Neural network differentiation of optic neuritis and anterior ischaemic optic neuropathy.

AIMS: The efficacy of an artificial intelligence technique, neural network analysis, was examined in differentiating two optic neuropathies with overlapping clinical profiles-idiopathic optic neuritis (ON) and non-arteritic anterior ischaemic optic neuropathy (AION). METHODS: A neural network was trained with data from 116 patients with 'gold standard' diagnoses of ON or AION. It was then tested with data from 128 patients with presumed ON or AION, and the correlation of the network's diagnosis with that of expert clinicians tabulated. RESULTS: The network agreed with the clinicians on 97.8%

Making sense of refractive surgery in 2001: why, when, for whom, and by whom?

Surgical alteration of the focusing or refractive properties of the eye has been performed on millions of patients. An array of procedures to correct myopia, hyperopia, astigmatism, and presbyopia have been introduced over the past 25 years with varying degrees of success. Improved technology has increased patient and physician satisfaction and enthusiasm. Currently available surgical procedures can be categorized as incisional, surface-altering, lamellar, and intraocular. The choice of procedure depends on individual patient indications and contraindications based on results of ocular examina

[Early correction of hyperopia and astigmatism in children leads to better development of visual acuity].

BACKGROUND: From the investigations of Wiesel and Hubel an influence of early spectacle correction of refractive errors on visual development is to be expected. Nevertheless the benefit of early spectacle correction in young children is being discussed controversially. PATIENTS AND METHODS: In a retrospective study the records of 103 strabismic children were analyzed. Myopic children and those with hypermetropia less than 2.0 diopters without astigmatism were excluded from this study. Visual acuity of the dominant eye only was compared between the following subgroups: early spectacle correcti

Binocular vision interference from unequal inputs in an adult patient with monocular dense acquired cataract.

INTRODUCTION: The effect of the unequal visual inputs in uniocular cataracts is addressed through the hypothesis that a marked inequality of the visual inputs will have an interference effect that will degrade the binocular vision relative to that of the better eye. We tested this hypothesis of reverse interference effect by both clinical and functional Magnetic Resonance Imaging assessments in an acute unilateral cataract patient. The patient was 63 year old woman who rapidly developed a dense sub-capsular cataract following steroid radio-chemotherapy treatment for breast cancer. METHODS: Cl

Indirect scatter laser photocoagulation to subfoveal choroidal neovascularization in age-related macular degeneration.

BACKGROUND: Occult choroidal neovascularization (CNV), poorly defined on fluorescein angiography, is present in the majority of patients with exudative complications of age-related macular degeneration. For patients who present with this type of subfoveal CNV but who have useful visual acuity, no form of treatment is of proven benefit. Accordingly, a pilot randomized trial of indirect laser treatment was performed. The rationale of this treatment was to inhibit the CNV through laser-induced effects on the retinal pigment epithelium. METHODS: Patients with occult subfoveal CNV without retinal

Immediate visual capability after cataract surgery: topical versus retrobulbar anesthesia.

This study evaluated vision immediately after surgery in 56 consecutive patients (31 who had topical anesthesia and 25 who had retrobulbar anesthesia) for cataract extraction by phacoemulsification and intraocular lens implantation. An unbiased, trained nurse measured uncorrected and best corrected visual acuities one hour, one day, and one week after surgery. One hour after surgery, patients in the topical anesthesia group had significantly better visual acuity than those in the retrobulbar anesthesia group. One day and one week after surgery there was no difference between the groups in visu
