INTRODUCTION: Ipsilateral fractures of the shaft of the femur combined with hip dislocations are extremely rare injuries, presenting a difficult diagnostic and therapeutic challenge. Diagnosis of hip dislocation is often delayed, due to the focus on femoral fracture.
CASE PRESENTATION: This article presents a case never described before of a 19-year-old patient who sustained this unusual combination of injuries as a result of a road traffic accident. The patient had an irreducible posterior hip fracture-dislocation associated with an ipsilateral femoral shaft fracture. Closed reduction attemp

In proximal humeral fractures with first degree displacement and in fractures with second degree displacement, which primarily prove to be stable, reduction is not necessary. Closed reduction is indicated in unstable fractures with second degree displacement and in all fractures with third degree displacement. If closed reduction fails to improve the position of the fracture, open reduction and Kirschnerwire transfixation should be performed. Retention for two to three weeks is sufficient.

STUDY DESIGN: Case report.
OBJECTIVE: To describe a novel approach to treating traumatic spondyloptosis of the lumbar spine.
SUMMARY OF BACKGROUND DATA: Traumatic spondyloptosis of the lumbar spine is a relatively uncommon injury because it requires a significantly large amount of energy to occur. Reduction of these fracture dislocations in the operating room may be difficult through a solely open approach because it usually requires a significant degree of bone removal and spinal manipulation with instrumentation.
METHODS: In this case report, the authors present a unique method of closed

Introduction Mandibular fractures have the highest incidence next to nasal bone fractures of which condylar fractures account for one-third of it. Various approaches for condylar fracture include intraoral and extraoral approaches such as coronal, preauricular, postauricular, endaural, endoscopic, rhytidectomy, transparotid, submandibular, and retromandibular approaches. The purpose of this study was to evaluate the mini-preauricular incision in open reduction and internal fixation of condylar and subcondylar fractures of the mandible. Materials and methods Twenty patients with condylar fractu

OBJECTIVE: The aim of arthroscopically assisted treatment of tibial plateau fractures is to achieve minimally invasive reduction and internal fixation of the joint fracture of the tibial plateau. Using the arthroscopic procedure, both the approach morbidity and the control of the articular reduction can be optimized.
INDICATIONS: Displaced tibia plateau fractures of AO type A1 and B1/2/3 or Tscherne P2.
CONTRAINDICATIONS: Strongly displaced tibial plateau fractures, which require an open surgical approach and stabilization with plate fixation (e.g., AO type C fractures or Moore type 5 fractu

INTRODUCTION: T-condylar fractures of the humerus are very rare in children. The treatment options vary from simple reduction and percutaneous stabilisation to open reduction and fixation with plates, screws and Kirchner wires. We evaluated 19 patients who underwent different types of treatment aiming to compare two different approaches of reduction, postoperative results and complications.
METHODS: The study encompassed total of 19 patients with T-condylar fracture, aged from 3 to 16 years, who underwent either closed or opened reduction between February 2005 and September 2012. Closed reduc

Nasal bone fractures are the most common among facial fractures and are the third most common fractures in the human frame. Although many forms of treatment have been introduced, controversy regarding the optimal treatment still remains. Nasal bone fractures are complex, with significantly varying types that are often undermanaged in closed reduction procedures. The authors' experiences with nasal bone fractures have shown that the baseline for surgical intervention depends on the type of fracture and the method of maintenance after reduction, both of which have considerable impact on the fina

Fractures of the patella are relatively rare injuries for children. Fourteen patients with patellar fractures were reviewed. There were 12 boys and two girls with the age ranging between 9 and 15 years and an average of 11.4 years. Sleeve fractures were the most common type of patellar fractures observed (eight cases), followed by transverse fractures (four cases) and comminuted fractures (two cases). All the fractures were treated with open reduction and followed up for 2 to 20 years. Overall results were good in 13 patients, and flexion limitation of the knee was seen in one. Fractures of th

OBJECTIVES: Bennett's fracture, the most common intra-articular fracture occurring at the base of the thumb, accounts for less than 1% of all hand fractures. It typically results from specific traumatic mechanisms, deforming forces, and ligamentous injuries. To compare the functional outcomes and sequelae in patients with Bennett's fracture treated surgically using: Open reduction and internal screw fixation, closed reduction and percutaneous fixation, arthroscopy-assisted reduction and screw fixation.
METHODS: A systematic literature review was conducted to identify studies on Bennett's frac

Salter-Harris type I (SH-I) fractures of the distal fibula are commonly encountered in pediatric orthopedics. We describe 2 unique cases of adolescents with completely displaced SH-I distal fibula fractures that were treated operatively. In the first case, a closed reduction attempt failed and the patient required open reduction and internal fixation of the distal fibula and syndesmosis. The syndesmotic ligaments were avulsed from the distal fibular metaphysis. In the second case, closed reduction of the distal fibula fracture was partially successful, but anatomic reduction could not be achie

We report a case of trans-scaphoid perilunate fracture-dislocation with concomitant lunotriquetral ligament disruption of the right wrist in a 44-year-old man, sustained from a 10-m fall landing on his outstretched right hand. Open reduction was performed 1 day after injury; at first the palmar dislocation of the lunate was reduced with the palmar approach. Under direct view with the dorsal approach, the scaphoid was comminuted and then treated with open reduction and internal fixation with a double threshold screw using a dorsal approach and a bone graft from the distal radius. Although the s

A 13-year retrospective review of 53 patients with laryngeal trauma seen between 1965 and 1975 at the University of Texas Health Science Center at Dallas affiliated hospitals is presented. Patient management fell into four groups: observation following direct laryngoscopic examination or observation alone (7 patients), direct laryngoscopic examination and tracheostomy (5 patients), open reduction of fractures without stenting (14 patients), and open reduction of fractures with stenting (27 patients). These cases were analyzed according to initial signs and symptoms, duration between time of in

Scapular fractures account for only 1% of all fractures, and displaced intra-articular glenoid fractures are very rare. This report describes the operative treatment of a displaced intra-articular fracture of the glenoid in a 33-year-old man. The fracture was repaired by open reduction and internal fixation, using a pneumatically powered stapler for initial stabilization of the fracture fragment and anatomic reduction of the articular surface.

INTRODUCTION: The long-term results of pilon fractures are rarely documented. The present study evaluated the long-term results related to each fracture pattern of a pilon fracture treated with open reduction and plating.
PATIENTS: One hundred and twenty-eight pilon fractures with a mean 10-year follow-up were divided into three groups, based on the Ruedi-Allgower classification. Group A was composed of 39 patients with Ruedi-Allgower type I fractures; group B comprised 62 patients with type II fractures; and group C included 27 patients with type III fractures. The radiographs were reviewed

We describe an articular talar dome fracture treated with the use of an arthroscopically assisted placement of a transfibular Acutrak screw (Accumed, Hillsboro, OR). This minimally invasive technique can be used to avoid the large incision used in the traditional open reduction and internal fixation of these fractures. Accurate reduction and stable fixation of fractures of the articular surface of the talus is a must, and this technique allows this while avoiding the morbidity of the open approach.

INTRODUCTION: Femoral shaft fractures are a common entity in trauma surgery. The gold standard is closed reduction and intramedullary nailing. However, in some fracture patterns, the surgeon may choose to open the fracture for reduction. The purpose of this retrospective study was to analyze the outcome of femoral shaft fractures treated with open reduction, cerclage wiring and intramedullary nailing.
METHODS: We included adult patients with femoral shaft fractures treated with open reduction, cerclage wiring and intramedullary nailing at a level 1 trauma center in Switzerland during an 11-ye

OBJECTIVE: Operative treatment of vertebral fractures is focused on stabilization of the spine and decompression of the spinal cord and nerves. In German-speaking countries, it is common to restore the sagittal profile by fracture reduction. The use of percutaneous systems has been limited due to inferior reduction possibilities compared to open systems. The improved reduction options offered by newer percutaneous systems extend their application to highly deformed fractures.
INDICATIONS: Unstable thoracic, lumbar and lumbosacral spine fractures; unacceptable spinal deformities.
