Is there hope for persistent airflow obstruction? From trial to practice: real-world efficacy of Tezepelumab in severe asthma with persistent airflow obstruction.

BACKGROUND: Persistent airflow obstruction (PAO) is a challenging phenotype of severe asthma, characterized by accelerated lung function decline, increased morbidity, and poor response to conventional therapies. Tezepelumab, a human monoclonal antibody targeting thymic stromal lymphopoietin (TSLP), has demonstrated efficacy in reducing airway inflammation and improving lung function. OBJECTIVE: We report a real-world case of a 72-year-old male with long-standing severe allergic and eosinophilic T2 asthma, PAO, and multiple comorbidities, uncontrolled despite optimized therapy. Narrative Review of Body-First Versus Brain-First Prodromal Neuropsychiatric Symptoms in Parkinson's Disease.

Parkinson's disease (PD) is a multisystem neurodegenerative disorder in which nonmotor symptoms often precede classical motor manifestations by years. Neuropsychiatric symptoms such as REM sleep behavior disorder (RBD), depression, anxiety, apathy, and mild cognitive impairment are increasingly recognized as clinically meaningful prodromal features, yet they remain underrecognized in routine practice. This narrative review summarizes literature identified through PubMed, Scopus, and Google Scholar between 2003 and 2025 using terms related to PD, prodromal symptoms, neuropsychiatric symptoms, a

A 3-N nose-to-brain urolithin a nanomotor targeting microglial mitophagy in neuroinflammation.

Cognitive impairment is the primary manifestation of neuroinflammation-related central nervous system diseases. Intranasal administration is an effective method, bypassing the blood-brain barrier and delivering drugs to the brain. Herein, we designed a biomimetic self-propelled nanomotor with an inflammation-targeting capacity. This nanomotor comprised a hollow mesoporous manganese dioxide (HMnO2) core and a polydopamine (PDA) shell. HMnO2 effectively catalyzed the conversion of endogenous H2O2 into H2O and O2, enabling the movement of the nanomotor into a wider area to reduce neuroinflammatio

Nanosystems at Nexus: Navigating Nose-to-Brain Delivery for Glioblastoma Treatment.

Glioblastoma multiforme (GBM) is considered to be one of the most devastating brain tumors with a shorter life expectancy. Several factors contribute to the dismal prognosis of GBM patients including the complicated nature of GBM, the ability of tumor cells to resist treatment, and the difficulty of delivering drugs to the brain because of barriers like the blood-brain barrier (BBB) and blood-tumor barrier (BTB). The unique challenges posed by the BBB in delivering therapeutic agents to the brain have led to the development of innovative nanotechnology-based approaches. By exploiting the olfac

Exposure of Zinc-induced Parkinson's disease-like non-motor and motor symptoms in relation to oxidative/nitrosative stress mediated neurodegeneration in the brain of Drosophila melanogaster.

Parkinson's disease (PD) is the second most prevalent idiopathic neurodegenerative disorder, characterized by the loss of dopaminergic neurons in the substantia nigra pars compacta, leading to locomotor impairment. Despite extensive research, the etiology of PD remains unclear, and existing experimental models for pharmacological evaluation do not fully replicate the disease's hallmarks, necessitating the development of a cost-effective and reliable alternative. In recent past, Drosophila melanogaster has been utilized as a model organism for various neurodegenerative diseases, including PD. T

Ceftriaxone-loaded albumin-based nanoparticles incorporated into in situ ionic sensitive nasal gels: Development, characterization, and nasal drug deposition studies.

Ceftriaxone (CFT) is recommended as the drug of choice for the empirical treatment of bacterial meningitis. However, conventional drug delivery has difficulty in crossing the blood-brain barrier (BBB), leading to insufficient therapeutic efficacy. Therefore, nasal drug administration emerged as a viable strategy, providing the opportunity for brain-targeted delivery to reach effective brain concentrations. In this work, we present the development of in situ nasal gels of CFT-loaded albumin nanoparticles for nose-to-brain delivery. The obtained formulations were characterized in terms of partic

Intranasal Delivery of BACE1 siRNA and Rapamycin by Dual Targets Modified Nanoparticles for Alzheimer's Disease Therapy.

Alzheimer's disease (AD), as a progressive and irreversible brain disorder, remains the most universal neurodegenerative disease. No effective therapeutic methods are established yet due to the hindrance of the blood-brain barrier (BBB) and the complex pathological condition of AD.  Therefore, a multifunctional nanocarrier (Rapa@DAK/siRNA) for AD treatment is constructed to achieve small interfering RNA of β-site precursor protein (APP) cleaving enzyme-1 (BACE1 siRNA) and rapamycin co-delivery into the brain, based on Aleuria aurantia lectin (AAL) and β-amyploid (Aβ)-binding peptides (KLVFF) m

Function of BRD4 in the pathogenesis of high glucose‑induced cardiac hypertrophy.

Diabetic cardiomyopathy is one of the major complications of diabetes, and due to the increasing number of patients with diabetes it is a growing concern. Diabetes‑induced cardiomyopathy has a complex pathogenesis and histone deacetylase‑mediated epigenetic processes are of prominent importance. The olfactory bromodomain‑containing protein 4 (BRD4) is a protein that recognizes and binds acetylated lysine. It has been reported that the high expression of BRD4 is involved in the process of cardiac hypertrophy. The aim of the present study was to investigate the function of BRD4 in the process of

Patched1 is required in neural crest cells for the prevention of orofacial clefts.

Defects such as cleft lip with or without cleft palate (CL/P) are among the most common craniofacial birth defects in humans. In many cases, the underlying molecular and cellular mechanisms that result in these debilitating anomalies remain largely unknown. Perturbed hedgehog (HH) signalling plays a major role in craniofacial development, and mutations in a number of pathway constituents underlie craniofacial disease. In particular, mutations in the gene encoding the major HH receptor and negative regulator, patched1 (PTCH1), are associated with both sporadic and familial forms of clefting, ye

Brain and testicular tumors in mice with progenitor cells lacking BAX and BAK.

The proapoptotic BCL-2 family proteins BAX and BAK serve as essential gatekeepers of the intrinsic apoptotic pathway and, when activated, transform into pore-forming homo-oligomers that permeabilize the mitochondrial outer membrane. Deletion of Bax and Bak causes marked resistance to death stimuli in a variety of cell types. Bax(-/-)Bak(-/-) mice are predominantly non-viable and survivors exhibit multiple developmental abnormalities characterized by cellular excess, including accumulation of neural progenitor cells in the periventricular, hippocampal, cerebellar and olfactory bulb regions of t

Evolutionary divergence of the reptilian and the mammalian brains: considerations on connectivity and development.

The isocortex is a distinctive feature of the mammalian brain, with no clear counterpart in other amniotes. There have been long controversies regarding possible homologues of this structure in reptiles and birds. The brains of the latter are characterized by the presence of a structure termed dorsal ventricular ridge (DVR), which receives ascending auditory and visual projections, and has been postulated to be homologous to parts of the mammalian isocortex (i.e., the auditory and the extrastriate visual cortices). Dissenting views, now supported by molecular evidence, claim that the DVR origi

Single cell transcriptome analysis of mouse carotid body glomus cells.

KEY POINTS: Carotid body (CB) glomus cells mediate acute oxygen sensing and the initiation of the hypoxic ventilatory response, yet the gene expression profile of these cells is not available. We demonstrate that the single cell RNA-Seq method is a powerful tool for identifying highly expressed genes in CB glomus cells. Our single cell RNA-Seq results characterized novel CB glomus cell genes, including members of the G protein-coupled receptor signalling pathway, ion channels and atypical mitochondrial electron transport chain subunits. A heterologous cell-based screening identified acetate (w

Region-Specific Transcriptional Control of Astrocyte Function Oversees Local Circuit Activities.

Astrocytes play essential roles in brain function by supporting synaptic connectivity and associated circuits. How these roles are regulated by transcription factors is unknown. Moreover, there is emerging evidence that astrocytes exhibit regional heterogeneity, and the mechanisms controlling this diversity remain nascent. Here, we show that conditional deletion of the transcription factor nuclear factor I-A (NFIA) in astrocytes in the adult brain results in region-specific alterations in morphology and physiology that are mediated by selective DNA binding. Disruptions in astrocyte function fo

Vilazodone Hydrochloride-Loaded Nanostructured Lipid Carrier In Situ Gel for Intranasal Nose-to-Brain Antidepressant Delivery.

The Oral delivery of vilazodone hydrochloride (VLZ), a serotonin partial agonist-reuptake inhibitor for major depressive disorder, suffers from extensive hepatic first-pass metabolism, food-dependent absorption, and poor blood-brain barrier (BBB) penetration, limiting its central therapeutic impact. Here, VLZ-loaded nanostructured lipid carriers (NLCs) were engineered and embedded within a Poloxamer 407/188-chitosan thermosensitive in situ gel to enable direct nose-to-brain delivery. The NLCs were fabricated by combined high-shear and high-pressure homogenisation and optimised through central

Integrative analysis of single-cell multi-omics reveals heterogeneity in piriform cortex principal neurons and their selective vulnerability in Alzheimer's disease.

BACKGROUND: The piriform cortex (PIR) is a key region of the olfactory system and is increasingly implicated in Alzheimer's disease (AD). However, the cellular heterogeneity of principal neurons (PN) in this region and their selective vulnerability in AD remain poorly understood. METHODS: Here, we integrated single-cell multi-omics datasets to systematically characterize PIR PN heterogeneity. We identified four transcriptionally and epigenetically distinct PN subtypes with different spatial distributions across the anterior and posterior PIR. RESULTS: In AD mouse models, the abundance of the

Systematic review of tactile-based interventions combined with multisensory stimulation protocols in the rehabilitation of patients with disorders of consciousness.

Social touch interventions have the potential to modulate psychological and physiological stress in intensive care settings. In individuals with disorders of consciousness (DOC), tactile stimulation combined with other sensory modalities, (e.g., visual, auditory, olfactory, gustatory, and proprioceptive), has been proposed to enhance arousal, behavioural responsiveness, and recovery, while mitigating sensory deprivation. However, tactile stimulation encompasses heterogeneous forms of touch and the extent to which clinical benefits can be attributed to specific tactile qualities remains unclear

New advances in the treatment of chronic rhinosinusitis with nasal polyps from pathophysiology to biologic targeted therapy.

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNP) is a chronic airway disease characterized mainly by type 2 inflammation, which seriously impairs patients' quality of life and imposes a huge economic burden. Traditional treatments cannot eradicate underlying immune disorders, leading to high postoperative recurrence. This review aims to summarize the pathophysiological basis of CRSwNP and the clinical application advances of biologics in its precision treatment. METHODS: A narrative review was performed to synthesize key literature on the pathophysiology of CRSwNP, focusing on th
