OBJECTIVES: Bidirectional cavopulmonary shunt (BCPS) has played an important role in the staged Fontan approach; however, the timing remains controversial, especially in younger patients. Therefore, we examined the outcomes of BCPS in infants younger than 3 months of age.

In this article, we present a case of a desaturated Fontan patient with an infra-diaphragmatic venous collateral to the pulmonary vein, which was too tortuous to attempt closure at the source. A trans-septal approach was successfully used to close the collateral in a retrograde manner.

Different patch materials have been utilized in repairing ventricular septal defect (VSD) with great success. In this report, in addition to the right ventricular outflow tract reconstruction, the VSD was repaired successfully in all cases by fashioning a patch from a segment of the Contegra xenograft conduit. The freedom from infection, thromboembolism, and reintervention during follow-up, in addition to the advantage of ready availability and cost-effectiveness offered by using the same bovine material, imply that Contegra xenograft is a promising alternative patch material for VSD repair.

INTRODUCTION: Hypoplastic left heart syndrome (HLHS) is the main cause of mortality due to congenital heart disea se. The Norwood surgery is the first procedure of the surgical staging process towards a single ventri cle physiology or Fontan-type operation and has a mortality rate of 10% to 30%. Extubation failure during the postoperative period occurs in up to 18% of these patients and is associated with increased mortality.
