Neurodevelopmental Outcomes of Extremely Preterm Infants Randomized to Stress Dose Hydrocortisone.

OBJECTIVE: To compare the effects of stress dose hydrocortisone therapy with placebo on survival without neurodevelopmental impairments in high-risk preterm infants. STUDY DESIGN: We recruited 64 extremely low birth weight (birth weight ≤1000 g) infants between the ages of 10 and 21 postnatal days who were ventilator-dependent and at high-risk for bronchopulmonary dysplasia. Infants were randomized to a tapering 7-day course of stress dose hydrocortisone or saline placebo. The primary outcome at follow-up was a composite of death, cognitive or language delay, cerebral palsy, severe hearing lo

Duration of Exposure to General Endotracheal Anesthesia during Cesarean Deliveries at Term and Perinatal Complications.

OBJECTIVE: To examine whether the duration of time from initiation of general endotracheal anesthesia (GETA) to delivery for cesarean deliveries (CDs) performed is related to perinatal outcomes. STUDY DESIGN: This is a retrospective study of patients with singleton nonanomalous gestations undergoing CD ≥37 weeks of gestation under GETA with reassuring fetal status at a single tertiary care center from 2000 to 2016. Duration from GETA initiation until delivery was calculated as the time interval from GETA induction to delivery (I-D), categorized into tertiles. Outcomes for those in the tertile

Physiotherapy in Primary Ciliary Dyskinesia-What Is the Evidence?: A Narrative Review and Personal Experience.

Primary ciliary dyskinesia (PCD) is a rare, hereditary disorder characterized by impaired motile ciliary function, resulting in abnormal mucociliary clearance (MCC). This leads to persistent infection and inflammation, progressive airway damage, and bronchiectasis. Physiotherapy, particularly airway clearance therapy (ACT), is considered a cornerstone of PCD management to compensate for the mechanical MCC deficit, aiming to reduce infection risk, prevent disease progression, and improve quality of life (QOL). However, evidence supporting specific physiotherapy regimens in PCD remains limited.

Quantitative assessment of chlorine gas inhalation injury based on endoscopic OCT and spectral encoded interferometric microscope imaging with deep learning.

Chlorine exposure can cause severe airway injuries. While the acute effects of chlorine inhalation are well-documented, the structural changes resulting from the post-acute, high-level chlorine exposure remain less understood. Airway sloughing is one of the standards for doctors to evaluate the lung function. Here, we report the application of a high-resolution swept-source optical coherence tomography system to investigate the progression of injury based on airway sloughing evaluation in a chlorine inhalation rabbit model. This system employs a 1.2 mm diameter flexible fiberoptic endoscopic p

Long-term home non-invasive ventilation in chronic hypercapnic chronic obstructive pulmonary disease patients: Real-world impact on lung function, acute exacerbations and survival.

INTRODUCTION: Although long-term home non-invasive ventilation (H-NIV) has been used among chronic hypercapnic COPD patients, its clinical benefit is still on debate. We aim to assess the impact of H-NIV in chronic hypercapnic COPD patients. METHODS: COPD patients who initiated H-NIV between January 2010 and December 2017 were included. Patients with concomitant respiratory disorders were excluded. Acute exacerbation (AE) before and 2 years after H-NIV initiation was assessed as main outcome. Secondary outcomes included lung function and gas exchange parameters. Survival since H-NIV initiatio

Management of a Preterm Infant with Renal Tubular Dysgenesis: A Case Report and Review of the Literature.

Renal tubular dysgenesis (RTD) is the absence or poor development of the renal proximal tubules caused by gene mutations in the renin-angiotensin system. Although RTD has been considered fatal, improving neonatal intensive care management has enhanced survival outcomes. However, little has been reported on the survival of extremely preterm infants. This study reports the survival of an extremely preterm infant with RTD and discusses the appropriate management of RTD by reviewing the literature. A female infant weighing 953 g was delivered at 27 weeks' gestation by Cesarean section because of o

[Medium and long-term respiratory outcome in patients operated from congenital diaphragmatic hernia: From a series of 56 patients].

INTRODUCTION: Recent advances in prenatal diagnosis, high frequency oscillatory ventilation and pulmonary arterial hypertension (PAH) therapies have progressively improved the survival of patients with congenital diaphragmatic hernia (CDH). However, CDH is associated with high morbidity that affects about half of surviving children. The main sequelae observed are respiratory (asthma, persistent PAH [PPAH], bronchopulmonary dysplasia, respiratory infections) and digestive (gastroesophageal reflux disease [GERD]). METHOD: This retrospective study focuses on the medium and long-term respiratory

Risk factors and molecular epidemiology of Acinetobacter baumannii bacteremia in neonates.

BACKGROUND: Acinetobacter baumannii infections in neonates are not uncommon but rarely studied. METHODS: Clinical and molecular epidemiology of 40 patients with A. baumannii bacteremia in the neonatal intensive care units (NICUs) of a medical center from 2004 to 2014 was analyzed. RESULTS: Multi-drug resistance was found in only 3 isolates (7.5%). Sequence types (STs) of A. baumannii defined by multilocus sequencing typing were diverse, and 72.4% identified isolates belonged to novel STs. Majority of the isolates were susceptible to antibiotics tested. Among the 3 imipenem-resistant A. bauma

Impact of Tennis-Specific Hitting and Running Loads on Respiration Patterns Compared to Treadmill Running With Similar Oxygen Uptake.

This study aimed to compare respiratory patterns and gas exchange during tennis and treadmill running at similar oxygen uptake. On three experimental days, 15 elite players (7 female, 8 male) completed a standardized tennis protocol (TP), a treadmill-based incremental running test (RT), and a metabolically matched running protocol (RP). TP (Day 1) included low or high running loads (RL, RH) and low or high stroke velocities (SL, SH) which were combined in four incremental ball-machine stages (TP1:RL + SL; TP2:RL + SH; TP3:RH + SL; TP4:RH + SH). RT (Day 2) determined running speeds matching the

Interactive effects of maternal cigarette smoke, heat stress, hypoxia, and lipopolysaccharide on neonatal cardiorespiratory and cytokine responses.

Maternal cigarette smoke (CS) exposure exhibits a strong epidemiological association with Sudden Infant Death Syndrome, but other environmental stressors, including infection, hyperthermia, and hypoxia, have also been postulated as important risk factors. This study examines whether maternal CS exposure causes maladaptations within homeostatic control networks by influencing the response to lipopolysaccharide, heat stress, and/or hypoxia in neonatal rats. Pregnant dams were exposed to CS or parallel sham treatments daily for the length of gestation. Offspring were studied at postnatal days 6-8

The success of non-invasive mechanical ventilation in lung cancer patients with respiratory failure.

INTRODUCTION AND AIM: Despite the improvement in survival among patients with lung cancer as a result of the development of novel treatment options, acute respiratory failure (ARF), which may occur because of the disease itself, comorbidities or complications in treatment may be life threatening. The most commonly utilised treatment option in cancer patients with ARF is invasive mechanical ventilation (IMV). The prognosis of lung cancer patients admitted to the intensive care unit is poor. The use of non-invasive mechanical ventilation (NIMV) in the setting of ARF not only supports the respira

Inhaled ozone induces distinct alterations in pulmonary function in models of acute and episodic exposure in female mice.

Ozone is an urban air pollutant known to cause lung injury and altered function. Using established models of acute (0.8 ppm, 3 h) and episodic (1.5 ppm, 2 h, 2 times/wk, 6 wk) inhalation exposure, we observed distinct structural changes in the lung; whereas acutely, ozone primarily disrupts the bronchiolar epithelial barrier, episodic exposure causes airway remodeling. Herein we examined how these responses altered pulmonary function. A SCIREQ small animal ventilator was used to assess lung function; impedance was used to conditionally model resistance and elastance. Episodic, but not acute oz

Alveolar capillary dysplasia in a neonate: emphasising the importance of early recognition and biopsy in refractory pulmonary hypertension.

Alveolar capillary dysplasia with misalignment of pulmonary veins (ACD/MPV) is a rare congenital condition characterised by abnormal development of the pulmonary vasculature. It manifests as severe, refractory hypoxaemia and persistent pulmonary hypertension of the newborn (PPHN), often resulting in rapid progression to respiratory failure and early death despite aggressive interventions. Diagnosis relies on distinct histological features. We present the case of a term neonate born to parents with a history of third-degree consanguinity. The mother, a primigravida, delivered a male neonate vag

Type 2 Respiratory Failure as an Initial Manifestation of Lambert-Eaton Myasthenic Syndrome Complicated by Paraneoplastic Autoimmune Encephalitis.

Lambert-Eaton myasthenic syndrome (LEMS) is a paraneoplastic or autoimmune neuromuscular disorder that typically presents with limb weakness or autonomic dysfunction. Here, we report a rare case of LEMS with acute type 2 respiratory failure as the initial symptom. A 79-year-old woman was admitted with acute disturbance of consciousness and type 2 respiratory failure. On admission, she presented with confusion, seizures, and respiratory acidosis requiring non-invasive positive pressure ventilation. Cerebrospinal fluid analysis revealed pleocytosis and elevated protein levels without infection.
