Spironolactone is a synthetic aldosterone receptor antagonist, with a role off-label in various dermatological conditions. Its antiandrogenic properties make it suitable for diseases in which excess androgen production results in unwanted and psychologically distressing manifestations in susceptible females. Treatment with spironolactone aims to attenuate androgen-mediated conditions including acne, hidradenitis suppurativa, female pattern hair loss and hirsutism. We discuss the emerging utility of spironolactone in dermatology, its potential adverse effects and considerations for monitoring.

Acetylhomotaurine was labeled with (11)C via N-acetylation with [(11)C]acetyl chloride. The synthesis yielded 48.2+/-3.8%, decay corrected to end of bombardment. The specific activity of the (radio)chemically pure product was 20.8+/-2.0 GBq/micromol at EOS. In vivo studies revealed a very fast clearance of the tracer from the blood and a uniform distribution in the different brain regions. Unfortunately, the poor passage through the blood brain barrier makes the tracer not suitable for PET studies.

Two congeners Q4 and Q5 inferred in earlier analyses to be cyclic succinimide-type dehydration products of recombinant hirudin (variant 1) were structurally fully characterized. After isotopic labeling by ring-opening with H2(18)O, the suspected anhydro-positions Asp53 and Asp33 were confirmed by tandem mass spectrometry (MS/MS) sequencing of relevant smaller peptides directly in the enzymatic hydrolysates (V8 protease) using electrospray MS/MS. The chosen strategy proved highly efficient and sensitive.

A 5 year old spanish boy was studied in whom the clinical phenotype was that of classic Lesch-Nyhan syndrome. Activity of hypoxanthine guanine phosphoribosyl transferase (HGPRT) was studied in an intact fibroblast system in which the pattern of incorporation of isotope of 14C-labeled hypoxanthine into its purine products was assessed. This method has been effective in distinguishing among HGPRT variants even when activity in erythrocyte lysates is zero. In this patient the activity was 0,4 percent of normal, consistent with Lesch-Nyhan disease.

Leptospira biflexa were isolated from urine cultures of two patients with clinical and laboratory findings compatible with leptospirosis. Neither patient had detectable leptospiral agglutinins. The source of the L. biflexa was water labeled " "sterile for tissue culture" purchased from M. A. Bioproducts, formerly Microbological Associates. M. A. Bioproducts water is sterilized by filtration through a 0.22-mum-pore size membrane filter. Leptospiral contamination can occur in products sterilized by filtration. Heat sterilization of water eliminates this possibility.

Adult splenectomized mice exhibit increased resistance to infection with Listeria monocytogenes. Phagocytosis, by reticulo-endothelial cells, of test particles (51Cr-labelled sheep erythrocytes) is the same in splenectomized and control mice. However, 24 h exposure to Listeria, which fails to influence phagocytic activity of normal mice, greatly enhances the blood clearance and liver uptake of the test particles in splenectomized mice. The presence of a cell population and/or product in the spleen which modulates macrophage activation upon the exposure to appropriate stimuli is postulated.
