Improving the detection of infectious diseases in at-risk migrants with an innovative integrated multi-infection screening digital decision support tool (IS-MiHealth) in primary care: a pilot cluster-randomized-controlled trial.

BACKGROUND: There are major shortfalls in the identification and screening of at-risk migrant groups. This study aims to evaluate the effectiveness of a new digital tool (IS-MiHealth) integrated into the electronic patient record system of primary care centres in detecting prevalent migrant infections. IS-MiHealth provides targeted recommendations to health professionals for screening multiple infections, including human immunodeficiency virus (HIV), hepatitis B and C, active tuberculosis (TB), Chagas disease, strongyloidiasis and schistosomiasis, based on patient characteristics (including va…

Systematic Review of Strongyloides stercoralis Infection Diagnosis in Southeast Asia: Insights from Parasitological, Molecular, and Serological Approaches.

Accurate diagnosis of strongyloidiasis is crucial for effective treatment and prevention of complications. We reviewed the current landscape of diagnostic assays used in detecting Strongyloides infection in Southeast Asia. A literature search was performed using Scopus, PubMed, and Web of Science databases spanning the last three decades. Based on the exclusion and inclusion criteria, 52 papers were included in this review. We outlined the diagnostic methods used and their advantages and drawbacks. Insensitive parasitological methods were commonly used, thus underscoring the underestimation of…

The Unique Life Cycle of Strongyloides stercoralis and Implications for Public Health Action.

Strongyloides stercoralis has one of the most complex life cycles of the human-infecting nematodes. A common misconception in medical and pu … tion in the host, the profound pathology occurring in hyperinfection and systemic manifestations of strongyloidiasis, and (b) a limited external cycle with a single generation of free-living adults. This paper aims to review and discuss original research on the unique life cycle of S. stercoralis that distinguishes it from other helminths and highlight areas where …

Hyperinfection of Strongyloides stercoralis in an immunocompetent patient.

Strongyloides stercoralis is an intestinal nematode causing endemic infection, mostly in immunocompromised individuals, in tropical and subt … ee occasions showed plenty of S. stercoralis larvae. Patient responded well to albendazole therapy. Strongyloidiasis is not always associated with compromise in immune status. It should be suspected in immunocompetent individuals with history of long-term diarrhea and weight loss.

Immune reconstitution disease associated with parasitic infections following antiretroviral treatment.

HIV-associated immune reconstitution disease (IRD) is the clinical presentation or deterioration of opportunistic infections that results fr … aniasis (visceral, cutaneous, mucosal and post kala azar dermal leishmaniasis), schistosomiasis and strongyloidiasis. Since the major burden of HIV lies in resource-limited countries where access to ART is now rapidly expanding, increased awareness and knowledge of these phenomena is important. Here we review the clinical spectrum and pathogenesis of IRD associate …

The mutual influence of nematode infection and allergy.

Several studies have now shown that the prevalence of helminth infections is negatively correlated with the prevalence and/or severity of allergic diseases. Here, we describe studies in rodents infected with Strongyloides venezuelensis examining the mutual influence of nematode infection and allergy. S. venezuelensis has a lung cycle, much akin to the human hookworm and Strongyloidiasis, and induces airway eosinophilia, local IgE and mucus production, and airway hyperreactivity. Both the Th2 and functional responses are relevant for the ability of rodents to deal with S. venezuelensis infectio…

Ivermectin in human medicine.

Ivermectin is a potent macro-cyclic lactone causing paralysis in many nematodes and arthropods through an influx of chloride ions across cell membranes. It is currently the drug of choice for human onchocerciasis and shows potent microfilaricidal activity against the other major filarial parasites of human (Wuchereria bancrofti, Brugia malayi, Loa loa and Mansonella ozzardi) but not against M. perstans. Whether or not it also kills the adult-stage of these parasites is currently under study. Ivermectin also has excellent efficacy in both human strongyloidiasis and cutaneous larva migrans for w…

Intestinal helminthic and protozoal infections and urinary schistosomiasis in Egyptian children.

To extend information on the current distribution and frequency of parasitic infections in Egypt, one stool and one urine specimen from 2945 children, aged 6 to 12 years old were examined. The children were from 10 schools, one in Cairo, 2 in Giza Governorate, and 7 in the Nile delata. Frequency rates for S. mansoni by school were 0.3% in Cairo, 8-15% in Giza, and 3-79% in the delta; for S. haematobium, rates were 3% in Cairo, 25-71% in Giza, and 0-33% in the delta. Rates for strongyloidiasis, trichuriasis, and trichostrongliasis were low in each location never exceeding 1%. Frequency rates fo…

Evaluation of novel recombinant antigen-based (NIE/SsIR) immunochromatographic rapid tests for Strongyloides stercoralis: an accuracy study.

BACKGROUND: Strongyloidiasis is a chronic parasitic disease that results in relevant human morbidity, caused by the nematode Strongyloides stercoralis. This nematode has a unique and complex life-cycle. There is so far no perfect test for this helminthiasis. Rapid immunochromatographic tests (RDTs) are of interest, specifically due to their feasibility for use in the field, where public health control of strongyloidiasis is recommended. The aim of this study was to evaluate two novel RDTs, one detecting immunoglobulin (Ig) G and the other detecting IgG4, based on a combination of recombinant a…

Molecular diagnosis of Strongyloides stercoralis among transplant candidates.

Strongyloidiasis can occur without any symptoms or as a potentially fatal hyperinfection or disseminated infection, principally in immunosuppressed patients. Our study aimed to evaluate the application of conventional polymerase chain reaction (cPCR) and real-time PCR (qPCR). Polymerase chain reaction (PCR) and real-time PCR (qPCR) targeting the 18S rRNA gene for detection of Strongyloides stercoralis infection among transplant candidates were applied in stool samples obtained from 150 transplant candidates, preliminarily analyzed by parasitological methods. S. stercoralis larvae were visualiz…

Human T-cell lymphotropic virus-I in Latin America.

HTLV-1 infection is endemic in several Latin American countries. HTLV-1-associated myelophathy/tropical spastic paraparesis (HAM/TSP) and adult T-cell leukemia lymphoma (ATLL) are emerging diseases in the region. Documented risk factors for acquiring the virus include breast-feeding, contaminated blood transfusion, and sexual intercourse, all of which are amenable to prevention efforts. Strongyloides stercoralis hyperinfection syndrome and therapeutic failure in apparently healthy patients with nondisseminated strongyloidiasis may be markers of HTLV-1 infection. HTLV-1 co-infection may adverse…

[Initial assessment of immigrant patients in primary care].

The clinical interview of immigrant patients requires cultural competence to ensure good understanding and correct communication, in addition to collecting specific information that differs from that of native patients, such as origin and migratory route or cultural identity. Screening for latent tuberculosis infection is recommended in certain cases and screening for other infections, both cosmopolitan with a higher prevalence in migrants (HIV, syphilis, hepatitis B and C) and imported (Chagas, intestinal parasites, strongyloidiasis, schistosomiasis), depending on origin. It is essential to c…

Ivermectin concentration in breastmilk of a woman with Strongyloides stercoralis and human T-lymphotropic virus-I co-infection.

Ivermectin is a widely used drug for the treatment of various neglected tropical diseases, such as lymphatic filariasis, onchocerciasis, and strongyloidiasis among others. Despite its excellent safety profile, there are few published studies of the use of ivermectin in children, pregnant and nursing women. In the present study, we report clinical data on ivermectin concentrations in breastmilk of a woman with Strongyloides stercoralis and HTLV-I coinfection. Ivermectin levels in breastmilk ranged from 1.4 to 20.8 ng/ml, with a mean of 9.26 ng/ml after a single dose of 200 µg/kg. We estimated t…
