Public-private partnerships for UAV-assisted emergency medical services in aging communities: an evolutionary game approach.

OBJECTIVE: To analyze the current "market failure" in the large-scale deployment of UAV-assisted automated external defibrillator (AED) delivery networks for aging communities, and to explore innovative "Public-Private Partnership" (PPP) business models to overcome the "cold-start" institutional deadlock. METHODS: This study introduces a three-party evolutionary game model comprising community representatives, medical institutions, and UAV operators. By integrating principles of "health economics," we simulated the interactive decision-making processes under bounded rationality. Multi-scenari

Percutaneous laser discectomy guided with stereotactic computer-assisted surgical navigation.

BACKGROUND AND OBJECTIVE: Percutaneous laser discectomy at various wavelengths has been used for minimally invasive surgery of herniated intervertebral discs. Using a high-intensity diode laser at 980-nm wavelength, we aimed to improve the safe insertion of the laser trocar with the aid of a stereotactic computer-assisted surgical navigation system. STUDY DESIGN/MATERIALS AND METHODS: The experiments were performed on ex vivo porcine spines with intact soft tissue. Before laser irradiation, each specimen was imaged by computed tomography (CT) with fiduciary markers. The Digital Imaging and Co

The impact of trauma faculty workforce expansion on advanced practice provider experience: Is there a ripple effect?

BACKGROUND: Appropriate trauma team staffing is essential for managing high patient volumes while supporting provider well-being. Although expanding attending surgeon coverage is a common strategy to address physician workload, the impact of additional faculty and team structure on advanced practice providers is not well defined. This study evaluates the effect of implementing a 2-surgeon trauma staffing model on patient outcomes and advanced practice provider-reported perceptions. METHODS: We performed a retrospective review of adult trauma patients before (January 2018-March 2020) and after

Teleneonatology: a major tool for the future.

Hospitals have, for centuries, maintained a central position in the health care system, providing care for critically ill patients. Despite being a cornerstone of health care delivery, we are witnessing the beginning of a major transformation in their function. There are several forces driving this transformation, including health care costs, shortage of health care professionals, volume of people with chronic diseases, consumerism, health care reform, and hospital errors. The neonatal intensive care unit (NICU) at Utah Valley Regional Medical Center in Provo, Utah, began an aggressive redesig

Community-based active tuberculosis case finding in poor urban settlements of Phnom Penh, Cambodia: a feasible and effective strategy.

BACKGROUND: In light of the limitations of the current case finding strategies and the global urgency to improve tuberculosis (TB) case-detection, a renewed interest in active case finding (ACF) has risen. The WHO calls for more evidence on innovative ways of TB screening, especially from low-income countries, to inform global guideline development. We aimed to assess the feasibility of community-based ACF for TB among the urban poor in Cambodia and determine its impact on case detection, treatment uptake and outcome. METHODS: Between 9/2/2012-31/3/2013 the Sihanouk Hospital Center of HOPE co

Understanding safety threats and resilience supports in the operating room: a mixed-methods protocol study using surgical video analysis and clinician interviews.

INTRODUCTION: Preventable intraoperative adverse events (iAEs) are common despite widespread implementation of surgical quality improvement initiatives. These events often result from the interaction of multiple system-based factors (safety threats, STs) that coalesce to compromise safety. Existing research does not fully capture how STs vary across institutions, and how surgical teams either recover from or anticipate challenges (resilience supports, RSs). Consequently, efforts to design and align interventions are hindered by an incomplete understanding of the system-level contributors to pa

The effectiveness of intravenous (IV) to oral (PO) antibiotic switch (IVOS) interventions in managing community- and hospital-acquired pneumonia-a systematic review.

BACKGROUND: Prolonged intravenous (IV) antibiotic therapy in hospitalized patients with pneumonia increases healthcare costs and hospital length of stay compared to oral (PO) therapy. Antimicrobial stewardship (AMS) interventions promoting timely IV-to-PO antibiotic conversion may reduce healthcare burden without compromising patient outcomes. Optimal design and implementation of such interventions in different healthcare settings, including respiratory medicine remain unclear. OBJECTIVES: The aim of this study was to evaluate the effectiveness of interventions designed to improve IV-to-PO an

Disposition at Equipoise: A Qualitative Study of Emergency Physicians' Decision-Making About Hospitalizing People With Dementia.

BACKGROUND: People with dementia are often hospitalized from the emergency department (ED) for conditions that could potentially be treated in the outpatient setting. In scenarios of clinical equipoise-where either admission or discharge may be appropriate-little is known about how emergency physicians make disposition decisions. METHODS: We conducted a qualitative study with attending emergency physicians at a single academic medical center. Interviews included open-ended questions about disposition decision-making in general as well as structured case scenarios using a think-aloud protocol.
