Detection of vertical root fractures of different thicknesses in endodontically enlarged teeth by cone beam computed tomography versus digital radiography.

INTRODUCTION: This study was performed to compare the accuracy of cone beam computed tomography (CBCT) scans and digital radiography (DR) for detecting simulated vertical root fractures (VRFs) with different thicknesses in extracted human teeth. METHODS: Eighty teeth were endodontically prepared and divided into 4 groups, 3 experimental and 1 control. The teeth in experimental groups were artificially fractured and fixed together with different thicknesses of 0.2 mm (0.2-mm VRF group), 0.4 mm (0.4-mm VRF group), and smaller than 0.2 mm (crack group). Teeth in control group were kept intact. T

Prophylactic root resection and periapical grafting for anterior implant aethetics: an integration of tissue-reconstruction and tissue-preservation concepts.

The presence of active infections involving the labial bony plate of a failing maxillary anterior tooth presents a challenging situation for aesthetic implant replacement. Not only would the septic state of an abscess be detrimental for immediate implant placement and bone augmentation, but the accompanying bony destruction would also increase the likelihood and magnitude of gingival recession, making this a bioaesthetic challenge. This article describes a technique that integrates tissue-reconstruction and tissue-preservation concepts to achieve peri-implant tissue aesthetics as well as its c

Dental erosion in Cuban children associated with excessive consumption of oranges.

Marked erosion at the mesial edges of upper front teeth was observed during an examination of Cuban children. The preferential erosion of mesial edges produced characteristic V-shaped defects on upper central incisors, and the aim of the present study, carried out on 12-yr-old children (N = 1010) in 10 communities in the Province of Havana was to establish the frequency of dental erosion and explain its occurrence. The symmetrical erosion of teeth 11 and 21 (excluding crown injuries and attrition) were clinically classified into four grades: 0.5 = objectionable; 1 = abnormal mesial shortening

A modified surgical/prosthetic approach for optimal single implant supported crown. Part I--The socket seal surgery.

Optimal implant placement can be achieved only if the ridge maintains its dimensions and the quality of bone. To prevent the resorption of the ridge and to enhance the quality of regenerated bone, two main approaches have been suggested. Part I of this article presents a modified regenerative technique-the "socket seal surgery" (SSS). Part II will present a modified prosthetic technique--the "cervical contouring concept" (CCC)--and it will be published in the May, 1994, issue of PP&A. The learning objective of this article is to supplement reader knowledge of methods and techniques for prevent

Effects of cavity designs on fracture behavior of mandibular first premolars with moderately curved C-shaped canal.

This study was to analyze the fracture behavior of lower first premolars with moderately curved C-shaped canal using extended finite element method. After scanning mandibular first premolars with diverse types of moderately curved C-shaped canal with Micro-CT, models of the intact tooth, conservative endodontic cavity (CEC) and traditional endodontic cavity (TEC) were generated. A total load of 600 N was applied vertically to buccal cusp and buccal side of distal fossa for each model. Distributions of maximum principal stress (MPS) and crack evolution of enamel and dentin were evaluated. Large

Finite element analysis of the human mandible to assess the effect of removing an impacted third molar.

BACKGROUND AND AIM: Finite element analysis (FEA) was used to generate 3-dimensional models of a human mandible with impacted third molars. The aim was to analyze the effects of removing various amounts of bone around an impacted mandibular third molar and to predict the possibility of iatrogenic fracture. MATERIALS AND METHODS: Data were acquired from cone beam computed tomography (CBCT) scans of a patient using numerically calculated mechanical parameters. Virtual surgery was then performed on the mandibular models, and standardized chewing forces were applied to the resulting simulations.

Finite element evaluation of the biomechanical feasibility of a multi-shield socket shield technique for a vertically fractured maxillary central incisor.

The socket shield technique (SST) preserves peri-implant tissues during immediate implant placement in teeth with intact roots, yet its biomechanical behavior in the presence of vertical root fractures (VRFs) remains unquantified. This study evaluated the mechanical feasibility of a modified multi-shield SST (MS-SST) designed for single-fracture-line scenarios in maxillary central incisors. Three-dimensional finite element models representing conventional immediate implantation (CII), classic SST (C-SST), and MS-SST were subjected to 45° oblique occlusal loading from 10 to 100 N in 10 N increm

Managing Failed Fragment Reattachment: Influence of Surface Preparation and Chamfer Technique on Fracture Resistance.

BACKGROUND/AIM: Fragment reattachment is a conservative option for anterior crown fractures, yet failure after the initial procedure creates a clinical dilemma. Evidence on effective retreatment strategies is lacking. This study investigated how surface preparation and chamfer design influence fracture resistance and failure patterns in failed fragment reattachments. MATERIALS AND METHODS: Sixty bovine mandibular incisors were sectioned 3 mm below the incisal edge and reattached using a standardized protocol with adhesive and flowable composite. After thermocycling (5000 cycles, 5°C-55°C), ba

Fatigue survival of endodontically treated teeth restored with different fiber-reinforced composite resin post strategies versus universal 2-piece fiber post system: An in vitro study.

STATEMENT OF PROBLEM: Various strategies have been proposed to reduce the cement space of foundation restorations for endodontically treated teeth. However, they may add more operative steps, or the dentist must keep different sizes of drills and posts in stock. A 2-piece universal adjustable post system has been developed to overcome this problem, but whether the system has acceptable fatigue survival performance is unclear. PURPOSE: The purpose of this in vitro study was to evaluate the fatigue survival and stress distribution of endodontically treated teeth without a ferrule and restored w

Cementodentinal tear: a case report with 7-year follow-up.

BACKGROUND: Cemental tears are an uncommon form of root fracture that can lead to rapid localized attachment loss. Typically, the separation of the fragment occurs at the cementodentinal junction. In this report, we present a case of periodontal involvement associated with a cemental tear-like fragment that proved to be a cementodentinal tear. We also review the literature on the clinical presentation of cemental tear associated periodontal lesions. METHODS: A 52-year-old male presented with acute pain on the maxillary right second premolar. Clinically, probing depth of 7 mm and inflammation

In vitro evaluation of effects of ultrasonic root-end preparation on resected root surfaces.

This in vitro study investigated structural alterations in resected roots that had root-end preparations made with a conventional microhead handpiece and ultrasonics at two intensity levels. Root ends were examined with fluorescence confocal microscopy. Serial histologic sections to the 2 mm levels and then at the level of 3 mm and 4 mm from the resected surface were examined. Statistical analysis of the confocal data indicated no significant difference between the groups in the number and length of the fractures. Results of the histologic data indicated that root ends prepared by ultrasonics

Chronic sinusitis of odontogenic origin due to an undiagnosed displaced root fragment in the maxillary sinus and the role of cone beam computed tomography in successful management.

Radiographic imaging is an important criteria in diagnosis, treatment planning, and follow-up of an endodontic treatment. When clinicians encounter with diagnostic difficulties and persistent symptoms, the need for advanced diagnostic aids, especially imaging methods, become imperative and essential. This paper reports a case, in which primary endodontic therapy having failed, the patient had persisting symptoms such as pain, swelling, and draining sinus along with signs and symptoms of maxillary sinusitis. Advanced diagnostic aids such as Cone Beam Computed Tomography was useful in accurate i
