Intraarticular injection of sodium hyaluronate plus steroid versus steroid in adhesive capsulitis of the shoulder.

To determine the efficacy of a combined treatment--namely hyaluronan and corticosteroid injection plus physical exercises in the management of established idiopathic capsulitis of the shoulder--30 consecutive subjects with adhesive capsulitis were selected for the study. The diagnosis of adhesive capsulitis was established on the basis of a clinical history of spontaneous shoulder pain, shoulder examination showing passive limitation in conformity with capsular pattern, cervical examination excluding significant dysfunction of this area, plain radiographs excluding other significant shoulder d

Cystic lesions around the knee joint: MR imaging findings.

A wide variety of cystic lesions may be encountered in the soft tissues and bones during routine MR imaging of the knee. These lesions represent encapsulated fluid collections and exhibit low signal intensity on T1-weighted images and high signal intensity on T2-weighted images because of their high content of free water. Many lesions produce clinical features suggestive of internal derangement of the knee, so correct MR diagnosis is needed to avoid unnecessary arthroscopy and to guide specific therapy. The purpose of this essay is to illustrate the MR appearance of cystic lesions that can be

Exercise and the aging foot.

Overall physical fitness has become increasingly popular in the 1990s, especially in the geriatric community. More runners, bikers, swimmers, and aerobic advocates over the age of 55 are requiring medical attention for foot complaints, as the feet are a necessary tool for almost any athletic activity. This paper discusses the most common foot disorders in active geriatric patients and outlines a variety of conservative and surgical approaches to management. The goal is early return to pain-free activity.

Hemiplegic shoulder pain.

This article reviews the literature relevant to the possible causes, prevention, and treatment of hemiplegic shoulder pain. Shoulder pain and stiffness impede the rehabilitation of patients with hemiplegia. The cause of this complication is unknown, but it may be related to the severity of neurological deficits, preexisting or posthemiplegic soft tissue injury, subluxation, brachial plexus injury, or shoulder-hand syndrome. Shoulder pain may be preventable if risk factors can be identified and appropriate prophylaxis applied. Resolution of the condition depends on diagnosis and effective treat

External oblique fasciocutaneous flap for elbow coverage.

The external oblique fasciocutaneous pedicle flap can be used for reconstruction of soft-tissue defects of the elbow. This flap has been used in five patients, and results have been good. The technique is appropriate in patients with recurrent defects of the elbow in whom local tissue has been previously used and is no longer available. With the development of local fasciocutaneous units, this method may have limited application. However, because of the relationship of this flap to the elbow, the procedure can be done easily and rapidly with minimal immobilization. It is a clinical impression

Clinical Phenotypes in Frozen Shoulder Based on Psychological and Sensory Phenotypes: Associations With Pain and Disability Over Time.

OBJECTIVES: This study aimed to identify phenotypes of people with frozen shoulder (FS) based on baseline measures, including pain intensity, disability, pain-related psychological factors, and measures of central pain processing. A secondary objective was to evaluate whether these baseline phenotypes were associated with distinct longitudinal trajectories of pain intensity and disability over 9 months. METHODS: People with FS were assessed at baseline for clinical (pain intensity and disability) and psychophysical variables (pain catastrophizing and hypervigilance, self-reported symptoms rel

Platelet-rich plasma inhibits ferroptosis and inflammation to alleviate frozen shoulder via activating the CST1/GPX4 signaling pathway.

Platelet-rich plasma (PRP) has been shown to be beneficial to frozen shoulder (FS), but the mechanism of PRP's intervention in FS is still incomplete. Ferroptosis and inflammation are important pathological factors of cartilage injury, but their role in FS has not been explored. In vivo, we found that PRP treatment significantly enhanced the joint range of motion and mitigated joint histopathological damage in FS rats. Notably, levels of iron ions, the ferroptosis marker prostaglandin-endoperoxide synthase 2 (PTGS2), reactive oxygen species (ROS), malondialdehyde (MDA), and pro-inflammatory cy

Upper Extremity Surgeon's Guide to the Evaluation of the Shoulder Girdle and Diagnosis of Associated Pathology.

Shoulder injuries can pose as a diagnostic challenge for clinicians. Owing to the wide variety of pathologies and the fact that they frequently coexist, diagnosis can be difficult. Accurate diagnosis and management not only require a thorough understanding of each condition but the ability to narrow down the broad differential through detailed physical examination. This review article provides an in-depth examination of common shoulder conditions and outlines key physical examination techniques for upper extremity surgeons and other clinicians who manage these conditions. It begins by providin

Patient experiences of hydrodistension as a treatment for frozen shoulder: A longitudinal qualitative study.

BACKGROUND: Frozen shoulder is a condition associated with severe shoulder pain and loss of function impacting on a persons' physical and mental health. Hydrodistension treatment that has been widely adopted within the UK National Health Service for the condition. However, evidence of clinical effectiveness and understanding of the patient experiences of this treatment are lacking. This study explored the experiences of people with a frozen shoulder who received hydrodistension treatment. METHODS: A qualitative design with repeat semi-structured interviews was used to explore participants' ex

Inertial Measurement Unit-Based Frozen Shoulder Identification from Daily Shoulder Tasks Using Machine Learning Approaches.

Frozen shoulder (FS) is a common shoulder condition accompanied by shoulder pain and a loss of shoulder range of motion (ROM). The typical clinical assessment tools such as questionnaires and ROM measurement are susceptible to subjectivity and individual bias. To provide an objective evaluation for clinical assessment, this study proposes an inertial measurement unit (IMU)-based identification system to automatically identify shoulder tasks whether performed by healthy subjects or FS patients. Two groups of features (time-domain statistical features and kinematic features), seven machine learn
