From Seizure to Cerebral Actinomycosis: A Deceptive Case and an Unexpected Diagnosis Behind a Pseudotumoral Lesion.

Cerebral actinomycosis is a rare bacterial infection of the central nervous system, often under-recognized due to its non-specific clinical … case highlights the importance of including cerebral actinomycosis in the differential diagnosis of brain abscesses and pseudotumoral lesions, even in patients without obvious risk factors. Diagnosis relies on histopathological examination, and management requires a combination of surgical intervention and prolonged antibiotic therapy. Clinical vigilance is cru …

Hypervirulent Klebsiella pneumoniae infection presenting as endocarditis and liver abscess.

Hypervirulent Klebsiella pneumoniae infection, reported commonly from South-east Asia, is predominantly community-acquired and affects young healthy adults. Although abscesses of liver, brain and muscles, endophthalmitis or osteomyelitis have been reported, Infective endocarditis is a rare manifestation. This report illustrates a patient with uncontrolled diabetes mellitus who presented with clinical features of liver abscess with an incidental finding of infective endocarditis. Hypervirulent K. pneumoniae, which was isolated from blood culture of the patient carried the plasmid borne key viru

[Experience of transnasal endoscopic drainage of rhinosinusogenic frontal lobe abscess].

UNLABELLED: Intracranial complications in inflammatory diseases of the ear and nose are currently not frequent, but their lethality remains high. The choice of optimal, safe and effective access in surgical treatment of purulent-inflammatory intracranial complications remains a subject of discussions and is based on an individual approach, depends on the volume, localization and clinical condition of the patient. OBJECTIVE: To demonstrate a clinical case of successful drainage of the brain frontal lobe abscess using endoscopic transnasal access under the control of the navigation system. CON

Orbital Perivenous Abscess Complicating the Diagnosis and Management of Orbital Cellulitis.

An 11-year-old female presented with orbital cellulitis, bacterial sinusitis, enlarged left superior ophthalmic vein, dural venous sinuses, and internal jugular vein. The patient underwent endoscopic sinus surgery and was started on intravenous antibiotics and anticoagulation with limited improvement in orbital signs and symptoms. A magnetic resonance imaging/magnetic resonance venography of the orbits and brain revealed a dilated left superior ophthalmic vein with absence of flow but no clearly discernible orbital abscess. Intravenous corticosteroids resulted in dramatic improvement of pain,

Disseminated tuberculosis presenting as mesenteric and cerebral abscess in HIV infection: case report.

Disseminated tuberculosis in HIV infection involves multiple organs. Pulmonary and lymph node involvement are the commonest form of tuberculosis in HIV infection [1, 2]. Other forms of tuberculosis in the absence of lung and lymph node involvement are rare. Various forms of abdominal [3, 4] and neurological [5, 6] tubercular involvement in HIV infection have been reported. But tuberculosis presenting simultaneously with mesenteric and brain abscess has not been reported yet. We report a case of disseminated tuberculosis presenting as mesenteric and cerebral abscess in a HIV case without involv…

Diagnosis and treatment of intracranial complications of paranasal sinus infections.

Complications and local extension of paranasal sinus infections most often involve the orbit and periorbita. Because of the widespread use of antibiotics since World War II, intracranial extension of maxillofacial sinusitis is rarely seen today. Nevertheless, the clinician must be aware of the potential for these complications, because late recognition of this condition and delays in treatment can increase morbidity and mortality rates. A comprehensive, current review of sinogenic intracranial complications is presented, with illustrative cases of brain abscess, subdural empyema, meningitis, c…

Frontal sinus ablation by cranialization. Report of 21 cases.

Twenty-one patients who had a cranialization of the frontal sinus following massive penetrating wounds were reviewed; 75% had injuries to other areas, 50% were admitted in shock, 42% were comatose, and 25% had a tracheostomy. Death ensued within two weeks of surgery in 25%. Postoperatively, four patients had a CSF leak from elsewhere in the cranial floor, but no patients suffered from meningitis or brain abscess. All leaks stopped spontaneously. One patient had a small wound abscess and in another a subcutaneous aerocele formed. The remaining patients healed uneventfully. Of the patients follo…

Rapidly growing tumor-like brain lesion.

Listeria monocytogenes accounts for 8-11% of the cases of bacterial meningitis which is associated with high mortality in patients with serious underlying diseases or those receiving immunosuppressive treatment. Brain abscess due to L. monocytogenes is a very rare occurrence. The case reported here concerns a 54-year-old female patient with a rapidly growing tumor-like brain lesion. L. monocytogenes type 4b could be cultured from blood and brain biopsy. Despite antimicrobial therapy with ampicillin and gentamicin, the patient died 11 days after admission to the hospital. The growing numbers of…

Is the brain biopsy obligatory or not for the diagnosis of Schilder's disease? Review of the literature.

BACKGROUND: Schilder's myelinoclastic diffuse sclerosis (Schilder's disease) is a rare demyelinating disorder. Clinical features and neuroimaging findings of this disease might mimic an intra-cranial mass lesion including an abscess or a tumour. CASE REPORT: Clinical and radiological findings of two children with the diagnosis of Shilder's disease are reported, and the role of brain biopsy as a diagnostic tool is discussed.

Meningoencephalitis with septic intracerebral infarction: a new feature of CNS listeriosis.

The broad spectrum of CNS listeriosis is expanded by the observation of an intracerebral infarction associated with meningoencephalitis. Sequential CCT scans in a 66-year-old immuno-compromised woman led to the clinical diagnosis of brain abscess. Autopsy revealed an infected intracerebral infarction most probably due to temporal occlusion of the middle cerebral artery by septic microemboli or septic endothelial damage.

Tumefactive multiple sclerosis.

Tumefactive multiple sclerosis (MS) is a subtype of atypical and rare MS that presents with tumor-like lesions in the central nervous system. The lesions may demonstrate a mass effect, edema, with ring enhancement. They can be mistaken for brain tumors or brain abscesses radiologically and clinically. Here we describe an instructive case of a 55-year-old woman with tumefactive MS who presented with occasional numbness in her right arm and leg, headache, thought confusion, and blurred vision for 2 years.

Orbital and intracranial complications after acute rhinosinusitis.

BACKGROUND/OBJECTIVES: Nowadays, intracranial abscess is a rare complication of acute rhinosinusitis. The consequent orbital and intracranial complications of acute rhinosinusitis are rare but must be mutually excluded in complicated rhinosinusitis even when proper surgical and medical treatment tend to efficiently heal the orbital complication. METHODS: We report a case of a patient who primarily revealed symptoms of orbitocellulitis as a complication of odontogenous rhinosinusitis. Proper diagnostic and therapeutical measures were undertaken to manage the disease immediately after stationar
