[Case of acute ischemic stroke due to cardiac myxoma treated by intravenous thrombolysis and endovascular therapy].

A 48-year-old woman with no previous neurological diseases was transferred to our hospital because of sudden-onset unconsciousness. On arrival, she showed consciousness disturbance (E1V1M3 on the Glasgow Coma Scale), tetraplegia, right conjugate deviation and bilateral pathological reflexes. These symptoms resulted in a NIH stroke scale score of 32. Brain diffusion-weighted MR imaging (DWI) showed multiple hyper-intense lesions, and MR angiography revealed occlusions of the basilar artery (BA) and superior branch of the right middle cerebral artery (MCA). Transthoracic echocardiography disclos

Deviation of the subjective vertical in long-standing unilateral vestibular loss.

We evaluated changes in the subjectively perceived gravitational vertical as an index of imbalance in the function of the right and left otolith organs. In addition to normal subjects (n = 25), we measured patients with a longstanding (mean 4.5 year +/- 3.2 SD; range 0.5-11.5 years) unilateral vestibular loss after surgery for acoustic neuroma (n = 32), patients with partial unilateral vestibular loss (n = 7) and patients with bilateral vestibular hyporeflexia (n = 8). Normal subjects could accurately align a vertical luminous bar to the gravitational vertical in an otherwise completely dark r

A rare case of recurrent spinal hydatid cyst in a 17-year-old man with neurological deficits and balance impairment.

BACKGROUND: Hydatid cysts are caused by the larval stage of the tapeworm parasite Echinococcus granulosus, leading to a rare but significant zoonotic infection. This disease is primarily observed in regions with prevalent agricultural and livestock practices. While the liver, lungs, and brain are most affected, spinal involvement is notably rare. Hydatid cyst disease develops gradually and is usually asymptomatic in the early stages. Symptoms usually emerge when the cysts enlarge sufficiently to cause compression. Effective treatment typically combines surgical intervention with antiparasitic

Hearing Impairment and Audiological Alterations in Euthyroid Hashimoto's Thyroiditis.

INTRODUCTION: Hearing loss may be associated with autoimmune diseases, but it was less studied in Hashimoto's thyroiditis (HT). We aimed to evaluate hearing impairment and audiological alterations in adults with euthyroid HT. METHODS: Adult patients with euthyroid HT (normal thyroid functions, positive antithyroid peroxidase (anti-TPO)/anti-thyroglobulin, and sonographic findings) were compared with controls. We excluded pregnant or older patients (>40 years), those with a history of otological/audiological disease or surgery, otitis media, acoustic trauma, chronic illnesses, use of alcohol,

COVID-19 Recombinant mRNA Vaccines and Serious Ocular Inflammatory Side Effects: Real or Coincidence?

PURPOSE: To report two cases; bilateral arteritic anterior ischemic optic neuropathy (AAION) and bilateral acute zonal occult outer retinopathy (AZOOR) after COVID-19 mRNA vaccination. CASE REPORT: The first patient was a 79-year-old female was presented to us 35 days after a sudden bilateral loss of vision, which occurred two days after receiving the second recombinant mRNA vaccine (Pfizer) injection. Temporal artery biopsy was compatible with AAION. At presentation, the best-corrected visual acuity was 20/1250 and 20/40 in the right and left eyes on the Snellen acuity chart, respectively. T

An unruptured posterior communicating artery aneurysm ruptured during angiography: A case report.

INTRODUCTION: Cerebrovascular imaging is the gold standard for diagnosis of intracranial aneurysms. Rupture of intracranial aneurysm is rare in cerebrovascular angiography, especially in unruptured intracranial aneurysm. PATIENT CONCERNS: A 74-year-old woman was admitted to the hospital for sudden onset of left eyelid ptosis for 1 day with no obvious inducement. The patient had a history of hypertension. Physical examination revealed that she had clear consciousness and normal speech, but the left eyelid drooped. The left pupil diameter was 5 mm and light reflex was absent. The left eyeball c

Inter-hemispheric remapping between arm proprioception and vision of the hand is disrupted by single pulse TMS on the left parietal cortex.

Parietal cortical areas are involved in sensori-motor transformations for their respective contralateral hemifield/body. When arms of the subjects are crossed while their gaze is fixed straight ahead, vision of the hand is processed by the hemisphere ipsilateral to the arm position and proprioception of the arm by the contralateral hemisphere. It induces interhemispheric transfer and remapping. Our objective was to investigate whether a single pulse TMS applied to the left parietal cortical area would disturb interhemispheric remapping in a similar case, and would increase a simple reaction ti

[An adult Guillain-Barré syndrome patient with enhancement of anterior roots on spinal MRI and severe radicular pain relieved by intravenous methylprednisolone pulse therapy: A case report].

A 37-year-old man was admitted to our hospital because of progressive abnormal gait and severe pain in the low back and lower extremities, more severe on the right side, especially in his right posterior thigh. The pain appeared one week after he suffered from slight fever and diarrhea. On admission, a neurological examination revealed mild distal weakness of the all extremities and loss of Achilles tendon reflex, associated with positive Lasègue sign. However, sensation was intact except for slightly impaired vibratory sensation on the trunk. An electrophysiological study showed diminished am

[Potentiation of metaphit-induced audiogenic epilepsy with N-methyl-D-aspartate in rats].

INTRODUCTION: Audiogenic seizures (AGS) are induced by high intensity sound stimulation in genetically susceptible rats or in animals subjected to chemical or electrical manipulation. Epileptic seizure may result from an impaired balance between excitation and inhibition in the CNS. The effect of NMDA (N-methyl-D-aspartic acid) on metaphit 1-(1(3-isothiocyanatophenyl-ciclohexyl)-piperidine) induced audiogenic seizures was evaluated in rats. METHODS: Male Wistar albino rats were divided into 4 groups: 1) saline; 2) metaphit (10 mg/kg); 3) metaphit + NMDA; 4) NMDA (70 mg/kg). Animals were injec

Maintenance of atrial fibrillation in anesthetized and unanesthetized sheep using cholinergic drive.

Atrial fibrillation (AF) was induced electrically and the duration of AF was measured in six isoflurane-anesthetized sheep (weight range 54.5-72.7 kg), and in five unanesthetized sheep (weight range 60-75 kg). In the anesthetized sheep, AF was induced by direct electrical stimulation of the right atrium with a catheter electrode and the duration of AF was determined. Intravenous neostigmine (10 micrograms/kg IV) was administered and the duration of AF was again measured. Then cholinergic drive was increased by bilateral electrical vagal stimulation; AF was induced and the duration of AF was me

[The value of blue light reflectance analysis in the multimodal diagnosis of macular telangiectasia].

Differentiating idiopathic macular telangiectasia (MacTel type 1) from secondary macular telangiectasias remains challenging, and available diagnostic methods such as blue light reflectance (BLR) analysis are still underused. This study presents the case of a 80-year-old man with a visual acuity of 20/800 in the right eye (OD). Multimodal imaging revealed perifoveolar telangiectasias, diffuse loss of the foveal reflex, and hyporeflective intraretinal cystoid spaces consistent with edema. The patient received 18 intravitreal injections of anti-VEGF agents (aflibercept, faricimab), with improvem

Clinical Reasoning: A 28-Year-Old Man with Subacute-Onset Ataxic Gait.

The diagnostic approach to subacute progressive polyneuropathy can be challenging, particularly when accompanied by systemic features. We present a 28-year-old man with subacute-onset ataxic gait. Four months before his initial evaluation at our center, he was assessed by orthopedics for a poorly differentiated tumor of the right tibia. He also reported intermittent fever and unintentional weight loss. Neurologic examination revealed marked weakness in all four limbs, with deep tendon reflexes absent in the lower extremities. Sensory testing demonstrated a stocking-glove distribution of hypoes

Recurrent Optic Neuritis With MRI-Negative Myelopathy in Myelin Oligodendrocyte Glycoprotein Antibody-Associated Disease (MOGAD).

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is an inflammatory demyelinating disorder that frequently presents with optic neuritis or myelitis, although early diagnostic evaluation may be challenging when neuroimaging is normal despite significant neurological deficits. We describe a 29-year-old woman who initially developed unilateral optic neuritis, followed one month later by sequential contralateral involvement and new sensory disturbances. She subsequently presented with acute bilateral lower-limb weakness, brisk reflexes, and lumbosacral sensory disturbance, a

Intestinal duplication in a miniature Schnauzer dog.

A 9-year-old spayed female 18.8 kg mixed breed boxer dog was referred for evaluation of a 7-month history of difficulty swallowing and prehending food, regurgitation, hypersalivation, and an abnormal dorsiflexion of the tongue. Prior to referral, a barium study was performed, which revealed a mildly dilated esophagus. Treatment with sucralfate, cisapride, and prednisone was initiated. Physical examination revealed bilateral, symmetric atrophy of the temporalis muscles, dorsiflexion of the distal aspect of the tongue with concurrent muscle atrophy, and a reduced gag reflex. Challenges of Large Vessel Vasculitis Stroke Patient with Complicated Endovascular Thrombectomy: A Case Report.

PURPOSE: The causes of acute stroke in patients with SLE are multifactorial. Antiphospholipid-associated hypercoagulability and inflammation-induced platelet activation are major causes of ischemic stroke in SLE patients. As such patients underwent intravenous thrombolysis and endovascular thrombectomy, they may have higher risk of complications and less favorable outcome. CASE REPORT: A 30-year-old woman with underline SLE and Takayasu arteritis who presented with right CCA and MCA occlusion status post rtPA and endovascular thrombectomy. Twelve hours after the procedure, head CT was ordered

The health and environmental risks and rewards of modernity that shape scientific optimism.

Individuals in high-income countries increasingly express less scientific optimism than in lower-income societies. In this article, we utilize risk society theory to understand the complicated relationship between individual- and country-level factors, and optimism toward the role of science in society in "reflexively modern" societies. We use multilevel modeling with 16 high-income countries to determine the individual-level and country-level factors that shape scientific optimism. Next, we look at the individual characteristics that affect scientific optimism in each country individually. At

A link between frontal white matter integrity and dizziness in cerebral small vessel disease.

One in three older people (>60 years) complain of dizziness which often remains unexplained despite specialist assessment. We investigated if dizziness was associated with vascular injury to white matter tracts relevant to balance or vestibular self-motion perception in sporadic cerebral small vessel disease (age-related microangiopathy). We prospectively recruited 38 vestibular clinic patients with idiopathic (unexplained) dizziness and 36 age-matched asymptomatic controls who underwent clinical, cognitive, balance, gait and vestibular assessments, and structural and diffusion brain MRI. Case report of cavernous hemangioma with a 4 year follow up.

Cavernous hemangiomas are vascular malformations that can affect any part of the central nervous system. In general, epidural hemangiomas are secondary extensions of spinal lesions. These tumors grow slowly and are expressed as slow spinal cord compression syndromes, radiculopathy, or both. History, clinical examination, routine radiographs, MRI, and histopathologic studies are aids to a definitive diagnosis. This is a 61-year-old chronic smoker with a history of cholecystectomy in 2017. History of the disease: dates to 1 month by a progressive installation of heaviness of the left lower limb,

Stroke-like attack: first episode of sporadic hemiplegic migraine.

Hemiplegic migraine (HM) is a rare migraine with aura; it can be familiar or sporadic. A 46-years-old man presented left migraine followed by right hemiparesis with bilateral plantar flexion of cutaneous plantar reflex (CPR). Brain CT and CT-angiography were normal. The next day patient got worse. The EEG showed left fronto-temporal cuspidate delta waves and brain MRI showed a minimal hyperintensity at T2-sequences in the left frontal cortex with a minor representation of the cortical veins at susceptibility weighted imaging sequences. After 3 days, he had a progressive neurological improvemen
