BACKGROUND: Montgomery salivary bypass tubes (MSBTs) have been used for decades in the treatment of stenoses and fistulae of the hypopharynx and the cervical esophagus. Generally, MSBT use is not associated with serious complications. Only a small number of cases with severe MSBT-associated complications have been reported in the literature.

Six patients (5 women and 1 man, with a median age of 71 years) were operated on for gallstone ileus in the 10-year period 1988-98. The diagnosis was made before operation in 2 patients. The obstructing stones were located in the terminal ileum in 3 patients and in the proximal ileum or jejunum in 3 patients. Choledochoduodenal fistula was present in all patients. Inflammation in ulcerative colitis is concentrated in the mucous membrane. Therefore, increased frequency of perianal and anal lesions is not to be expected and therapy does not differ from that in patients without inflammatory bowel disease. This study concentrates mainly on Crohn's disease and provides an overview on skin disorders, skin tag, fissure in ano, fistulae and abscess, stenosis, incontinence and the management of these conditions. Accidental ingestion of foreign bodies is a common problem in children. Magnetic bead toys are hazardous, having potentially lethal consequences if ingested. The aim of this paper is the analysis of the complications occurred after surgical and radiological procedures, for staging and treatment of malignant lymphomas and testicular carcinoma with an evaluation of the possible efficacy of their surgical treatment. A case of a 55 years-old male with long-term Crohn's disease without response to medical treatment and many intestinal fistula is presented. After the last bowel resection, home parenteral nutrition was started. He presented chronic hepatopathy and pancytopaenia. After 9 months of home parenteral nutrition hepatic function and pancytopaenia began to deteriorate. Bone marrow examination revealed an infiltrate of sea-blue histiocytes. He made unsatisfactory progress and died due to a multiorganic failure.

Multiple magnet ingestion is an unexpected health hazard in children that can lead to significant gastrointestinal morbidity. The magnets are attracted to each other across the bowel wall and this may lead to pressure necrosis, resulting in perforation, fistula formation, and/or intestinal obstruction. We report herein a case of small bowel obstruction following ingestion of two magnets. The public and clinicians should be aware of the health hazard of such devices.

Diverticulosis is found in approximately 5% of people living in western industrialized countries. 95% of these are without any symptoms and do not need any therapy. For prophylaxis of diverticulitis a diet containing non-digestable cellulose and swelling substances is recommended. Complications of diverticulosis like diverticulitis, perforation abscess, or stenosis can necessitate a resection of the sigmoid colon. This should be done before life-threatening complications like free perforation or massive bleeding occur.

Colovesical and rectovesical fistulas are relatively rare, and there is no consensus on the gold standard for diagnostic workup and treatment. Here we retrospectively analyzed clinical courses from four patients with colovesical or rectovesical fistulas diagnosed at our hospital between January 2024 and December 2024. We report a case of intraductal papillary mucinous tumor of the pancreas (IPMT) degenerated and fistulized into the stomach, the duodenum and the colon. Diagnoses of malignancy and fistulization were proven preoperatively. Pancreaticoduodenectomy extended to the body was performed. Pathological examination revealed a malignant IPMT with an invasive component. Of the 3 fistulizations tracts, both gastric and duodenal ones were malignant. To our knowledge, this is the first reported case of IPMT with colonic fistulization. Twelve months later, the patient is alive without signs of recurrence.

Fistulating Crohn's disease is present in 17-35% of non-surgically treated patients and in up to 45% of surgically treated ones. Among the several therapeutic alternatives for this disease is intravenous immunoglobulin administration. We present a 28-year-old woman with refractory fistulating Crohn's disease who improved after prolonged immunoglobulin administration (32 months).

In this case report a persisting fistula between the right flexure of the colon and the abdominal wall was observed after a laparoscopic cholecystectomy. This was caused by a lost gallstone and eventually resulted in the resection of a colon segment. As a consequence of this case it can be said that intra-abdominally lost gallstones--especially larger ones--have to be recovered completely. In risky cholecystectomies we should consider the use of a laparoscopic bag or the installation of additional working cannulas.

The problems of surgical treatment of the low fistulous forms of anal atresia in children with the use of postero-sagittal approach are considered. A modification of this method is described. We report gastric or duodenal fistulas in 6 patients with Crohn's colitis or ileocolitis. Two patients had duodenocolic fistulas, 1 had a duodenoileal fistula, 2 had gastrocolic fistulas, and 1 had gastric and duodenal fistulas from an ileocolic anastomosis. A female in her 70s, who underwent pancreaticoduodenectomy for pancreatic head carcinoma after preoperative chemoradiotherapy, developed ascites and portal vein stenosis 5 months after the surgery, necessitating metallic stent placement. Arterioenteric fistulas (AEF) are rare and devastating complications of colorectal/pelvic malignancies.
