Recurrent dislocation of tibialis posterior tendon. A report of two cases.

We successfully treated two patients with recurrent dislocation of the tibialis posterior tendon by creating a bone block. Sudden resistive contraction of the tibialis posterior muscle is considered to be the mechanical cause of the initial traumatic injury, and a shallow tibialis posterior tendon sulcus may be the predisposing factor. Once the flexor retinaculum is torn during the initial trauma, recurrent dislocation is inevitable, and surgical treatment is mandatory. When treating patients with a complaint of long-standing pain around the medial malleolus, we must bear in mind the possible

[Lesions of zone II flexor tendons: splints and rehabilitation].

Rehabilitation of the hand after flexor tendon injuries in zone 2 is a challenge for both surgeons and therapists. Prevention of adherences is based on early controlled motion programs. Two different and therefore often complementary protocols are presented: 1. Kleinert: an active extension--passive flexion method; 2. Duran: a controlled passive method, as well as the splints required at this early stage of reeducation. The angles of flexion for the wrist and the metacarpal joints are discussed. When the splint is removed, active exercises and activities are proposed according to the interval …

Dorsal dislocation of the lunate with multiple extensor tendon ruptures.

An old dorsal lunate dislocation with associated multiple extensor tendon ruptures is described. Treatment consisted of proximal row carpectomy and transfer of the extensor indicis proprius to the distal stumps of the ruptured extensor tendons to the long, ring, and small fingers.

[The quadriga phenomenon of the extensor tendon system and the superficial transverse metacarpal ligament].

The aponeurosis of the common extensor tendons is connected by intertendinous connections proximal to the metacarpophalangeal joints. These fibers are responsible for tenodesis effects in case of maximal finger extension and flexion and by this means they diminish the function of the other metacarpophalangeal joints. This mechanism can be described as the Quadriga-Syndrome of the extensor apparatus. These connecting structures also help to stabilize the transverse metacarpophalangeal arch and the position of the extensor tendons over the metacarpal heads. Cadaver dissections as well as operati

[Subcutaneous rupture of the long extensor tendon to the thumb].

The "spontaneous" subcutaneous rupture of the tendon of the extensor pollicis longus is reported and the etiology and the pathologie are discussed. In our opinion is the transfer of the extensor indicis the only operation to do in this case and gives the best results.

The magic-angle effect of the supraspinatus tendon.

CONTEXT: Rotator cuff injuries are common and result in either a tear or tendinopathy within the distal portion of the supraspinatus tendon. Magnetic resonance (MR) imaging shows the injury as a high signal region within the distal tendon. Similarly, a magic-angle effect also appears within healthy patients in this same region on T1- or proton density (PD)-weighted MR sequences. OBJECTIVE: This study was conducted to evaluate the prevalence of the magic-angle effect found with MR imaging within the supraspinatus tendon. METHODS: In this study, 300 consecutive patients were selected from thos

Comparison of open, percutaneous, or mini-open repair in the treatment of Achilles tendon ruptures: a systematic review and meta-analysis based on comparison studies.

BACKGROUND: The Achilles tendon rupture is a common injury of the lower extremity, inducing pain and physical impairment. Surgical treatment methods include open, percutaneous, and mini-open repair techniques. This study aims to compare the outcomes of these three techniques. METHODS: PubMed, Google Scholar, and EMBASE were searched until November 2023. Randomized controlled trials (RCT) and retrospective comparative studies (RCS) comparing either open versus percutaneous repair, open versus mini-open repair, or percutaneous vs. mini-open repair of the Achilles tendon rupture were included wi

Wide-awake local anaesthesia no tourniquet (WALANT) vs regional or general anaesthesia for flexor tendon repair in adults: protocol for a systematic review and meta-analysis.

BACKGROUND: Flexor tendon injuries most commonly occur following a penetrating injury to the hand or wrist. These are challenging injuries and the standard treatment is surgical repair under general or regional anaesthesia. 'Wide-awake' surgery is an emerging technique in hand surgery where a conscious patient is operated on under local anaesthetic. The vasoconstrictive effect of adrenaline (epinephrine) creates a 'bloodless' operating field and a tourniquet is not required. The potential advantages include intra-operative testing of the repair; removal of the risks of general anaesthesia; red…

Premature hamstring graft amputation during harvesting in ACL reconstruction.

INTRODUCTION: There are some possible complications during or after hamstring graft harvesting such as premature graft amputation, medial collateral ligament injury and infrapatellar branch of the saphenous nerve injury. Premature graft amputation can occur by inadequate removal of the accessory branches of the hamstring tendons, poor surgical technique and/or too sharp tendon stripper. In this study, we report a case of premature hamstring graft amputation due to degeneration caused by osteochondroma at the posteromedial aspect of the proximal tibia. CASE PRESENTATION: We reported the case o

Distal patellar tendon avulsion in association with high-energy knee trauma: A case series and review of the literature.

BACKGROUND: Patellar tendon rupture is rare in the general population. Typically, failure occurs proximally or at the mid-substance. Distal avulsion from the tibial tubercle in adults is rare and not well described in the orthopedic literature. METHODS: We present the largest series of patients with distal patellar tendon injury with associated multi-ligamentous disruption of the knee. A series of six patients with distal patellar tendon avulsion were identified at a single institution. The cases were reviewed and are presented. RESULTS: Each case of distal patellar tendon rupture was associ

The spectrum of sports-related interphalangeal joint injuries.

Sports participation places the fingers at risk for injury, especially from end-on contact with many kinds of balls and other objects. Certain sports are likely to produce specific injuries. Joint sprains and dislocations are especially common, whether or not accompanied by articular fractures. Tendon ruptures in the digit may not be recognized until many days or weeks later. The athlete must be aware of the need for early, accurate diagnosis, precise treatment, and complete rehabilitation for these often apparently innocuous injuries.

Endoscopic flexor hallucis longus transfer for acute Achilles tendon ruptures is associated with a high re-rupture rate: A prospective case series.

INTRODUCTION: Achilles tendon rupture (ATR) is one of the most common injuries to the musculoskeletal system. Recently, endoscopic FHL transfer has been successfully used to treat acute injuries in soccer players, with good return to play rates and good functional outcomes. The aim of this study is to analyze patient-reported questionnaires, functional capacity assessment (FCA), and re-rupture rates following endoscopic FHL transfer for acute ATR. METHODS: This is a prospective case series with 26 patients (15 men, 11 women, mean age 47.7 years) who suffered an ATR and underwent surgical reco

Morphological Changes and MRI Characteristics of the Achilles Tendon in Amateur Marathon Runners With Different Running Experience.

BACKGROUND: With the escalating popularity of marathon running, Achilles tendon injuries, particularly gradual-onset Achilles tendon injury, have become common, often causing substantial training disruptions. However, the influence of running experience on the Achilles tendon structure in amateur runners remains largely unclear. This study aimed to investigate the association between running experience and asymptomatic Achilles tendon pathology as well as its structural changes. METHODS: This was a cross-sectional observational study. Forty-eight amateur marathon runners were categorized into…

Stiffness of Muscles and Tendons of the Lower Limb of Professional and Semiprofessional Athletes Using Shear Wave Elastography.

OBJECTIVE: Shear wave elastography (SWE) allows assessment of muscle and tendon stiffness and can be used to diagnose soft tissue pathologies such as tendinopathies. In sports medicine, SWE may have the potential to uncover structural changes early on before they lead to functional impairment. To systematically analyze possible differences in tendon and muscle stiffness of the lower limb between professional (PG) and semiprofessional female athletes (SG) using SWE and to compile reference values for developing preventive medicine approaches for professional athletes. METHODS: Standardized SWE
