening from 1998 to 1999 (derivation cohort).…

High prevalence of vasomotor reflex impairment in newly diagnosed leprosy patients.

BACKGROUND: Initial nerve damage in leprosy occurs in small myelinated and unmyelinated nerve fibers. Early detection of leprosy in the peripheral nervous system is challenging as extensive nerve damage may take place before clinical signs of leprosy become apparent. PATIENTS AND METHODS: In order to determine the prevalence of, and factors associated with, peripheral autonomic nerve dysfunction in newly diagnosed leprosy patients, 76 Brazilian patients were evaluated prior to treatment. Skin vasomotor reflex was tested by means of laser Doppler velocimetry. Blood perfusion and reflex vasocon…

Forced mouth opening reaction: a primitive reflex released from cortical inhibition.

We report the case of a 6-year-old girl with congenital adrenal hyperplasia, who showed 'forced mouth opening reaction' during the course of acute encephalopathy due to adrenal crisis. When an object was moved towards her mouth, or when the corner of her mouth was stroked with a tongue depressor, she would immediately open her mouth fully and hold it open. This reaction appeared transiently during the course of her illness in association with other frontal release signs including the rooting, groping and palmomental reflexes. Magnetic resonance imaging showed bilateral widespread lesions invol…

Ablation of cerebellar nuclei prevents H-reflex down-conditioning in rats.

While studies of cerebellar involvement in learning and memory have described plasticity within the cerebellum, its role in acquisition of plasticity elsewhere in the CNS is largely unexplored. This study set out to determine whether the cerebellum is needed for acquisition of the spinal cord plasticity that underlies operantly conditioned decrease in the H-reflex, the electrical analog of the spinal stretch reflex. Rats in which the cerebellar output nuclei dentate and interpositus (DIN) had been ablated were exposed for 50 d to the H-reflex down-conditioning protocol. DIN ablation, which in …

Near reflex substituting for acquired horizontal gaze palsy: a case report.

BACKGROUND: Some patients with acquired horizontal gaze palsy overcome the adduction palsy by utilizing convergence. This substitution phenomenon is very rare. We report a patient with horizontal gaze palsy who was able to use convergence to compensate for the lack of adduction in the left eye. CASE: The patient was a 31-year-old woman with an arteriovenous malformation in the fourth ventricle. She suffered right gaze palsy and right abducens palsy after tumor surgery and radiation therapy. OBSERVATIONS: Three years after the vascular accident, she was found to be able to adduct the left eye

Recovery of the high-acceleration vestibulo-ocular reflex after vestibular neuritis.

Vestibular neuritis (VN) usually leads to a sudden gain asymmetry of the high-acceleration horizontal vestibulo-ocular reflex (VOR). We asked whether this asymmetry decreases over time indicating peripheral recovery and/or central compensation. The horizontal VOR during rapid rotational head impulses to both sides was recorded with search coils in 37 patients at different time periods (1-240 weeks) after the onset of VN. In ten patients, sequential measurements were performed. Gains of the VOR during head impulses toward the ipsilesional side significantly increased after the initial drop (ave…

Dynamic pelvic-pudendal reflex plasticity mediated by glutamate in anesthetized rats.

Reflex plasticity between pelvic afferent nerve (PAN) and pudendal efferent nerve (PEN), as well as external-urethral sphincter (EUS) activity was examined in anesthetized rats. A progressive increase in the number of evoked action potentials per stimulus occurred in PEN and EUS activity when PAN was repetitively stimulated (1 Hz). This potentiation in pelvic-pudendal reflex (PPR) activity induced by repetitive stimulation was abolished by APV (D-2-amino-5-phosphonoraleric acid, i.t. 100 microM, 2-5 microl) and attenuated by the NBQX (2, 3-dihydroxy-6-nitro-7-sulfamoyl-benzo (F) quinoxaline, i…
