Propriospinal myoclonus at sleep onset.

AIMS: To describe the clinical and polygraphic features of propriospinal myoclonus (PSM) at sleep onset. MATERIAL AND METHODS: PSM was first described in 1997 in patients with jerks occurring in the relaxation period preceding sleep. EMG showed jerks to arise in spinally innervated muscles, propagating thereafter to rostral and caudal muscles at a low speed, typical of propriospinal pathways. RESULTS: PSM arose when EEG alpha activity spread over the scalp and disappeared during either active wakefulness or actual sleep. In some patients EMG activity could sometimes remain localized to the a…

Familial benign neonatal sleep myoclonus.

Benign neonatal sleep myoclonus is a nonepileptic syndrome characterized by myoclonic jerks during sleep. It occurs in the first weeks of life and disappears, in most cases, within 3 months. There are no sequelae, and psychomotor and cognitive development are normal. The syndrome is usually sporadic; only a few familial cases have been reported in the literature. This case report describes three members of a single family with benign neonatal sleep myoclonus and discusses its differentiation from other types of myoclonus.

[Sleep disorders in Parkinson syndromes].

About 90% of neurodegenerative diseases with parkinsonism are associated with sleep disorders including daytime sleepiness, sleep-related breathing disorders and parasomnias. It is hard to define what ratio of insomnia and daytime hypersomnia is caused by the antiparkinsonian treatment, by the somatic and mental-emotional symptoms of the neurodegenerative disease and by the neurodegenerative brain process itself. Recent research suggests that the latter group is more important than expected. In Parkinson syndromes the structures included in sleep regulation--mainly within the brainstem--are al…

[Sleep disorders in prion diseases].

Prion diseases are a group of encephalopathies with neurodegenerative changes caused by an altered protein named prion whose characteristic datum is transmissibility. In most cases they occur in a sporadic form although a group of them are familial associated with mutations in the gene of the prion protein. Genetic polymorphism seems to determine the different family variants. One of the most enigmatic and unusual is Fatal Familial Insomnia (FFI), a hereditary disorder characterised by loss of physiological sleep with oneiric stupor, autonomic and motor hyperactivity, and motor anomalies. The …

[Cigarette smoking and sleep disturbance].

INTRODUCTION: Sleep disturbances may make smoking cessation more difficult and relapses more frequent. STATE OF THE ART: Nicotine increases vigilance and decreases sleeping time. Cigarette smoking is associated both with difficulty initiating and maintaining sleep. The disruption is associated with arousals on EEG recordings. Smoking promotes snoring and obstructive sleep apnoea. Insomnia is recognised as one of the criteria for nicotine withdrawal syndrome. Awakening from sleep and daytime sleepiness can occur frequently in this situation and are predictive factors for relapse. Nicotine repl…

Pharmacological management of sleep apnoea.

Obstructive sleep apnoea poses a significant health hazard that is associated with leading causes of mortality and morbidity. Nasal continuous positive airway pressure is the primary treatment modality, with surgical treatments as alternatives. Oral appliances and pharmacological therapy remain adjunctive modalities. Non-specific treatments include weight loss, postural therapy and behavioural measures. Pharmacotherapy goals include the reduction of risk factors for sleep apnoea; correction of underlying predisposing metabolic diseases, such as hypothyroidism or acromegaly; treatment of associ…

Gender differences in sleep disorders.

PURPOSE OF REVIEW: To evaluate recent evidence regarding gender differences in sleep. RECENT FINDINGS: Women have better sleep quality compared with men, with longer sleep times, shorter sleep-onset latency and higher sleep efficiency. Despite this, women have more sleep-related complaints than men. The amount of slow-wave sleep decreases with age in men and women. Normal physiologic periods, including puberty, menstruation, pregnancy, and menopause, are associated with alterations in sleep patterns. Gender differences in normal sleep may underlie the observed differences in risk of sleep dis…

Euthyroid goitre and sleep apnea.

A number of predisposing factors (obesity, nasal obstruction, adenoidal hypertrophy, macroglossia, etc) have been related to obstructive sleep apnea syndrome (OSAS). In addition hypothyroidism and large goitres have been reported to be associated to OSAS, but this association has not been adequately studied. We describe an obese patient with euthyroid goitre associated with OSAS. The patient showed a body mass index (BMI) of 47 and a large neck with a circumference of 60 cm. The flow-volume curve demonstrated an expiratory plateau suggesting an intrathoracic upper airway obstruction. Arterial …

Surgery for obstructive sleep apnoea.

BACKGROUND: Obstructive sleep apnoea/hypopnoea syndrome(OSAHS) is the periodic reduction or cessation of airflow during sleep. The syndrome is associated with loud snoring, disrupted sleep and observed apnoeas. Surgery for obstructive sleep apnoea/hypopnoea syndrome aims to alleviate symptoms of daytime sleepiness, improve quality of life, and reduce the signs of sleep apnoea recorded by polysomnography. OBJECTIVES: The objective of this review was to assess the effects of any type of surgery for the treatment of the symptoms of obstructive sleep apnoea/hypopnoea syndrome in adults. SEARCH S…

Sleep quality in hospitalized patients.

AIMS AND OBJECTIVES: The objective of this study was to evaluate and compare sleep quality of the hospitalized patients and matched healthy controls. BACKGROUND: Although the functions of sleep are not clearly understood, it is generally accepted that it is necessary for the maintenance of good health. Hospitalized patients' sleep may not be refreshing or restorative. The reasons for this can be categorized into three groups: environmental, physiological and psychological. DESIGN AND METHODS: This research was conducted at the Cumhuriyet University Hospital in Turkey. One hundred and fifty h…

Who has time to sleep?

BACKGROUND: Both marriage and education appear to confer a protective effect on health. Few studies have examined the extent to which both undersleeping and oversleeping explain these relationships. I examined whether marital status, educational background, and other sociodemographic variables are associated with higher-risk sleep durations. METHODS: Over 7000 individuals aged 25-64 collected time-use diary data during a 24-h period. For both weekdays and weekend days, separate multinomial logistic regression models were estimated using three categories of sleep duration as the outcome catego…

Sleep issues in Parkinson's disease.

Sleep disturbances in patients with Parkinson's disease (PD) are common, are often severe, and are typically underrecognized and ineffectively treated. After the recognition that some patients with PD could fall asleep unexpectedly when driving, with resulting dangerous consequences, it became evident not only that PD medications might be partly responsible but that there were many additional factors contributing to sleep disturbance in these patients. This review discusses the myriad sleep disorders seen in patients with PD, presents their diagnostic features, and gives recommendations on the…

Pathophysiology of obstructive sleep apnea.

This article explores the physiologic basis and symptoms of obstructive sleep apnea--a general term encompassing central sleep apnea and obstructive sleep apnea. The former is relatively uncommon while the latter is much more common. Episodic collapse and blockage of the upper airway occur during sleep despite continuous respiratory effort. Three types of sleep obstructive breathing--apnea, hypopnea, and airway resistance--are associated with respiratory-related arousals from sleep.

Pathogenesis of obstructive sleep apnea.

The pathogenesis of obstructive sleep apnea (OSA) has been under investigation for over 25 years, during which a number of factors that contribute to upper airway (UA) collapse during sleep have been identified. Structural/anatomic factors that constrict space for the soft tissues surrounding the pharynx and its lumen are crucial to the development of OSA in many patients. Enlargement of soft tissues enveloping the pharynx, including hypertrophied tonsils, adenoids, and tongue, is also an important factor predisposing to UA collapse, inasmuch as this can impinge on the pharyngeal lumen and nar…
