Clinico-pathological study of colorectal carcinoma.

INTRODUCTION: Colorectal carcinoma (CRC) ranks as the fourth most frequent cancer in men (after lung, prostate and stomach cancer), and third in women (after cancers of breast and uterine cervix).Adenocarcinoma of the colon is the most common malignancy of the GI tract and is a major cause of morbidity and mortality worldwide. It is also one of the most common cancers in Nepal. METHODS: A descriptive study was conducted in a Kathmandu based Hospital. This study included 50 cases of colorectal carcinoma diagnosed on colonoscopic/sigmoidoscopic biopsies over a period of two years. RESULTS: A t

[A patient with sigmoid colon cancer( cStage IV) who responded to preoperative chemotherapy with mFOLFOX6 plus bevacizumab and was subsequently able to undergo a radical operation].

There are a growing number of reports of unresectable, advanced colorectal cancer and multiorgan invasive colorectal cancer for which extended surgery was avoided or a radical operation was performed after down-staging, or tumor size reduction, was achieved by chemotherapy. Here we describe a case of sigmoid colon cancer (cStage IV) for which preoperative chemotherapy improved the outcome of surgery. The patient was a 57-year-old man with sigmoid colon cancer of sufficient size to block the passage of the endoscope. The cancer was found to be widely infiltrated and adherent to the peritoneum o

[A long-term survival case of advanced sigmoid colon cancer with massive metastases to the para-aortic lymph nodes].

We report a long-term survival case of advanced sigmoid colon cancer with massive metastases to the para-aortic lymph nodes. The patient was a 63-year-old male. He underwent sigmoidectomy with D3 + para-aortic lymphadenectomy for advanced sigmoid colon cancer. Histological examination showed a moderately differentiated adenocarcinoma, ss, ly2, v2, n4 (total 30/64, para-aortic lymph nodes 18/39). From 6 months to 4 years after the operation, tegafur/uracil (UFT) was performed as chemotherapy. After 8-year disease-free interval, CT showed solitary mediastinal lymph node metastases. He underwent

[Open surgery of the rectum].

This article describes the individual techniques and strategies of conventional surgery of the rectum, including photographic documentation of the important steps. Our own figures of postoperative complications and long-term oncological results obtained by this approach are added. In our experience, operations for radical surgery of rectal cancer have been the most frequent procedures. In the pelvis, the important planes between which dissection occurs are (1) the mesorectal fascia up to the origin of the inferior mesenteric artery and continuing laterally to the mesosigmoid fascia and (2) on

Superior mesenteric and portal vein thrombosis following laparoscopic-assisted right hemicolectomy. Report of a case.

PURPOSE: This article describes a case of superior mesenteric and portal vein thrombosis following laparoscopic-assisted right hemicolectomy. METHODS: A retrospective case review was performed. RESULTS: Data continue to grow regarding safety and technical feasibility of laparoscopic-assisted colectomy. As this minimally invasive alternative to open colonic resection becomes more popular, it is inevitable that information on benefits and complications associated with it will continue to expand. We report a case of superior mesenteric and portal vein thrombosis following laparoscopic-assisted

Hartmann's procedure for carcinoma of the rectum and sigmoid colon.

A review of the Hartmann's operation for patients with rectal and sigmoid cancer over an 18 year period is presented. There were 1063 patients who had a resection for carcinoma of the rectum or sigmoid colon and 4.4% of these had a Hartmann's procedure. This operation was particularly useful in the management of patients who presented with a proximal obstruction or perforation at the tumour site. It was also effective in the elective treatment of elderly unfit patients who had locally advanced tumours or those with distant metastases. Re-anastomosis is recommended in those patients who are rel

[Sixty-one cases of acute obstructions of the colon caused by cancer. Indications for emergency surgery].

Sixty-one cases of acute malignant colonic obstruction were operated upon within 24 hours after the patient's admission, between 1979 and 1989: 10 for a right colon carcinoma, 51 for a left-sided lesion. Indications, for emergency operation were: complete clinical obstruction, iliac tenderness, radiologic ileal fluid levels and arrest on barium enema. The initial procedures consisted of 8 right hemicolectomies, and for the left-sided obstructions, 32 colostomies, 7 colectomies with immediate anastomosis, and 12 colectomies without anastomosis. Overall mortality was nil for the right side and 4

A relationship between perioperative blood transfusion and recurrence of carcinoma of the sigmoid colon following potentially curative surgery.

Preoperative blood transfusions are used to improve graft survival in renal transplantation. If such an immunomodulating effect occurred in cancer surgery perioperative blood transfusion may be detrimental to patient outcome. A retrospective study of 68 patients undergoing potentially curative surgery for adenocarcinoma of the sigmoid colon, over a 10 year period was performed. Thirty-three patients (49%) had a perioperative blood transfusion of which two-thirds received either one or two units. Transfused patients had a poorer prognosis compared to non-transfused patients (0.28 and 0.53 five

Colonoscopy and the detection of dysplasia in patients with longstanding ulcerative colitis.

The role of colonoscopy in the surveillance of patients with ulcerative colitis for dysplasia was investigated in 43 patients with longstanding disease extending proximally beyond the splenic flexure. The examination was well tolerated and no complications were recorded. Dysplasia was found in 9 patients in one or more biopsies and was severe in 2, moderate in 1, and mild in 6. Both the patients with severe dysplasia had a carcinoma at subsequent operation. Colonoscopy may allow complete examination of the colon and biopsy of suspicious lesions and, since dysplasia may spare the rectum, colono

Single incision laparoscopic total abdominal colectomy with ileorectal anastomosis for synchronous colon cancer.

Single incision laparoscopy is currently performed mostly for basic laparoscopic procedures involving single abdominal quadrants. The aim of this case report is to show that single incision laparoscopic techniques can be utilized for complex abdominal laparoscopic procedures with a large target organ and a working space involving all quadrants of the abdominal cavity. A single incision laparoscopic total abdominal colectomy with an ileorectal anastomosis and intraoperative CO(2) colonoscopy was performed for a patient with synchronous adenocarcinoma of the cecum and the sigmoid colon. The pati

Reconstruction of urinary tract utilizing transverse colon conduit for a cystectomized patient with post-ureterocutaneostomy complications and sigmoid colon cancer.

A seventy-four years old man had been suffering from symptomatic and infectious complications associated with conjoined cutaneoureterostomy following the radical cystectomy for advanced transitional cell carcinoma of the bladder 3 years ago. He underwent urinary reconstruction using transverse colon conduit after diagnostic exclusion of recurrent urothelial tumor in the upper urinary tract, evaluation of performance status and endoscopic examination of the colon. Enteroscopy incidentally revealed he had an early stage adenocarcinoma in his sigmoid colon to be resected, and the resection was fo

The role of colonoscopy in the assessment of patients with colorectal cancer.

A retrospective review of 176 patients with adenocarcinoma of the colon who underwent total colonoscopy preoperatively demonstrated synchronous carcinomas in 3.4 percent and synchronous polyps in 55.1 percent. Full-column barium enemas (68 patients) failed to identify cancer in 22 percent of patients and synchronous polyps in 58 percent of patients, a statistically significant (P less than 0.001) number of false-negative examinations. Double-contrast barium enemas (30 patients) failed to identify cancer in 27 percent of patients and synchronous polyps in 42 percent of patients, also a statisti

Evaluation of epidermal growth factor receptor, carcinoembryonic antigen and Lewis carbohydrate antigens in human colorectal and liver neoplasias.

The expression of LewisY related carbohydrate antigens and the content of epidermal growth factor receptor (EGF-R), the carcinoembryonic antigen (CEA) and the alpha-fetoprotein (AFP) in colorectal and liver tumors were determined. These included 30 large bowel adenocarcinomas (7 colon, 6 sigma, 5 caecum, 12 rectum), 12 hepatocellular carcinomas and 6 liver metastases. Histologically normal tissue excised along with the tumors were used as controls. All plasma membranes studied showed specific EGF binding, and tumor plasma membranes had an EGF receptor level higher than that of the normal count

Factors influencing the application of transrectal natural orifice specimen extraction performed laparoscopically for colorectal cancer: A retrospective study.

BACKGROUND: A few factors influence the feasibility of transrectal natural orifice specimen extraction (NOSE) surgery for colorectal cancers. However, little is known about the underlying factors of NOSE surgery. METHODS: Consecutive patients with rectal and sigmoid colon cancers treated laparoscopically between January 2014 and April 2017 were enrolled in this study. The transrectal NOSE performed laparoscopically was the first choice of all patients. When NOSE failed, the specimen was removed through a midline abdominal wall incision. Univariate and multivariate logistic regression analyses

Multiple metastases of bones and sigmoid colon after mastectomy for ductal carcinoma in situ of the breast: a case report.

BACKGROUND: The prognosis of ductal carcinoma in situ (DCIS) is reportedly well. Extremely rare patients with DCIS develop distant breast cancer metastasis without locoregional or contralateral recurrence. This is the first report of multiple bones and sigmoid colon metastases from DCIS after mastectomy. CASE PRESENTATION: A 43-year-old woman was diagnosed with DCIS, and she received mastectomy, followed by endocrine therapy and target therapy. During the following-up, convulsions and pain on the legs were complaint. Therefore, Computed Tomography (CT) on bones and positron emission tomograph

Natural History of Pancreatic Ductal Adenocarcinoma Diagnosed During Observation of Other Organ Cancers.

BACKGROUND Pancreatic ductal adenocarcinoma (PDAC) is a rapidly progressive malignancy that exhibits an extremely poor prognosis, with most cases diagnosed at an advanced stage. To date, few reports have explored the natural history of PDAC, and the period leading up to the detection of PDAC as a tumor with contrast-enhanced computed tomography (CECT) remains unclear. Here, we report 3 PDAC cases diagnosed incidentally by repeating imaging examinations during observation of other organ cancers. CASE REPORT Two patients were undergoing postoperative follow-up for colorectal cancer; owing to the
