Kidney transplants were first performed in Tunisia in 1986, and transplants soon extended to other organs including the heart, liver, and pancreas. Live-related donor and deceased-donor kidney transplants were both began in the summer of 1986. An organ procurement and transplant law was passed in March 1991, and the National Centre for Advancement of Organ Transplantation was created in 1995. The number of transplantation units has increased to 7 throughout the country, and the yearly transplant number has progressively increased to 139 in 2010, including 20% from deceased kidney donors. Despi…

PURPOSE OF REVIEW: In this review, we discuss the mechanism of action, side-effects, and role of everolimus (EVR) in liver transplant, specifically the most recent de-novo (within 1 month of transplant) and conversion (months to years after transplant) trials in the literature.
RECENT FINDINGS: Everolimus was recently approved by the Food and Drug Administration for use in liver transplantation. Its primary benefit over other immunosuppressive agents is the absence of renal toxicity. De-novo liver recipients receiving EVR with reduced-dose tacrolimus had similar rates of death, graft loss, an

Intestinal transplantation may become necessary in patients with short bowel syndrome (SBS) who fail intestinal rehabilitation. Most children requiring intestinal transplantation (68%) have SBS due to anatomic loss. Intestinal transplantation can occur in isolation or in combination with other organs. Many children will have advanced liver disease at the time of referral and will undergo combined liver-small bowel transplantation. Considerable progress in immunosuppression has led to decreased rates of acute rejection after transplantation and to improved early allograft survival while minimiz

Glaucoma is the leading cause of irreversible vision loss post-keratoplasty and an important cause of graft failure. With newer techniques, such as lamellar, endothelial, and laser-assisted keratoplasty as well as keratoprosthesis gaining popularity, clinicians will need to consider the incidence, risks, evaluation, and management of glaucoma for each type of keratoplasty when determining which type of transplant may be most appropriate. A comprehensive literature search of glaucoma in the setting of corneal transplantation was performed and serves as the basis for this review. Preexisting gla

As the ability to transplant organs and tissues has grown, the demand for these procedures has increased as well--to the point at which it f … ransplant waiting lists in the United States and other nations. In many poor nations, those needing transplants simply die because there is no capacity or a very limited capacity to perform transplants. Although the demand for organs now exceeds the supply, resulting in rationing, the size of waiting lists would quickly expand were there to suddenly be an equ …

The biologics used in transplantation clinical practice include several monoclonal and polyclonal antibodies aimed at specific cellular receptors. The effect of their mechanisms of action includes depleting or blocking specific cell subpopulations, complement system, or removing circulating preformed antibodies and blocking their production. They are used in induction, desensitization ABO-incompatible renal transplantation, rescue therapy of steroid-resistant acute rejection, treatment of posttransplant recurrence of primary disease such as nephrotic syndrome or atypical hemolytic-uremic syndr

Orthotopic liver transplantation is the only definitive treatment for end stage liver failure and the shortage of donor organs severely limits the number of patients receiving transplants. Liver tissue engineering aims to address the donor liver shortage by creating functional tissue constructs to replace a damaged or failing liver. Despite decades of work, various bottoms-up, synthetic biomaterials approaches have failed to produce a functional construct suitable for transplantation. Recently, a new strategy has emerged using whole organ scaffolds as a vehicle for tissue engineering. This tec…

Advancements in transplantation offer promising treatment options for a variety of diseases. Organ donation, an essential part of organ transplantation, may be performed at hospitals even if the facility is not a transplant center; therefore, perioperative nurses should be aware of developments in transplantation and the implications for patient care at their facilities. Considerations for organ transplant protocols include the type of documentation required, the elements of the transplant OR time out, and the use of intraoperative anti-rejection medications. Considerations for organ donation

For most patients with chronic kidney failure, kidney transplantation has the greatest potential for restoring a healthy and productive life. The risk of acute rejection is the highest in the first months after transplantation (induction phase) and diminishes afterwards (maintenance phase). Immunosuppression should be at the highest level in the early period and reduced for long-term therapy. At present, conventional immunosuppressive protocols consist of the triple therapy: a calcineurin inhibitor, an adjunctive agent, corticosteroids. The development of new immunosuppressives drugs is aimed

Liver and kidney transplantations have been performed for almost 50 years and is nowadays a routine procedure for the treatment of terminal liver failure and terminal-stage renal failure. Under given optimal conditions and increasing experience good results can be achieved. Improvements in surgical techniques have led to a decrease in the incidence of surgical complications after transplantation. Nevertheless after liver and kidney transplantation complications can occur and increase the morbidity and mortality. There are a number of possible complications which range from harmless wound heali

Techniques for dental transplantation today are taking better advantage of the potential for healing of the periodontal ligament than in the past and have reduced the risk of ankylosis and root resorption to below 1.5%. The resulting improvement in prognosis has made it possible to more confidently plan procedures that include orthodontic treatment. Transplantations can offer a good end result in certain clinical situations that would otherwise be difficult to manage: ectopic teeth, transpositions, tooth trauma sequellae, advanced decay or advanced periodontitis, implanted hard to treat impact

Graft urethroplasty using free transplants has become a standard procedure in the therapy of complicated urethral strictures. Various types of tissues can be used as graft material and different criteria are important for the suitability of tissues for urethroplasty. It was recognized early on that the prepuce was an easy to harvest tissue with low morbidity and excellent functional results. In this article the suitability of this tissue for functional results will be discussed within the context of the biology of free transplants and the available literature.

Nocardiosis is a rare opportunistic infection caused by Nocardia spp., an aerobic actinomycete, that mainly affects patients with cell-mediated immunity defects, such as transplant recipients. Despite recent progress regarding Nocardia identification and changes in taxonomic assignment, many challenges remain for the diagnosis or management of nocardiosis. This opportunistic infection affects 0.04 to 3.5 % of patients with solid organ or hematopoietic stem cell transplantation, depending on the organ transplanted, cytomegalovirus (CMV) infection, corticosteroids dose and calcineurin inhibitors

HCV-related cirrhosis represents the leading indication for liver transplantation in the Western countries. HCV reinfection after liver transplantation occurs in virtually all patients transplanted for HCV-related liver disease Histological evidence of chronic HCV infection develops in 50 to 90 % of patients by 12 months after liver transplantation, and cirrhosis occurs in about 20 % of patients within 5 years after transplant. Several studies have evaluated host, viral, and transplant-related factors that might be associated with the severity of HCV recurrence. Among host factors, immunosuppr

Transplantation is an established therapy for end-stage diseases of kidney, lung, liver, and heart among others. Osteoporosis and fragility fractures are serious complications of organ transplantation, particularly in the first post-transplant year. Many factors contribute to the pathogenesis of osteoporosis following organ transplantation. This review addresses the mechanisms of bone loss that occurs both in the early and late post-transplant periods, including the contribution of the immunosuppressive agents as well as the specific features to bone loss after kidney, lung, liver, cardiac, an

PURPOSE OF REVIEW: Corneal transplantation surgery has moved from an era of conventional penetrating keratoplasty to selective replacement of the diseased corneal layer with complementary healthy donor corneal tissue. Anterior lamellar transplantation surgeries do not involve replacement of corneal endothelium, consequently eliminating the occurrence of endothelial rejection. Similarly, in diseases affecting the corneal endothelium, selective replacement with a lamellar lenticule bearing healthy endothelium provides better outcomes in terms of ocular surface, lesser astigmatism and quick visua

Renal transplantation is now considered the best treatment to be offered to patients with a stage 5 chronic kidney disease. It produces an improvement in the patients quality of life and more importantly a prolongation of their survival. These good results have created an organ shortage due to the discrepancy between the ever growing demand of patients and the lesser increase in the number of available kidneys to be transplanted. The success of kidney transplantation relies on both the quality of the graft itself and also on the immunosuppressive therapy aiming at the prevention of allograft e

Renal transplantation transforms chronically ill children with end stage renal disease (ESRD) into near normal resulting in improvement in nutrition, growth, neurodevelopment and quality of life, and is the goal of therapy. However, the benefits of transplantation come at a price of life-long treatment with immunosuppressive medications, increased risk of infections and malignancy. Children younger than 10 y of age have the best, and adolescents have the worst 5-y graft survival likely due to non-adherence with medications in the adolescents. Long-term complications include ongoing issues rela
