[Ulnar parametacarpal flap. Experience with 10 clinical cases].

The parametacarpal ulnar flap is raised from the dorsal and ulnar aspect of the hand. It is vascularized by the distal division of the dorsal branch of the ulnar artery. The skin flap is innervated by the dorsal sensory branch of the ulnar nerve helpfull for the reconstruction of skin loss of the palm. At the level of the metacarpo-phalangeal joint of the fifth finger, the medial division is connected by a constant anastomosis with the fifth ulnar collateral artery. This anatomic disposition held the flap to be raised in two manners: as direct flow useful to cover the defects of the palmar asp

Clinical and experimental studies on ulnar ray deficiency.

Thirteen cases with ulnar ray deficiency and thirty-three cases with radial ray deficiency were analysed. The differences of these two anomalies are not only affected parts, but also clinical features. The severity of absence of the fingers in ulnar deficiency was classified into five types. The degree of ulnar arrest was closely related to the severity of deficiency of the fingers and also to abnormalities of the elbow joint. Ulnar deficiency was induced by myleran in rat fetuses. The features of ulnar ray deficiency in rats were the same as in humans. The critical period of ulnar deficiency

Cross-finger subdermal pocketplasty as a salvage procedure for thumb tip replantation without vascular anastomosis: a case report.

Objectives Fingertip amputation is often encountered in emergency departments, especially in hospitals located near industrial areas. Replantation of the fingertip can be considered when the normal architecture is preserved in cases of sharp amputation. The goal of replantation is to preserve cosmesis and function, especially for the thumb because of its involvement in grasping and the key pinch. Even when microsurgical vascular anastomosis is applied, the absence of venous anastomosis along with the high rate of failure of arterial anastomosis in zone 1A fingertip amputation may lead to repla

[Surgical treatment of chronic mallet finger by shortening--suture of the tendon scar. Sixty six cases].

Acute mallet fingers are commonly treated by splinting. Treatment of chronic injuries is more debated. Since 1989, a "shortening and suture" technique have been used for such chronic injuries on the elongated tendon scar. Sixty six of 77 patients treated on a 10 years period were reviewed with a mean follow-up of 21 months. The mean active extension lag at the distal interphalangeal (DIP) joint was 4.5 degrees (41 degrees of improvement) with 52% of fingers which recovered a full extension, representing 77% of good and excellent results according to Abouna's and Brown's modified criteria. Long-term results of extensor tendon repair.

A retrospective analysis was done in 62 patients with 101 digits having extensor tendon injury. Quality of outcome and parameters that might influence outcome were evaluated. The majority of patients were treated with conventional static splinting. Sixty percent of all fingers sustained an associated injury (fracture, dislocation, joint capsule or flexor tendon damage). Patients without associated injuries achieved 64% good/excellent results, and total active motion of 212 degrees. This difference was statistically significant (p less than 0.05). Distal zones (1 to 4) had a significantly poore

Modified Distraction Lengthening Via a Cross-Nail Bed Approach for Aesthetic Reconstruction of Traumatic Fingertip Defects.

OBJECTIVE: To report the clinical effect of distraction lengthening through a cross-nail bed incision for the correction of traumatic nail deformity. METHODS: A retrospective case series of patients with traumatic short nail deformity who underwent distraction lengthening of the distal phalanx from March 2020 to March 2025 was conducted. A transverse osteotomy was made at the mid-portion of the residual distal phalanx. One screw was inserted on both the distal and proximal sides of the osteotomy line, followed by placement of a mini external fixator. Gradual distraction was performed postoper

Delayed diagnosis of psoriatic arthritis presenting with infection-like finger inflammation: A case report.

Psoriatic arthritis is a chronic inflammatory disease associated with psoriasis, and its diagnosis can be challenging owing to nonspecific symptoms, absence of reliable biomarkers, and occasional delay in skin manifestations. Herein, we report a case of psoriatic arthritis that initially presented as an acute finger inflammation mimicking infection. A 46-year-old woman developed sudden swelling and pain in the left ring finger during chemotherapy for cervical cancer. Based on the results of the physical examination, laboratory tests, and magnetic resonance imaging, pyogenic flexor tenosynoviti

Swan neck deformities in rheumatoid arthritis: a qualitative study on the patients' perspectives on hand function problems and finger splints.

OBJECTIVE: To identify hand function problems and the reasons for choosing a specific finger splint in patients with rheumatoid arthritis (RA) and swan neck deformities. METHODS: A qualitative study was performed alongside a randomized, controlled cross-over trial comparing the effectiveness of two types of finger splints (the silver ring splint [SRS] and the prefabricated thermoplastic splint [PTS]) in 50 patients with RA and swan neck deformities. Questions on the patients' main hand function problem and reasons for choosing a specific splint type were performed at baseline and after using

The Effect of Forearm Shortening on Finger Flexion: A Biomechanical Study.

PURPOSE: Surgeons may shorten the forearm for many indications. We quantified the impact of shortening on finger flexion with a cadaver model. METHODS: Ten fresh cadaver proximal forearms were pinned to a static block. We pinned each distal forearm/hand to a block that could unlock, slide, and relock on a mounting track. This block allowed wrist-neutral or 30-degree extension. With the sliding block locked, we removed the central 10 cm of the radius/ulna. We placed sutures in the proximal end of each flexor digitorum profundus (FDP). After pretensioning, we simulated near-maximum baseline FDP

Redefining Zone II: Anatomy of the Flexor Digitorum Superficialis Insertion.

BACKGROUND: Flexor zone II is defined as the region spanning the proximal aspect of the A1 pulley to the insertion of the flexor digitorum superficialis (FDS) tendon. Descriptions of the FDS insertion are inconsistent in the literature, but zones of injury are frequently determined with reference to superficial landmarks. The purpose of this study was to describe the footprint of the FDS insertion and define its relationship to the proximal interphalangeal (PIP) skin crease. METHODS: The FDS insertion on the index, middle, ring, and small fingers was dissected in 6 matched pairs of fresh-froz

Dorsal surgical approaches to the proximal interphalangeal joint: a comparative anatomic study.

PURPOSE: Adequate exposure of the articular surface of the head of the proximal phalanx is essential for reduction of intra-articular fractures of the proximal interphalangeal (PIP) joint. We compared the articular exposure obtained by a dorsal extensor-tendon splitting (Swanson), an extensor tendon-reflecting (Chamay), and an extensor mechanism-sparing approach. METHODS: The PIP joints of 24 digits from 6 fresh-frozen cadaveric specimens were randomly assigned to 1 of 3 dorsal surgical exposures: an extensor tendon-splitting, extensor tendon-reflecting, or extensor mechanism-sparing approach

Early controlled passive motion regime for zone II extensor tendon injury--a case report.

Zone II extensor tendon injuries are often associated with sharp laceration and saw injuries. These injuries are usually been immobilized for 4 to 6 weeks with static splint followed with gradual increment in ROM at distal interphalangeal (DIP) joint. Zone II injuries are usually associated with stiff DIP joint. Any effects taken to improve the DIP joint flexion aggravates the extensor lag at DIP joint. We report a dynamic passive mobilization regime for treating zone II extensor tendon injuries. We found dynamic passive mobilization program was effective in managing these injuries in preventi

Camptodactyly: a unifying theory and approach to surgical treatment.

Camptodactyly is an isolated congenital flexion deformity of the proximal interphalangeal (PIP) joint. Surgical experience with 16 patients (18 fingers) between June 1983 and December 1994 is reported. Skin, fascia (retinaculum cutis), tendon sheaths, flexor digitorum superficialis tendon, lumbricals and interossei (particularly the lateral bands), joint surfaces, neck of the proximal phalanx, and central slip insertion were involved in all cases, although the degree of involvement can vary. Surgery must address all of these structures. Postoperative splinting is important. Fifteen patients ha

Irregularities at the base of the proximal phalanges: false indicator of early rheumatoid arthritis.

Fifty posteroanterior hand radiographs from normal adults were reviewed retrospectively to assess the incidence of cortical indistinctness at the base of the proximal phalanges in a normal population. This radiographic change, reported to be an early indicator of rheumatoid arthritis, was seen in at least one site in 10% of the films reviewed. Focal areas of cortical erosion or indistinctness at the bases of the proximal phalanges are not considered to be a reliable sign of early rheumatoid arthritis in the absence of juxtaarticular osteoporosis or changes in the metacarpal heads.

Treatment of an Extruded Middle Phalanx Epiphysis in a 6-Year-Old Boy: A Case Report.

CASE: A Salter-Harris II fracture of the small finger middle phalanx with complete extrusion of the epiphyseal fragment in a 6-year-old boy is described. Closed reduction was unsuccessful. Open reduction was performed, the epiphysis was reduced, and a single Kirschner wire was placed obliquely across the fracture and joint. CONCLUSION: This is an extremely rare injury. Open reduction and pinning was successful in preserving functional range of motion. Despite eventual growth arrest, there was no clinically significant angular deformity, and the joint was preserved. Surgical technique and earl

A novel way of treating mallet finger injuries.

Standard treatment protocols following a mallet finger injury involve lengthy periods of immobilization in an effort to ensure the terminal extensor tendon is able to maintain the distal interphalangeal joint in extension. This author describes a technique that utilizes a combination of an orthosis and kinesiotape, thereby creating a treatment protocol that shortens the immobilization phase for these patients. - Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.
