Using a Frontline Staff Intervention to Improve Cervical Cancer Screening in a Large Academic Internal Medicine Clinic.

BACKGROUND: Cervical cancer is the third most common malignancy affecting women. Screening with Papanicolaou (Pap) tests effectively identifies precancerous lesions and early-stage cervical cancer. While the nationwide rate of cervical cancer screening (CCS) is 84%, our urban general internal medicine (GIM) clinic population had a CCS rate of 70% in 2016. OBJECTIVE: To improve our clinic's CCS rate to match or exceed the national average within 18 months by identifying barriers and testing solutions. DESIGN: A quality improvement project led by a multidisciplinary group of healthcare provide

Implementation contextual factors related to community-based active travel to school interventions: a mixed methods interview study.

BACKGROUND: Active travel to school contributes to multiple physical and psychosocial benefits for youth, yet population rates of active travel to school are alarmingly low in the USA and many other countries. Though walking school bus interventions are effective for increasing rates of active travel to school and children's overall physical activity, uptake of such interventions has been low. The objective of this study was to conduct a mixed methods implementation evaluation to identify contextual factors that serve as barriers and facilitators among existing walking school bus programs. Most surgeons' daily elective lists in Florida comprise only 1 or 2 elective cases, making percent utilization unreliable for planning individual surgeons' block time.

STUDY OBJECTIVE: Operating room (OR) utilization has been shown in multiple studies to be an inappropriate metric for planning OR time for individual surgeons. Among surgeons with low daily caseloads, percentage utilization cannot be measured accurately because confidence limits are extremely wide. In Iowa, a largely rural state, most surgeons performed only 1 or 2 elective cases on their OR days. To assess generalizability, we analyzed Florida, a state with many high-population density areas. DESIGN: Observational cohort study. SETTING: The 602 facilities in Florida that performed inpatient

Confusion Reigns: An Analysis of Responses to U.S. Soccer Age Cut-Off Date Policy Change.

Relative age effects (RAEs) have been associated with the common practice of grouping athletes by chronological age. Development and selection advantages are often awarded to those who are born closer to, but following, the cut-off date employed by sport systems. In 2015, the U.S. Soccer Federation announced that it would be changing its birth-year registration cut-off date from August 1st to January 1st. This change was introduced to align the U.S. youth soccer calendar with international standards, and simultaneously provide clearer information on player birthdates to "lessen" RAEs. The magn

The North Carolina Breast Cancer Screening Program: foundations and design of a model for reaching older, minority, rural women.

Breast cancer screening programs do not reach all women at the same rate. Screening mammography use varies according to sociodemographic characteristics; mammography utilization is highest among women in their fifties but then decreases with age. In North Carolina, breast cancer is a particular burden for Black and lower-income women. Black women are more likely to be diagnosed with late stage disease, and their rate of breast cancer mortality is higher than it is for White women even though the incidence in White women is greater. Older, Black, and low-income women are less likely to obtain s

A Service-Constrained Two-Stage Stochastic Programming Framework for Hospital Pharmacy Medication Supply Under Demand Uncertainty: Heterogeneous Demand, Conditional GARMA Scenarios, and Longitudinal Validation.

Background/Objectives: Hospital pharmacies must maintain medication availability despite heterogeneous and time-varying demand. Statistical fit alone may not yield well-calibrated future scenarios or the best replenishment decision. This study prospectively evaluates a decision-support framework combining probabilistic demand modelling, conditional temporal adaptation, explicit service requirements, and stochastic inventory optimization. Methods: Forty-eight monthly requested-demand observations for 13 anonymized medicines from a Chilean public hospital were evaluated through 12 expanding-wind

Evaluating Nursing Work Systems and Identifying Barriers for Robotic Technology Integration: Observational Study.

BACKGROUND: Robotic technology has the potential to assist nurses, but the complexity and unpredictability of health care environments cannot be replicated in a laboratory setting. Furthermore, there is a lack of experiential evidence that robotic technology will meaningfully impact nursing. Collaborative development of technology and real-world usability studies offers the ability to address problems early in the design process when functional changes can be implemented. OBJECTIVE: The purpose of this study was to use an observational study and systematically evaluate the work system of inpa

Intellectual functioning in children with early shunted posthemorrhagic hydrocephalus.

OBJECTIVES: Early intervention for cognitive impairments seen in children with hydrocephalus is key to successful developmental outcome. Thus, the examination of the cognitive and behavioral functioning of children with hydrocephalus is important, given increasing survival rates and the potential to implement successful cognitive interventions. The current literature suggests that intellectual impairment is common in hydrocephalus patients, yet the findings vary concerning the extent and specifics of the cognitive deficits seen in these children. To better understand the pattern of cognitive i

Bridging gaps in breast cancer clinical trial enrollment: insights from a community-based research registry initiative.

OBJECTIVE: Cancer research registries are critical for ensuring representative participation in clinical trials. However, registries based primarily in hospitals may under-enroll women and other populations who experience structural, logistical, cultural, and informational barriers to care and research participation. Community-based organizations (CBOs) may help address this gap by developing registries that engage diverse populations of women at increased risk for and surviving with breast cancer more directly. The purpose of this study was to examine demographic, clinical, and organizational

From Anxiety to Action: Mental Health Education Transforming Pharmacy Students' Self-Care and Resilience.

OBJECTIVE: Pharmacy students experience alarmingly high rates of mental health challenges, with 18% to 60% reporting depressive symptoms, 21% to 50% experiencing anxiety, and up to 17% reporting suicidal ideation. Despite recognizing the importance of self-care, students struggle to prioritize their well-being, creating a critical knowledge-behavior gap, while professional resilience declines throughout training. METHODS: This study examined how a 2-session mental health educational intervention was associated with reported shifts in pharmacy students' cognition of stress, self-care, and prof

Understanding Primary Care Providers' Decisions to Recommend Complementary and Integrative Health Therapies: A Qualitative Analysis.

BACKGROUND: Evidence-based complementary and integrative health (CIH) therapies are now in clinical practice guidelines for common pain and pain-related conditions. Primary care providers (PCPs) are often the first point of contact for discussing CIH therapies as non-pharmacological health management. Yet, little is known about their decisions to recommend CIH therapies. OBJECTIVE: To understand: (1) which CIH therapies PCPs recommend and for what health conditions, (2) reasons for their recommendations or lack thereof, and (3) possible solutions to improve appropriateness of recommendations.
