Shiga toxin (Stx)-producing Escherichia coli (STEC)-induced enteropathic HUS (eHUS) is a major cause of acute kidney injury in children and substantial morbidity and mortality in elderly patients. Systemic intestinal absorption of Stx and rapid uptake, through its glycolipid receptor (Gb3), by small vessel endothelial cells, are essential steps in the pathophysiology of STEC disease. HUS is characterized by intravascular hemolytic anemia, thrombocytopenia and acute kidney injury (AKI) that develop abruptly within a week of onset of STEC diarrhea/colitis. Subtle thrombotic changes, attributed t

ABSTRACT: Trauma-induced coagulopathy (TIC) is a global inflammatory state accompanied by coagulation derangements, acidemia, and hypothermia, which occurs after traumatic injury. It occurs in approximately 25% of severely injured patients, and its incidence is directly related to injury severity. The mechanism of TIC is multifaceted; proposed contributing factors include dysregulation of activated protein C, increased tPA, systemic endothelial activation, decreased fibrinogen, clotting factor consumption, and platelet dysfunction. Effects of TIC include systemic inflammation, coagulation dera

Renal tubular dysgenesis (RTD) is a severe foetal disorder characterised by the absence or poor development of proximal tubules, early onset and persistent anuria (leading to oligohydramnios and the Potter sequence) and ossification defects of the skull. In most cases, early death occurs from pulmonary hypoplasia, anuria and refractory arterial hypotension. RTD may be acquired during foetal development or inherited as an autosomal recessive disease. Inherited RTD is genetically heterogeneous and linked to mutations in the genes encoding the major components of the renin-angiotensin system (RAS

BACKGROUND AND OBJECTIVES: Cemented hip arthroplasty is considered the standard of care for treating both osteoporotic femoral neck fractures and pathologic disease of the proximal femur due to the ability to achieve strong fixation in poor quality bone. There is minimal literature evaluating uncemented arthroplasty for pathologic disease of the proximal femur. This objective of this study is to compare outcomes of patients undergoing cemented and uncemented arthroplasty of the proximal femur for an oncologic indication.
METHODS: Patients who underwent hip arthroplasty procedures in one healt

BACKGROUND: Intramural duodenal haematoma is a rare entity that usually occurs in the context of patients with coagulation disorders. A minimum percentage is related to processes such as pancreatitis and pancreatic tumours.
CLINICAL CASE: The case is presented of a 45 year-old male with a history of chronic pancreatitis secondary to alcoholism. He was seen in the emergency room due to abdominal pain, accompanied by toxic syndrome. The abdominal computed tomography reported increased concentric duodenal wall thickness, in the second and third portion. After oesophageal-gastro-duodenoscopy, he

Anemia is frequent in older adults. The essential mechanisms underlying its pathogenesis and relationship with clinical outcomes have been investigated. Erythroid-stimulating agents, particularly recombinant erythropoietin (EPO), corrected anemia in older adults and improved their clinical outcomes. Notwithstanding, many questions remain unanswered. The aim of this study was to correlate a few components of the comprehensive geriatric assessment, such as frailty index, time to up and go test, and mini mental state examination with hemoglobin levels (Hb) after treatment with EPO and blood trans

BACKGROUND: Five cases of variant Creutzfeldt-Jakob disease (vCJD) infections were attributed to infusion of contaminated blood components, turning to real the interhuman transmissibility of this prion disease from asymptomatic carriers. Preventive policies rely on exclusion from blood donation and benefit of leukoreduction initially implemented against leukotropic viruses. In the absence of available antemortem diagnostic tests, the updated prevalence of silent vCJD infections (1/2000 in the United Kingdom) urges the necessity to enforce blood safety with more efficient active measures able t

BACKGROUND: Congenital cardiac surgery with cardiopulmonary bypass (CPB) alters patients' hemostasis. Viscoelastic testing is a modern technology identifying coagulation abnormalities. A new device, the Quantra-QPlus System (HemoSonics LLC, Charlottesville, VA) has not yet been investigated during congenital cardiac surgery.
AIM: This prospective observational pilot study investigated the correlation of Quantra and TEG-5000 in children undergoing cardiac surgery at Stanford Medicine Children's Health (SMCH).
METHODS: Patients (0-8 years) undergoing cardiac surgery with CPB were included afte

BACKGROUND AND OBJECTIVES: The local prevalence of HPgV-1 has been reported from different countries worldwide, but the global prevalence of HPgV-1 remains unknown. The aim of this systematic review and meta-analysis was to gather data from the literature to estimate the prevalence of HPgV-1 in healthy volunteer blood donors in the world.
MATERIALS AND METHODS: We searched PubMed, EMBASE, Scopus and Google Scholar databases for records up to January 2019 and included studies reporting HPgV-1 virus prevalence amongst healthy volunteer blood donors based on the detection of HPgV-1 RNA.

BACKGROUND: Dengue virus (DENV) is an arbovirus primarily transmitted through mosquito bite; however, DENV transfusion-transmitted infections (TTIs) have been reported and asymptomatic DENV RNA-positive blood donors have been identified in endemic countries. DENV is considered a high-risk pathogen for blood safety. One of the mitigation strategies to prevent arbovirus TTIs is pathogen inactivation. In this study we demonstrate that the amustaline and glutathione (S-303/GSH) treatment previously found effective against Zika virus in red blood cells (RBCs) is also effective in inactivating DENV.

BACKGROUND: In 1997 the FDA approved the first double-red-blood-cell (2RBC) collection device. Soon after, travel deferral for variant Creutzfeldt-Jakob disease (vCJD) risk was adopted. To show the importance of including 2RBCs in predictive models of the blood supply, an existing whole-blood (WB) model was updated to include 2RBC collection and then run to simulate the effect of vCJD deferral on total RBC availability.
STUDY DESIGN AND METHODS: The model simulates donation of allogeneic WB and 2RBCs, with donors stratified into eight age and sex groups. The model was updated with 2003 donati

OBJECTIVES: Blood donor deferral is an essential component of donor selection aimed at ensuring the safety of both donors and recipients. However, temporary deferral may negatively affect subsequent donor return and contribute to loss of potentially eligible donors. The present study aimed to evaluate the pattern of blood donor deferral and assess donor return following structured telephonic follow-up after temporary deferral in a tertiary care blood centre.
METHODS: This prospective observational study was conducted in the Department of Immunohematology and Blood Transfusion at National Inst

Massive obstetric hemorrhage still remains a major cause of maternal mortality and morbidity. The risk factors associated with this pathology must be identified in order to schedule the appropriate delivery with the necessary resources. A case is presented of an iliac artery occlusion with intravascular balloons for suspected placenta accreta during cesarean section. The perioperative treatment, as well as an analysis of the treatment options is described, along with their advantages and disadvantages, from the use of postpartum hemorrhage protocols, blood transfusion and procoagulant factors,

Transfusion rates in coronary artery bypass grafting (CABG) continue to vary substantially, although guidelines for allogeneic transfusion have been developed. In order to evaluate ongoing transfusion practices, we performed a multicenter audit in four Danish hospitals regarding the use of allogeneic blood products among patients undergoing first-time CABG. Data on patient characteristics, peri- and postoperative factors were retrieved from 600 patient records (150 records per hospital). Substantial differences were seen regarding preoperative intake of antiplatelet drugs, perioperative use of

BackgroundPlatelet transfusions are used in modern clinical practice to prevent and treat bleeding in people who are thrombocytopenic due to bone marrow failure. Although considerable advances have been made in platelet transfusion therapy in the last 40 years, some areas continue to provoke debate, especially concerning the use of prophylactic platelet transfusions for the prevention of thrombocytopenic bleeding. This is an update of a Cochrane review first published in 2004, and updated in 2012 that addressed four separate questions: prophylactic versus therapeutic-only platelet transfusion p

Two new total hip replacement (THR) procedures have been termed minimally invasive. It is claimed that these smaller incision techniques reduce soft-tissue trauma, decrease postoperative pain and blood loss, speed recovery, and reduce the length of the hospital stay compared with hip replacement using the standard incision technique. These new procedures use either one small 6- to 10-cm incision through a posterior, lateral, or anterior approach or two very small (a 2.5 cm posterior incision for placement of the femoral component and a 5-cm anterior incision for placement of the acetabular com

The authors hypothesize that particularly severely compromised and asphyctic term infants in need of resuscitation may benefit from delayed umbilical cord clamping (after several minutes). Although evidence is sparse, the underlying pathophysiological mechanisms support this assumption. For this review the authors have analyzed the available research. Based on these data they conclude that it may be unfavorable to immediately clamp the cord of asphyctic newborns (e.g., after shoulder dystocia) although recommended in current guidelines to provide quick neonatological support. Liver failure is a critical condition secondary to not only hepatopathy but also extrahepatic etiologies. This article presents a unique case of hyperthyroidism-induced liver failure complicated with Wolff-Parkinson-White syndrome and pneumonia. The 53-year-old male patient had a history of hepatitis B but no prior hyperthyroidism or medication use. He was admitted with jaundice, fever, and palpitations, and underwent one session of therapeutic plasma exchange in the intensive care unit. Other treatments included hepatoprotective agents, blood component transfusions, antithyroid drugs, antiarr…

Tumor lysis syndrome (TLS) is an oncologic emergency caused by release of intracellular tumor components due to massive tumor lysis and is rare in patients with hepatocellular carcinoma (HCC). We describe a case of TLS with rupture of HCC induced by lenvatinib in a patient with advanced HCC. A 72-year-old man who presented with right upper abdominal pain was diagnosed as having advanced HCC with a high tumor burden by contrast-enhanced computed tomography and percutaneous hepatic tumor biopsy. He was started on lenvatinib 12 mg once daily when his tumor progressed despite one-shot hepatic arte
