A new technique to correct facial paralysis.

In two cases of unilateral ficial paralysis, the advanced atrophy of the muscles forced us to try a new technique for dynamin reanimation. To our knowledge, this has not yet been reported in the literaturel. The technique combines a free nerve graft across the face with a free muscle graft or a muscle flap. The result in both our cases was good.

High-Pressure Injection Injuries to the Face: An Unusual Case, Review of the Literature, and Proposed Management Algorithm.

High-pressure injection injuries to the hand are well-described, but similar injuries to the face are far less common and sporadically reported. Due to the complexity of facial anatomy and benign initial clinical presentation, these injuries can be underappreciated, leading to a delay in treatment and undesirable outcomes. In this paper, the authors present an unusual case of a pressurized diesel fuel injection injury to the face via an entry wound at the base of the nare. The caustic fuel caused widespread liquefactive necrosis of the subcutaneous fat of the face, orbit, and neck, requiring s

Endoscopic repair of anterior table frontal sinus fractures.

Frontal sinus fractures account for 5 to 15% of all maxillofacial injuries. The majority of these fractures are the result of high-velocity injuries such as motor vehicle accidents, assaults, and sporting events. The treatment algorithm for complex frontal sinus fractures is controversial due to the associated risks of brain injury, meningitis, cerebrospinal fluid fistula, and mucocele formation. However, mild to moderately displaced anterior table fractures carry a relatively low risk of long-term morbidity and are generally treated as aesthetic deformities. Unfortunately, the coronal approac…

Reconstruction and rehabilitation of short-range, high-velocity gunshot injury to the lower face: a case report.

War injuries can range from the most minor to the devastating and life-threatening. Multidisciplinary care is required for successful management of survivors. In the acute phase, care may involve emergency surgeons, anaesthetists, neurosurgeons, ophthalmic surgeons, vascular surgeons and ENT specialists in addition to the oral and maxillofacial surgeon. Afterwards, definitive treatment of facial hard and soft tissue gunshot injuries depends ultimately on the abilities and skills of the oral and maxillofacial surgeon and his appreciation of such injuries. The timing and sequence of the surgical

Excision of a Lipoma Between the Parotid Gland and Masseter Muscle via the High Perimandibular Approach: A Case Report.

Subcutaneous tumors in the cheek region present a surgical challenge due to the complex anatomy involving the facial nerve and parotid duct. We report the case of a 72-year-old male with a soft, mobile mass in the left subcutaneous cheek. Magnetic resonance imaging revealed a well-defined 3 cm lipomatous lesion situated between the parotid gland and the masseter muscle. The mass was hyperintense on T1- and T2-weighted images and hypointense on fat-suppressed T2-weighted imaging, consistent with a lipoma. The tumor was successfully excised en bloc using the high perimandibular approach, which i

Improving documentation of visual acuity in patients suffering facial fractures.

Failure to identify eye injuries associated with facial fractures can lead to life-altering morbidity. Oral and maxillofacial surgery teams receiving referrals of patients with these injuries have a vital role in ensuring that visual acuity (VA) is recorded at the time of presentation. We present a clinical audit of documentation of VA in 126 patients who sustained orbital floor and zygoma fractures. Our intervention involved a focussed teaching session for trainees responsible for taking such referrals. VA was appropriately documented in 16.5% before the session and 57.1% afterwards. This stu

[Tracheal rupture in a patient with head and thoracic injuries].

A 49-year-old patient presented with cranioencephalic injuries including subdural hematoma and severe mass effect, facial and thoracic injuries with bilateral rib fractures but no parenchymatous lesions or extrapulmonary air. After surgical evacuation of the subdural hematoma, the patient entered the intensive care unit. Twenty-four hours later the patient pulled out the tubes, which were reinserted after signs of respiratory failure appeared. A series of clinical and radiological signs then followed, namely subcutaneous cervicothoracic emphysema, mediastinitis accompanied by hyperinsufflated

Cervical Branch Retrograde Superficial Parotidectomy for Tail of Parotid Lesions.

Facial nerve dysfunction following superficial parotidectomy is one of the most well-known and dreaded complications of the procedure, leading to significant postoperative impairments in affected patients. In lesions involving the parotid tail, the marginal mandibular branch is at particular risk. In contrast, injury to the cervical branch is usually of minimal consequence. Classically, facial nerve dissection in parotidectomy is performed anterograde from the main trunk. In patients presenting with benign superficial parotid tail lesions, however, we often begin with the identification of the

The dehiscent facial nerve canal.

Accidental injury to the facial nerve where the bony canal defects are present may result with facial nerve dysfunction during otological surgery. Therefore, it is critical to know the incidence and the type of facial nerve dehiscences in the presence of normal development of the facial canal. The aim of this study is to review the site and the type of such bony defects in 144 patients operated for facial paralysis, myringoplasty, stapedotomy, middle ear exploration for sudden hearing loss, and so forth, other than chronic suppurative otitis media with or without cholesteatoma, middle ear tumo

An anatomical study on the facial nerve trunk in fetus cadavers.

BACKGROUND/AIM: Facial nerve paralysis is the most worrying parotid surgery complication. Little attention has been paid to the facial nerve trunk, especially in children. Pathological variations, tumors, and anatomic variations of the facial nerve may cause parotid and facial nerve surgery difficulties. This study describes the facial nerve trunk. MATERIALS AND METHODS: We measured the facial nerve trunk from 8 female and 8 male fetus cadavers aged 21.0 to 35.5 gestational weeks. The locations and positions of the trunks were described. The length of the facial nerve trunk (K) was measured a

Diagnosis and management of nasal fractures.

Nasal fractures are very common in facial trauma but should not be considered minor injuries until they have been thoroughly assessed. Trauma care providers need to have an understanding of common mechanisms of injury, normal anatomy, and assessment techniques to make the diagnosis. Management of the injury will depend on whether the fracture is nondisplaced, displaced, open, or associated with a complication that necessitates a more aggressive approach.

Arthrogryposis, ectodermal dysplasia and other anomalies in two sisters.

Ectodermal dysplasia with arthrogryposis is an uncommon condition. We describe two daughters of a distant consanguineous couple with oligodentia, enamel abnormalities, camptodactyly, longitudinally broken nails, growth retardation, joint contractures with amyotrophy, hypohidrotic skin with tendency to excessive bruising and scarring after injuries and scratching, kypho-scoliosis, mild facial dysmorphia and microcephaly. The condition is probably due to an autosomal recessive gene, the parents being gypsies of the same ancestral origin.

Image-Guided Surgery in Complex Skull Base and Facial Fractures: Initial Experience on the Role of Intra-Operative Computer Tomography.

Surgery of fractures involving the skull base and the facial skeleton often presents challenges that should be addressed to prevent secondary brain injuries (i.e., cerebro-spinal fluid leak), preserve visual functioning, and guarantee a good esthetic result. Complex craniofacial reconstruction can be aided by navigation and pre-operative planning. In recent years, computerized planning of surgical reconstruction drastically increased the safety and efficacy of surgery, but the impact of intraoperative high quality image devices such as an intraoperative computed tomography (CT) scan has not be

Outcome and complications after treatment of facial fractures at different times in polytrauma patients.

Finding the correct point of time for operative treatment of facial fractures in multiply injured people remains one of the most important challenges in modern emergency medicine. Findings relating to pathophysiological mechanisms after severe trauma argue against the early operative treatment of non-life-threatening injuries. Our retrospective analysis investigated the effects on complications and outcome of different time points for operative treatment of maxillofacial fractures in multiply injured patients. Over a period of 10 years (2003-2012) we could identify 1543 patients, of whom 553 h

Sutureless repair of the facial nerve using biodegradable glass fabric.

OBJECTIVES/HYPOTHESIS: To compare a sutureless method of facial nerve repair using a biodegradable glass fabric with the standard method of microsurgical suture. STUDY DESIGN: The facial nerve was transected in groups of six sheep and repaired by either entubulation with a biodegradable glass fabric or standard microsurgical epineurial suture repair. METHODS: Both methods of repair were compared with each other and with a normal control group using electrophysiological and morphometric analysis. RESULTS: Maximum conduction velocity, axon and fiber diameter, and myelin-sheath thickness were

[Fractures of the petrous bone. An evaluation of 123 cases].

Out of 2,888 blunt head injuries that were treated at the Hermann Hospital Trauma Center of the University of Texas at Houston, only 123 temporal bone fractures were diagnosed in 110 patients. The clinical diagnosis was confirmed with high resolution computerized tomography of the temporal bones. Three main categories of fractures were identified: oblique in 74%, longitudinal in 13% and transverse in 13%. The clinical manifestations included hemotympanum in 83%, cerebrospinal fluid otorrhea in 21% and Battle's sign in 10.6%. One third of the patients had conductive hearing loss, 1/2 had mixed
