RATIONALE: Multiple evanescent white dot syndrome (MEWDS) is a self-limited multifocal chorioretinopathy that typically affects otherwise healthy young females in the second to fourth decades of life. Current understanding of the pathophysiology of MEWDS is still limited. One of the possible underlying causes is an infectious etiology.

Lyme serology is too commonly prescribed in absence of a well established differential diagnosis. Its low positive predictive value is very often unusable. IgG and IgM positive results are tricky to interpret and often lead to clinically unjustified antibiotherapy. Clinical and epidemiological approaches must integrate the serological results. The lysis centrifugation technique is preferred for culturing Brucella spp. at all stages of brucellosis because it yields 25% more positive results and on average provides results 10 days earlier than the Ruiz-Castaneda method. This lysis method is inexpensive and easier to use and may be used in laboratories with limited expertise or equipment if all safety precautions are taken.

Fifty sera collected from 12 different species of free-living game animals in game parks in the Northern Natal were tested against 8 Leptospira interrogans antigens using the microscopic agglutination test (MAT). Six out of 50 animals had titres, all less than 100. Three of these animals had titres to serovar mini, 1 animal to tarrasovi, and 3 animals had multi-serovar reactions, 1 to mini and hardjo, and 1 to tarrasovi, copenhageni and pomona.

Preliminary enzyme-linked immunosorbent assay (ELISA) with serum from a patient with onchoceriasis revealed extensive cross-reactions with various nematode antigens. Further tests on a batch of sera from people with proven O. volvulus infections using O. gutturosa antigen, showed that almost all the sera gave higher ELISA values than did control African sera.
