Refeeding syndrome in the gastroenterology practice: how concerned should we be?

Clinical nutrition is emerging as a major area in gastroenterology practice. Most gastrointestinal disorders interfere with digestive physiology and compromise nutritional status. Refeeding syndrome (RS) may increase morbidity and mortality in gastroenterology patients. Literature search using the keywords "Refeeding Syndrome", "Hypophosphatemia", "Hypomagnesemia" and "Hypokalemia". Data regarding definition, pathophysiology, clinical manifestations, risk factors, management and prevention of RS were collected. Most evidence comes from case reports, narrative reviews and scarse observational t

The impact of L-branched-chain amino acids and L-leucine on malnutrition, sarcopenia, and other outcomes in patients with chronic liver disease.

INTRODUCTION: Metabolic and hormonal disorders resulting from chronic liver diseases culminate in increased proteolysis and decreased protein synthesis, which contributes to the development and progression of malnutrition and, consequently, sarcopenia. Nutritional management of sarcopenia in liver cirrhosis is a continuously evolving field and data on essential amino acid supplementation in chronic liver diseases is scarce. AREAS COVERED: This review encompasses the current literature on oral amino acids supplementation in patients with chronic liver disease or patients with liver cirrhosis t

Impact of malnutrition on gastrointestinal disorders and gross motor abilities in children with cerebral palsy.

UNLABELLED: Children with cerebral palsy (CP) often demonstrate abnormal feeding behaviours, leading to reduced food consumption and malnutrition. Moreover, most of them present with gastrointestinal disorders, such as gastroesophageal reflux disease (GERD) and/or chronic constipation (CC), and poor motor function rehabilitation. The aim of our study was to assess the possible relationship between malnutrition and gastrointestinal problems and to evaluate the role of nutrition on their gross motor abilities in a population of children with CP and mental retardation. PATIENTS: Twenty-one conse

Nutritional effects of a D-methionine-containing solution on AH109A hepatoma-bearing rats.

The effects of a D-methionine-containing solution (DMCS) on the nutritional status of AH109A hepatomabearing rats receiving total parenteral nutrition were studied. The DMCS solution inhibited the decrease of transferrin in the plasma of tumor-bearing rats when compared with the effect of an L-methionine-containing solution. The survival time was also significantly prolonged in the DMCS-treated rats. These results indicate that DMCS had a beneficial effect on the malnutrition induced in tumor-bearing rats and would be a useful amino acid solution for the nutritional support of cancer patients.

Effects of therapy on nutritional status of the pediatric cancer patient.

Children with cancer are at high risk for major nutrition problems both from the tumor itself and from the treatment administered. Overt malnutrition is seen in as many as 17% of children with newly diagnosed localized tumors and 37% of those with metastatic disease. Weight loss in children with cancer is directly correlated with a poor nutritional status at the time of diagnosis and with a low serum albumin. Massive surgical resections are a common source of nutritional problems and are usually not indicated as primary therapy in children with cancer. Both radiotherapy and chemotherapy are as

Acute polyneuropathy in critically ill patients.

We studied five patients in the intensive care unit (ICU) with acute polyneuropathy. All had previously presented severe infectious processes, accompanied by diverse organ failure accompanied by the Adult Respiratory Distress Syndrome (ARDS) in all cases. Two patients died and the three survivors suffered severe motor deficiencies. Electromyographic studies revealed axonal damage which predominantly affected motor neurons. Cerebrospinal fluid was normal in all the patients except one, who showed a moderate increase of lymphocytes. The common causes of polyneuropathy were excluded, but in all c

Comprehensive care of the patient with Refeeding Syndrome.

Introduction: Refeeding syndrome (RS) is a serious complication of nutritional therapy in critically ill patients, to which patients on surgical wards are particularly vulnerable. It is a potentially life-threatening, complex metabolic disorder that occurs in response to the implementation of overly aggressive, inadequate adaptive patient nutritional therapy, often exacerbated by malnutrition. Moreover, if an appropriate diagnostic and therapeutic process is not undertaken, RS itself becomes the cause of subsequent systemic complications such as respiratory failure, neurolog

Biological mechanisms for nutritional regulation of maternal health and fetal development.

This review paper highlights mechanisms for nutritional regulation of maternal health and fetal development. Malnutrition (nutrient deficiencies or obesity) in pregnant women adversely affects their health by causing or exacerbating a plethora of problems, such as anaemia, maternal haemorrhage, insulin resistance, and hypertensive disorders (e.g. pre-eclampsia/eclampsia). Maternal malnutrition during gestation also impairs embryonic and fetal growth and development, resulting in deleterious outcomes, including intrauterine growth restriction (IUGR), low birthweight, preterm birth, and birth de

From Control to Optimisation: Evolving Strategies in the Nutritional Management of Inborn Errors of Protein Metabolism.

Undoubtedly the nutritional management of inborn errors of protein metabolism (IEPM) has improved since the early 1950s, but it is still associated with significant patient burden. The pace of development has not kept up with the increasing demands of the 'real world' or development in other areas of medicine. It is essential that research and advancements in nutrition therapy proceed to the next level in order to improve long-term outcomes and the quality of life for patients. Future nutrition innovation should be fuelled by potential advancements in technology, such as precision fermentation

[Nutritional support in the comprehensive management of cutaneous squamous cell carcinoma in congenital epidermolysis bullosa].

UNLABELLED: Congenital epidermolysis bullosa (EB) is a rare genetically heterogeneous disorder characterized by blistering and erosions of the skin and mucous membranes. The dystrophic form of EB is particularly associated with severe complications including cutaneous squamous cell carcinoma (cSCC) and gastrointestinal tract involvement (esophageal strictures, malabsorption). These conditions frequently lead to protein-energy malnutrition, significantly worsening patient prognosis and quality of life. The purpose of the study was to present a clinical case of nutritional support implementation

Understanding the Best Nutritional Management for Creutzfeldt-Jakob Disease Patients: A Comparison Between East Asian and Western Experiences.

(1) Background: Creutzfeldt-Jakob disease (CJD) is a rare and fatal neurodegenerative disorder caused by the accumulation of an altered prion protein, which usually leads to death within one year after clinical onset. CJD patients usually present with rapid cognitive impairment associated with declines in cerebellar, motor, visual, behavioral, and swallowing functions. Moreover, CJD patients lose their ability to eat and take medications orally very early on in treatment; nevertheless, there are no specific nutritional guidelines for this disease shared worldwide. (2) Methods: This review aims

Assessment of Nutritional Status by Bioelectrical Impedance in Adult Patients with Celiac Disease: A Prospective Single-Center Study.

The gluten-free diet [GFD] has been linked to an increased risk of weight gain and the development of metabolic disorders. Most of the studies have focused on the effect of GFD on the Body Mass Index [BMI]. We aimed to evaluate the nutritional status using specific nutritional parameters in patients with celiac disease [CeD] at diagnosis and on a GFD compared to healthy controls. We recruited subjects at our outpatient clinic at the University of Padua. We collected demographic and clinical data and values obtained with bioelectrical impedance analysis. A total of 24 CeD patients and 28 health

Analysis of the Impact of Selected Vitamins Deficiencies on the Risk of Disability in Older People.

Vitamin deficiencies have a serious impact on healthy aging in older people. Many age-related disorders have a direct or indirect impact on nutrition, both in terms of nutrient assimilation and food access, which may result in vitamin deficiencies and may lead to or worsen disabilities. Frailty is characterized by reduced functional abilities, with a key role of malnutrition in its pathogenesis. Aging is associated with various changes in body composition that lead to sarcopenia. Frailty, aging, and sarcopenia all favor malnutrition, and poor nutritional status is a major cause of geriatric mo

Early life nutrition and neuroendocrine programming.

Alterations in the nutritional environment in early life can significantly increase the risk for obesity and a range of development of metabolic disorders in offspring in later life, effects that can be passed onto future generations. This process, termed development programming, provides the framework of the developmental origins of health and disease (DOHaD) paradigm. Early life nutritional compromise including undernutrition, overnutrition or specific macro/micronutrient deficiencies, results in a range of adverse health outcomes in offspring that can be further exacerbated by a poor postna

Nutritional and Behavioral Approaches to Body Composition and Low-Grade Chronic Inflammation Management for Older Adults in the Ordinary and COVID-19 Times.

As more insight is gained into personalized health care, the importance of personalized nutritional and behavioral approaches is even more relevant in the COVID-19 era, in addition to the need for further elucidation regarding several diseases/conditions. One of these concerning body composition (in this context; bone, lean and adipose tissue) is osteosarcopenic adiposity (OSA) syndrome. OSA occurs most often with aging, but also in cases of some chronic diseases and is exacerbated with the presence of low-grade chronic inflammation (LGCI). OSA has been associated with poor nutrition, metaboli

Intrauterine-like growth rates can be achieved with premixed parenteral nutrition solution in preterm infants.

Growth failure in neonatal intensive care units is a major challenge for pediatricians and neonatologists. The use of early "aggressive" parenteral nutrition (PN), with >2.5 g/(kg ·d) of amino acids and at least 40 kcal/(kg ·d) of energy from the first day of life, has been shown to provide nutritional intakes in the range recommended by international guidelines, reducing nutritional deficit and the incidence of postnatal growth restriction in preterm infants. However, nutritional practices and adherence to recommendations may vary in different hospitals. Two ready-to-use (RTU), premixed paren
