Microdebrider cavitation and transcervical removal of parapharyngeal schwannomas approaching the skull base.

Removal of parapharyngeal space (PPS) schwannomas approaching the skull base through a purely transcervical approach requires adequate visualization of the surgical field to obtain complete resection with minimal sequelae. This is a retrospective series of four patients undergoing transcervical removal of sympathetic chain PPS schwannomas abutting the skull base by an intracapsular microdebrider tumor cavitation. Radiologic data, complications, functional outcomes, and follow-up status were considered. MRI was suggestive of PPS schwannoma in all cases, and correctly predicted the nerve of orig

A three-dimensional digital atlas of the dura mater based on human head MRI.

OBJECTIVE: The goal of this paper was to design a three-dimensional (3D) digital dural atlas of the human brain for assisting neurosurgeons during the planning of an operation, medical research and teaching activities in neurosurgical anatomy. METHODS: The 176 sagittal head magnetic resonance(MR) images of a 54-year-old female who suffered from the left posterior fossa tumor were processed and outlined, based on which a 3D dural model was created using the softwares of 3ds-max and Mimics. Then the model and images/anatomy photos were matched using the softwares of Z-brush and Photoshop to for

Surgical treatment of jugular foramen schwannomas.

OBJECTIVE: We present our experience with surgery of jugular foramen schwannomas with special consideration of clinical presentation, surgical technique, complications, and outcomes. METHODS: This retrospective study includes ten patients with jugular foramen schwannomas treated by the senior author between January 2007 and December 2012. Three patients had undergone partial tumour resection elsewhere. The initial symptom for which they sought medical help was hearing loss, dysphagia, hoarseness, and shoulder weakness. Preoperative glossopharyngeal and vagal nerve deficits were the most commo

Relationship between the pituitary stalk angle in prefixed, normal, and postfixed optic chiasmata: an anatomic study with microsurgical application.

BACKGROUND: The relationship between the optic apparatus and the skull base is important during approaches near the sella turcica. One relationship that dictates which approach is taken is whether the optic chiasm is prefixed or postfixed or in a "normal" location, (centered over the diaphragma sella). The relationship between the position of the chiasm and the angulation of the pituitary stalk has not been investigated. METHODS: Forty adult cadavers without intracranial pathology were dissected and parasagitally hemisected lateral to the sella turcica. The angulations between the pre- and po

Endoscopic rhino-neurosurgical approach for non-adenomatous sellar and skull base lesions.

OBJECTIVE: Since endoscopic endonasal transsphenoidal surgery requires skills of both neurosurgeons and otorhinolaryngologists, and the nose is the primary corridor of approach, we favour the term `endoscopic rhino-neurosurgery` and report on our interdisciplinary experience treating non-adenomatous lesions with skull base extension. METHODS: Between 2004 and 2009, 58 patients with 21 different disease patterns underwent endoscopic rhino-neurosurgical procedures. Mean age was 39.9 years, 50% were female. Seven had undergone prior surgery. Clinically, 34.5% presented with visual field deficits

Symptomatic syringomyelia occurring as a late complication of posterior fossa medulloblastoma removal in infancy in a boy also suffering from scaphocephaly.

INTRODUCTION: The association of a medulloblastoma and a syringomyelia has been already described in rare instances albeit without symptoms related to the syrinx. CASE REPORT: The case of a 23-year-old man operated in infancy for a medulloblastoma and then treated solely with adjuvant chemotherapy is reported. He was also operated in infancy for a scaphocephaly. With a very long time delay, he has developed a Chiari I and a symptomatic cervico-dorsal syringomyelia. The symptoms attributed to the syrinx consisted of a unilateral prurigo over the left arm which was so severe to lead to self-mut

Combined 6th and 12th Cranial Nerve Palsies (Godtfredsen Syndrome) due to Clival Lesions.

BACKGROUND: Godtfredsen syndrome (GS), originally described by Dr. Erik Godtfredsen in 1946, is the combination of cranial nerve (CN) 6th and 12th palsies. The most common etiology of GS is a lesion of the clivus. METHODS: A retrospective case series of GS is described. RESULTS: Three patients were included: 1) a 38-year-old woman with a clival breast cancer metastasis; 2) a 35-year-old woman with a clival chordoma; and 3) a 72-year-old woman with a partially embolized dural arteriovenous fistula involving the hypoglossal canal. CONCLUSIONS: GS is a combination of CN 6th and 12th palsies an

Composite Chondromucosal Nasoseptal Flap for Reconstruction of Suprasellar Defects.

BACKGROUND: The pedicled nasoseptal flap (NSF) is the mainstay for endoscopic skull base reconstruction. We present a novel technique using a semirigid chondromucosal NSF that improves the reinforcement and protection of intracranial structures. METHODS: Composite NSFs were performed to repair intraoperative high-flow cerebrospinal fluid leaks in 2 patients who had undergone endoscopic endonasal resection of a suprasellar mass. The surgical technique and postoperative outcomes are described. RESULTS: The flaps were sufficient for defect coverage, and the patients did not experience any cereb

Effect of Omega-3 Supplementation in Patients With Smell Dysfunction Following Endoscopic Sellar and Parasellar Tumor Resection: A Multicenter Prospective Randomized Controlled Trial.

BACKGROUND: Endoscopic endonasal approaches pose the potential risk of olfactory loss. Loss of olfaction and potentially taste can be permanent and greatly affect patients' quality of life. Treatments for olfactory loss have had limited success. Omega-3 supplementation may be a therapeutic option with its effect on wound healing and nerve regeneration. OBJECTIVE: To evaluate the impact on olfaction in patients treated with omega-3 supplementation following endoscopic skull base tumor resection. METHODS: In this multi-institutional, prospective, randomized controlled trial, 110 patients with

Craniectomy Versus Craniotomy for Posterior Fossa Metastases: Complication Profile.

OBJECTIVE: Surgical resection of posterior fossa metastases (PFM) includes either suboccipital craniotomy or suboccipital craniectomy. The optimal surgical technique is yet to be defined. We examined the association between the chosen surgical approach and the occurrence of postoperative complications. METHODS: We retrospectively evaluated medical records and imaging characteristics of patients who underwent resection of newly diagnosed PFM between 2003 and 2014 in our medical center to identify covariates that significantly affected postoperative complications. RESULTS: Of 917 patients with

Historical perspective on evolution in management of lateral skull base tumors.

This article gives a history of the major advances that have contributed to the current management of lateral skull base lesions. These advances include changes in surgical technique, better understanding of the natural history of these lesions, and the advent of stereotactic radiosurgery. An understanding of how treatment has evolved over time improves understanding of how the current treatment methods have been developed.

Endoscopic endonasal treatment of a large clival giant cell tumor invading the cavernous sinus and temporal lobe.

Giant cell tumors (GCTs) are extremely rare lesions, representing less than 5% of all bone tumors. They primarily occur in the long bones. However, a small percentage of these tumors occur in the pelvis, spine, or skull bones. These lesions are usually benign as well as locally aggressive and require complete removal. Nowadays, GCT and other skull base lesions can be treated using the extended endoscopic endonasal approach. We present a case report of a GCT located in the skull base, originating from the clivus and sphenoid bone, invading through the cavernous sinus, and treated using the full

Structural pedicled mucochondral-osteal nasoseptal flap: a novel method for orbital floor reconstruction after sinonasal and skull base tumor resection.

Unrepaired orbital floor defects after sinonasal and skull-base tumor resection can lead to herniation of orbital contents into the maxillary or ethmoidal sinuses, possibly resulting in infection and significant cosmetic and functional deficits. Orbital floor defects are usually repaired using prosthetic implants or autogenous material. Nasal septal cartilage has been used previously as a free graft for reconstruction. However, its reliance on local vascular supply can result in ischemia and necrosis in the postoperative period. The vascularized pedicled nasoseptal flap, consisting of mucoperi

Osteomalacia caused by skull base tumors: report of 2 cases.

BACKGROUND AND IMPORTANCE: Tumor-induced osteomalacia (TIO) is an uncommon paraneoplastic syndrome rarely encountered in neurosurgical practice. We report on 2 cases of TIO caused by skull base tumors. Although the diagnosis of TIO is difficult to make and often is delayed because of the insidious nature of the symptoms, mostly systemic pain and weakness, it is curable once it is diagnosed and properly treated. CLINICAL PRESENTATION: Both patients presented with severe pain developing in the lower extremities and moving out to the entire body, as well as difficulty moving. They were diagnosed
