Length-dependent MRI of hereditary neuropathy with liability to pressure palsies.

OBJECTIVE: Hereditary neuropathy with liability to pressure palsies (HNPP) is caused by heterozygous deletion of the peripheral myelin protein 22 (PMP22) gene. Patients with HNPP present multifocal, reversible sensory/motor deficits due to increased susceptibility to mechanical pressure. Additionally, age-dependent axonal degeneration is reported. We hypothesize that length-dependent axonal loss can be revealed by MRI, irrespective of the multifocal phenotype in HNPP. METHODS: Nerve and muscle MRI data were acquired in the proximal and distal leg of patients with HNPP (n = 10) and matched con

Bilateral symmetrically symptomatic cervical Hirayama disease diagnosed with dynamic magnetic resonance imaging.

BACKGROUND: Hirayama disease is a rare, self-limiting cervical myelopathy characterized by juvenile-onset distal upper-limb unilateral or asymmetrical weakness and wasting. Bilateral and nearly symmetrical involvement is rare and may mimic motor neuron disease. The diagnosis is typically established based on dynamic flexion magnetic resonance imaging (MRI). CASE DESCRIPTION: A 23-year-old male presented with a 6-year history of progressive distal upper-limb weakness and atrophy that initially involved the right side and later the left. On examination, he demonstrated bilateral distal upper ex

In Vitro Modelling of Nerve-Muscle Connectivity in a Compartmentalised Tissue Culture Device.

Motor neurons project axons from the hindbrain and spinal cord to muscle, where they induce myofibre contractions through neurotransmitter release at neuromuscular junctions. Studies of neuromuscular junction formation and homeostasis have been largely confined to in vivo models. In this study we have merged three powerful tools - pluripotent stem cells, optogenetics and microfabrication - and designed an open microdevice in which motor axons grow from a neural compartment containing embryonic stem cell-derived motor neurons and astrocytes through microchannels to form functional neuromuscular

Peripheral nerve hyperexcitability with preterminal nerve and neuromuscular junction remodeling is a hallmark of Schwartz-Jampel syndrome.

Schwartz-Jampel syndrome (SJS) is a recessive disorder with muscle hyperactivity that results from hypomorphic mutations in the perlecan gene, a basement membrane proteoglycan. Analyses done on a mouse model have suggested that SJS is a congenital form of distal peripheral nerve hyperexcitability resulting from synaptic acetylcholinesterase deficiency, nerve terminal instability with preterminal amyelination, and subtle peripheral nerve changes. We investigated one adult patient with SJS to study this statement in humans. Perlecan deficiency due to hypomorphic mutations was observed in the pat

Age-related motor neuron degeneration in DNA repair-deficient Ercc1 mice.

Degeneration of motor neurons contributes to senescence-associated loss of muscle function and underlies human neurodegenerative conditions such as amyotrophic lateral sclerosis and spinal muscular atrophy. The identification of genetic factors contributing to motor neuron vulnerability and degenerative phenotypes in vivo are therefore important for our understanding of the neuromuscular system in health and disease. Here, we analyzed neurodegenerative abnormalities in the spinal cord of progeroid Ercc1(Delta/-) mice that are impaired in several DNA repair systems, i.e. nucleotide excision rep

Birth trauma: its effect on the urine continence mechanisms.

Birth trauma is the most important etiological factor in the genesis of stress urinary incontinence in women (SUI). There is a high incidence of SUI during pregnancy, and after delivery SUI persists in a small percentage of women (2-3%). Almost all studies on perineal muscle function reveal decreasing intravaginal pressures in the days after delivery which rarely return to predelivery levels. A few urodynamics studies have demonstrated reduced urethral closure pressures and functional length after vaginal delivery, but the importance of such findings in the genesis of SUI is controversial. Entrapment Neuropathies of the Shoulder.

Entrapment neuropathies of the shoulder most commonly involve the suprascapular or axillary nerves, and they primarily affect the younger, athletic patient population. The extremes of shoulder mobility required for competitive overhead athletes, particularly in the position of abduction and external rotation, place this cohort at particular risk. Anatomically, the suprascapular nerve is most prone to entrapment at the level of the suprascapular or spinoglenoid notch; the axillary nerve is most prone to entrapment as it traverses the confines of the quadrilateral space.Radiographs should be ord

Spinal muscular atrophy: going beyond the motor neuron.

Spinal muscular atrophy (SMA) is a neuromuscular disease caused by abnormally low cellular levels of the ubiquitous protein SMN. Traditionally, reduced levels of SMN were thought to cause the selective death of lower motor neurons, leading to denervation and atrophy of skeletal muscles. However, numerous recent studies challenge the notion that SMA is solely a disease of lower motor neurons, indicating that SMA may actually be a multi-system disorder. There are several promising therapies for SMA, but effectively targeting treatment to all affected cells and tissues remains a major issue. Iden

Correction of temporo-masseteric contour deformity using the dual paddle thoracodorsal artery perforator adiposal flap.

In cases of facial asymmetry with denervation atrophy of the masticatory muscles associated with head and neck tumors involving the trigeminal nerve, facial contour depression occurs selectively over the temple and masseteric area, separated by an uninvolved area over the zygomatic arch. The authors developed a new thoracodorsal artery perforator flap with two separate adiposal paddles based on their own perforators from the same mother vessel, the thoracodorsal vessel. Nearly normal temporo-masseteric contour was achieved in two patients. By freely positioning two adiposal paddles supplied by

Electrophysiologic evaluation of beri-beri polyneuropathy.

The diagnostic utility of electroneuromyography including F-wave and H-reflex was studied in 68 patients with peripheral polyneuropathy due to nutritional deficiency of thiamine. Out of all the electrophysiologic evaluations assessed, denervation activity in electromyography, prolonged conduction velocity and reduced motor nerve action potentials and prolonged H-reflex latency were the most frequently found abnormal findings, followed by reduced sural nerve action potential and, much less frequently, prolonged F-wave latency. In experimental thiamine-deficiency evoked polyneuropathy in chicken

Gastric acid stimulating action of cysteamine in the rat.

Cysteamine was previously found to produce duodenal ulcers in rats. In the present study, the effect of cysteamine on gastric secretion was studied. Cysteamine administered by a parenteral or intraduodenal route induced a marked and sustained increase in gastric secretion in conscious and in anesthetized rats. The response to cysteamine or its metabolite, cystamine, appeared gradually and the maximum response was observed in 2 or 3 hr after the administration. The response to cysteamine was completely inhibited by vagotomy and inhibited dose-dependently by atropine methylbromide or hexamethoni

The role of skin nociceptive afferent nerves in blister healing.

Because sensory neuropeptides improve survival of critical skin and muscle flaps in rats, skin nociceptive sensory nerve function in blister healing was examined. Sensory nerve ablation by unilateral hindlimb denervation or cutaneous axon reflex enhancement by 14 days systemic nicotine treatment (5 mg kg-1 day-1) decreased and increased, respectively, peripheral motor functions of nociceptive (peptidergic) skin nerves. Effects on nociception were measured by a radiant heat tail-flick test. Axon reflex flares were evoked by transdermal iontophoresis of acetylcholine or noxious electrical stimul

Early and continued manual stimulation is required for long-term recovery after facial nerve injury.

INTRODUCTION: We previously have shown that manual stimulation (MS) of vibrissal muscles for 2 months after facial nerve injury in rats improves whisking and reduces motor end plate polyinnervation. Here, we seek to determine whether discontinuing or delaying MS after facial-facial anastomosis (FFA) leads to similar results. METHODS: Rats were subjected to FFA and received MS for (1) 4 months (early and continued), (2) the first but not the last 2 months (discontinued), or (3) the last 2 months (delayed). Intact animals and those not receiving MS (no MS) were also examined. RESULTS: Early an

Sympathetic efferent pathways projecting to the vagina in the dog.

The level of the thoracolumbar sympathetic outflow projecting to the vagina and the laterality of the signals passing through the splanchnic nerves to the vagina have not been well understood. This study was undertaken to explore the sympathetic innervation of the canine vagina by measuring intraluminal pressure of the vagina and contraction of the vaginal wall by electrical stimulation of each of the thoracolumbar splanchnic nerves with or without transection of unilateral hypogastric nerve (HGN). The 2nd-4th lumbar splanchnic nerve (LSN) stimulation elicited elevation of intraluminal pressur

Outcome of selective peripheral denervation for cervical dystonia.

30 patients with cervical dystonia were treated by selective peripheral denervation (SPD) (Bertrand procedures). Follow-up 12-75 (median 26) months after surgery showed strong group improvements in the clinical state (Toronto Western Spasmodic Torticollis Rating Scale) and in performance of ADL, impairment, lifestyle and degree of incapacity (rating scales used by Australia's Department of Veterans' Affairs). SPD can give long-term benefits important clinically, in the home and in society.

Ptosis associated with botulinum toxin treatment of strabismus and blepharospasm.

The incidence, severity, and duration of ptosis after botulinum neurotoxin type A (BAT) injections into extraocular or orbicularis muscles were reviewed retrospectively. Even though lid droop frequently complicated botulinum toxin treatment in this series, no loss of vision or permanent ptosis was encountered. Sixty-seven injections were completed in 44 patients between September 1982 and December 1983. The records were reviewed to determine the incidence of ptosis after these 67 injections. Information was not adequate to ascertain the presence or absence of ptosis after 10 injections. Some d

Mitochondrial-derived microprotein MOTS-c attenuates immobilization-induced skeletal muscle atrophy by suppressing lipid infiltration.

Mitochondrial open reading frame of the 12S ribosomal RNA type-c (MOTS-c), a mitochondrial microprotein, has been described as a novel regulator of glucose and lipid metabolism. In addition to its role as a metabolic regulator, MOTS-c prevents skeletal muscle atrophy in high fat-fed mice. Here, we examined the preventive effect of MOTS-c on skeletal muscle mass, using an immobilization-induced muscle atrophy model, and explored its underlying mechanisms. Male C57BL/6J mice (10 wk old) were randomly assigned to one of the three experimental groups: nonimmobilization control group (sterilized wa
