Late hydrocephalus in a case of wandering bullet into the pineal region.

We report a patient in whom a bullet in the brain migrated into the pineal region causing hydrocephalus 3 months later. In patients undergoing surgical removal of intracerebral or intraventricular bullets, it is recommended to obtain an x-ray or CT scan on the day of operation. Late hydrocephalus may occur several months after migration of the bullet due to scar tissue.

Delayed suture intravesical migration as a complication of a Stamey endoscopic bladder neck suspension.

We report our experience with two cases of late migration of the suture and bolster occurring 2 years after a modified Stamey endoscopic bladder neck suspension. Delayed migration of the suture and bolster after an endoscopic bladder neck suspension across tissue planes, with subsequent erosion into the bladder, is uncommon. Recurrent urinary tract infection and mild suprapubic discomfort were the only symptoms. Cystoscopy was the only helpful diagnostic tool and should be considered early in the evaluation of this kind of patients. The mechanism of migration of the cuff and the operative tech

Scleral fixation technique using 2 corneal tunnels for a dislocated intraocular lens.

Several techniques are used to reposition dislocated intraocular lenses (IOLs). Most place a suture loop around the end of the haptic. However, in cases of a dislocated capsular bag containing the IOL, a dislocated IOL with a large haptic, or a miotic pupil, it is not easy to see the haptic ends to place the suture loop. We describe a scleral fixation technique that uses 2 corneal tunnels. A double-armed 10-0 polypropylene suture loop can be introduced through 1 corneal tunnel and placed around any accessible part of the haptic with the help of a bent 26 gauge needle. This modified technique i

Gastrojejunal fistula after insertion of percutaneous endoscopic gastrostomy.

The authors report the case of a 12-year-old boy with cystic fibrosis, in whom a percutaneous endoscopic gastrostomy device migrated into the jejunum, forming a gastrojejunal fistula.

Proximal ureteral stent migration: an avoidable complication?

PURPOSE: The parameters responsible for proximal ureteral stent migration were quantitated. MATERIALS AND METHODS: We compared 16 patients seen between 1990 and 1993 with stent migration to 32 patients whose stents did not migrate. RESULTS: The stent indwelling time and number of stents with a proximal curl of less than 180 degrees were similar between the groups. More patients with migration had stents that were shorter than the ideal length, with a distal curl of less than 180 degrees and a proximal curl in the upper calix versus the renal pelvis. CONCLUSIONS: A shorter than ideal stent,

Migration of an acupuncture needle into the medulla oblongata.

A case of a delayed lesion of the medulla oblongata caused by migration of an acupuncture needle is presented. The patient was a 60-year-old woman who had undergone embedded-type acupuncture needle treatment around 1975. In 1993 she was admitted to our hospital with a 3-week history of progressive motor and sensory disturbance of her right upper extremity. CT demonstrated that one needle had penetrated the medulla oblongata transversely at the level of the foramen magnum. The needle was removed surgically without any complications. This is the first report of an acupuncture needle migrating in

Transesophageal echocardiography assisted retrieval of embolized inferior vena cava stent.

A 32-year-old male with Budd-Chiari syndrome and stenosis of the inferior vena cava presented to the hospital with severe abdominal pain. Two contiguous, 14 mm, overlapping (joined) Palmaz stents were placed across the area of stenosis in the inferior vena cava. The stents later embolized to the left pulmonary artery and were retrieved non-surgically over a Grollman balloon catheter with transesophageal echocardiographic assistance. This case report highlights the advantages and limitations of transesophageal echocardiographic assistance in such a procedure.

[Multilevel osteotomy with Rush pin fixation in treatment of osteogenesis imperfecta].

Fourteen multilevel osteotomies with Rush pin fixation have been performed in 5 children with osteogenesis imperfecta. The age of patients at the operation ranged from 6.5 to 15.5 years. Plaster of Paris immobilization never exceeded 8 weeks. An axial correction, improved ambulating and equalization of the limbs have been achieved in all cases. Complications of the method included varus deformity of the proximal tibia (1 case), migration of the pin (1 case) and supracondylar fracture of the femur at the level of pin end (2 cases).

Gastric pneumatosis: sign of biliary stent-related perforation.

Gastric pneumatosis developed 4 months after a Carey-Coons biliary endoprosthesis was placed in an 85-year-old woman with pancreatic carcinoma. Radiographs revealed a change in the position of the stent. This combination of findings suggested that more than simple stent migration had occurred. At endoscopy, duodenal perforation was confirmed and the stent repositioned. Subsequently, the pneumatosis resolved.

Duodenal perforation as a delayed complication of placement of a biliary endoprosthesis.

Duodenal perforation may occur at the time of insertion of a biliary endoprosthesis or following endoscopic manipulation of such a stent. The possibility of perforation as a late sequela of stent migration, after percutaneous placement of an endoprosthesis, however, has not been emphasized. In the authors' experience with a 12-F soft-stent endoprosthesis, duodenal perforation occurred in four of 50 patients following initially satisfactory stent placement. One intraperitoneal perforation necessitated immediate surgical intervention. The three retroperitoneal perforations were treated conservat

Retrieval of a fractured PICC fragment following umbilical venous insertion in a preterm infant: a case report.

BACKGROUND: Due to the immaturity of their organs, preterm infants often require central venous access to ensure effective treatment and nutritional support. Although umbilical vein catheterization is a commonly used method, rare but serious complications such as catheter fracture and migration can occur during the procedure, posing significant challenges and high risks to manage. METHODS: This report describes a case of a male preterm infant with a gestational age of 32 weeks who required a peripherally inserted central catheter (PICC device inserted via the umbilical vein as an off-label ap

Bladder outlet obstruction after multiple periurethral polytetrafluoroethylene injections.

Periurethral polytetrafluoroethylene (Teflon) injections have been reported to be successful for the treatment of urinary incontinence after transurethral resection or radical prostatectomy. However, the use of polytetrafluoroethylene is controversial due to reports of distant migration and granulomatous reaction after periurethral injection. We report on a patient with a history of periurethral polytetrafluoroethylene injection for postoperative stress incontinence in whom bladder outlet obstruction developed and who underwent repeat transurethral resection 9 years later. Pathological examina

Cyanoacrylate Glue Masquerading as an Obstructive Calculus: Rare Sequelae of Angioembolization for Renal Pseudoaneurysm.

Renal pseudoaneurysm is a well-known albeit rare vascular complication following renal trauma, percutaneous interventions, renal biopsy, and partial nephrectomy. Angioembolization has become an effective treatment option for pseudoaneurysm using Cyanoacrylate glue, Gel-foams, Micro-coils, polyvinyl alcohol, etc. We herein present a 20-year-old gentleman with infected left hydroureteronephrosis secondary to an impacted foreign body in a ureter, specifically, down-migrated cyanoacrylate glue. This is two weeks following glue angioembolization for a left upper polar segmental renal artery pseudoa

A very persistent chicken bone: two separate perforations from the same foreign object 2 months apart.

A 56-year-old man presented acutely with abdominal pain and raised inflammatory markers. Initial CT images demonstrated acute inflammation in the right upper quadrant surrounding a high-density linear structure. The appearance was of a chicken bone causing a contained small bowel perforation. This was managed conservatively with intravenous antibiotics and the patient was discharged 10 days later. The same patient returned to the hospital 2 months later, once again with an acute abdomen. CT imaging on this occasion showed distal migration of the chicken bone as well as free gas and fluid indic
