Staff perceptions of patient self-care in hemodialysis treatment tasks: a qualitative exploration using the Exploration Preparation Implementation Sustainment Framework.

BACKGROUND: People with end stage kidney disease receiving haemodialysis typically receive treatment in-center, where tasks are mainly performed by healthcare providers with limited patient participation. Incorporating patient participation in performing at least some of the haemodialysis care tasks can enhance safety, efficiency, and quality, yet is rarely performed. Implementing change requires a comprehensive strategy, addressing both patient and provider organizational factors. Using the Exploration Preparation Implementation Sustainment (EPIS) framework, we examined the factors that may a

Motor cortical functional connectivity changes due to short-term immobilization of upper limb: an fNIRS case report.

INTRODUCTION: A short-term immobilization of one hand affects musculoskeletal functions, and the associated brain network adapts to the alterations happening to the body due to injuries. It was hypothesized that the injury-associated temporary disuse of the upper limb would alter the functional interactions of the motor cortical processes and will produce long-term changes throughout the immobilization and post-immobilization period. METHODS: The case participant (male, 12 years old, right arm immobilized for clavicle fracture) was scanned using optical imaging technology of fNIRS over immobi

Relative independence of upper limb position sense and reaching in children with hemiparetic perinatal stroke.

BACKGROUND: Studies using clinical measures have suggested that proprioceptive dysfunction is related to motor impairment of the upper extremity following adult stroke. We used robotic technology and clinical measures to assess the relationship between position sense and reaching with the hemiparetic upper limb in children with perinatal stroke. METHODS: Prospective term-born children with magnetic resonance imaging-confirmed perinatal ischemic stroke and upper extremity deficits were recruited from a population-based cohort. Neurotypical controls were recruited from the community. Participan

Attempted Training of Alcohol Approach and Drinking Identity Associations in US Undergraduate Drinkers: Null Results from Two Studies.

There is preliminary evidence that approach avoid training can shift implicit alcohol associations and improve treatment outcomes. We sought to replicate and extend those findings in US undergraduate social drinkers (Study 1) and at-risk drinkers (Study 2). Three adaptations of the approach avoid task (AAT) were tested. The first adaptation - the approach avoid training - was a replication and targeted implicit alcohol approach associations. The remaining two adaptations - the general identity and personalized identity trainings - targeted implicit drinking identity associations, which are rob

AI-powered biomechanical modeling for ACL-reconstructed knees: predicting knee joint contact forces via computer vision and deep learning.

BACKGROUND: Patients undergoing anterior cruciate ligament reconstruction (ACLR) are at high risk of osteoarthritis or secondary injuries, with abnormal knee contact forces (KCFs) identified as a key factor in joint degeneration. Traditional KCF assessment relies on expensive lab systems while advances in computer vision and AI now enable low-cost alternatives. However, currently available methods oversimplify knee mechanics and neglect compensatory movements, highlighting the urgent need for intelligent, real-time monitoring tools for personalized rehabilitation. Therefore, the aim of this st

Stability and sensitivity analysis of effort-dependent CCFES for simulated hemiplegic hand assistance after stroke: computational model.

Objective.contralaterally-controlled functional electrical stimulation (CCFES) is an emerging intervention that has evidence for improving therapy for post-stroke hand hemiplegia more than cyclic stimulation (e.g. hand dexterity). Its key feature is that the amount of paretic hand opening assistance is controlled by the degree of non-paretic hand opening, which enables it to assist hand opening during task practice. However, current CCFES techniques do not prevent participants from reducing volitional effort, which is referred to as 'slacking' and does not benefit motor relearning. As purely a

Eye and hand coarticulation during problem-solving reveals hierarchically organized planning.

During everyday activities, such as preparing a cup of coffee or traveling across cities, we often plan ahead and execute sequences of actions. Yet how such planning unfolds when solutions must be formed from scratch, without external cues or routines, remains unclear. This study examines how participants coordinate gaze and cursor movements while solving path-tracing problems on a grid with multiple targets. By focusing on the action execution phase, we aimed to probe the structure of the gaze-cursor plans that participants used to solve the tasks. Our analysis reveals three main findings. Fi

Potential of Image2Image translation in reducing AI bias attributed to differences in CT reconstruction methods: proof-of-concept study on a paired dataset.

BACKGROUND: The choice of image reconstruction approach, such as Iterative Model Reconstruction (IMR) and iDose 4 , is critical in computed tomography (CT) as distribution shifts between them can introduce systematic bias in clinical AI models, posing a barrier to efficient, scalable AI deployment in radiology. While Image-to-Image (I2I) translation offers a path toward harmonization, efficient and translation-feasible AI in radiology demands that such approaches be validated not only on image quality metrics but also on downstream clinical task performance. METHODS: We compared three harmo

Hand-specific speed and accuracy optimization during early endoscopic forceps training: a comparative study of surgical aspirants and non-aspirants.

BACKGROUND: In endoscopic surgery, safe performance depends on functionally distinct roles of the dominant and non-dominant hands. However, skill acquisition is often evaluated using uniform, time-based metrics, which may obscure hand-specific learning patterns-particularly improvements in non-dominant-hand stability and accuracy. OBJECTIVE: To compare dominant- and non-dominant-hand learning patterns during a two-week endoscopic forceps training program between medical students with and without surgical career aspiration, focusing on speed-accuracy trade-offs and learning-direction transitio

CT-integrated pathophysiology-guided, clinically causal cognition-led interpretable lesion context characterization and risk reasoning in lung MWA.

BACKGROUND: Accurate preoperative prediction of complication risks in microwave ablation (MWA) for non-small cell lung cancer (NSCLC) is critical yet challenging due to the complex anatomical context. Beyond feature heatmaps, clinicians need decision rationales that align with pathophysiology. We therefore frame the task as an interpretable lesion-context characterization driven by clinical causal cognition, where imaging-derived concepts (e.g., tumor-pleura distance, vascular proximity, pleural adhesions) are organized into traceable causal pathways supporting downstream risk reasoning. Inhibiting the right dorsolateral prefrontal cortex selectively enhances unsupervised statistical learning.

The brain must balance the automatic extraction of regularities with top-down cognitive control, yet the hemispheric contributions of the dorsolateral prefrontal cortex (DLPFC) remain unresolved. Here, we applied inhibitory repetitive transcranial magnetic stimulation to the left, right, or bilateral DLPFC in 95 adults during a probabilistic sequence learning task. We found that inhibiting the right and bilateral DLPFC enhanced statistical learning compared to sham stimulation. Contrary to the hypothesis that interference from existing knowledge would mediate this effect, episodic memory perfo

A self attention based deep learning framework for accurate and efficient dental disease detection in OPG radiographs.

Oral diseases are increasing now-a-days and there is a high demand for the automatic diagnostic system that helps the clinician to detect these oral diseases with more accuracy and reduced human error. Utilizing the advancement of Deep Learning techniques, this study proposes a novel comparative approach for the diagnosis of teeth diseases using Orthopantomogram (OPG) images and recent transformer based architecture. Particularly, Vision Transformer (ViT) and Swin Transformer are employed for the development of the effective automatic system. Experimental results demonstrated that the Vision T

The role of age of arrival and language environment factors in Arabic heritage language development: A longitudinal study.

The Arabic development of Syrian refugee children (N = 133; mean age = 9;4 at Time 1) was examined over 3 time periods during their first five years in Canada. Children were administered sentence repetition and receptive vocabulary tasks in English and Arabic, and information about age-of-arrival (AOA), schooling in Arabic and language environment factors was obtained via parent report. Older AOA was associated with superior Arabic abilities across time, but regardless of AOA, children showed plateau/attrition patterns in Arabic and shifts to English dominance by Time 3. Increases in English o

Interbrain substrates of role switching during mother-child interaction.

Mother-child interaction is highly dynamic and reciprocal. Switching roles in these back-and-forth interactions serves as a crucial feature of reciprocal behaviors while the underlying neural entrainment is still not well-studied. Here, we designed a role-controlled cooperative task with dual EEG recording to explore how differently two brains interact when mothers and children hold different roles. When children were actors and mothers were observers, mother-child interbrain synchrony emerged primarily within the theta oscillations and the frontal lobe, which highly correlated with children's

[Legal and Practical Solutions for the Expanding the Roles of Medical Support Staff Nurses].

PURPOSE: Medical support staff nurses have traditionally performed various supportive tasks for physicians, often extending beyond standard nursing roles. Despite these long-standing practices, there is a notable lack of official recognition and legal protection for these expanded responsibilities, leading to increasing legal concerns. Therefore, there is a need for proposing a rational solution to address these issues. METHODS: The number of medical support staff nurses is rising, particularly as they fill gaps left by the 2024 resident physician strike. The study focuses on identifying pote

Daily-life affective instability in emotional distress disorders is associated with function and structure of posterior parietal cortex.

Affective instability (i.e., large and frequent shifts in negative emotions) is a key emotion dysregulation symptom in emotional distress disorders and can be reliably and validly assessed using ambulatory assessment. However, no study has examined whether affective instability is associated with brain function and structure. Using multimodal neuroimaging and ambulatory assessment, we examined associations between functional activation and cortical structure with ambulatory-assessed affective instability in emotional distress disorders (n = 27). Increased daily life-affective instability was a

Neurostimulation Reveals Context-Dependent Arbitration Between Model-Based and Model-Free Reinforcement Learning.

While it is established that humans use model-based (MB) and model-free (MF) reinforcement learning in a complementary fashion, much less is known about how the brain determines which of these systems should control behavior at any given moment. Here we provide causal evidence for a neural mechanism that acts as a context-dependent arbitrator between both systems. We applied excitatory and inhibitory transcranial direct current stimulation over a region of the left ventrolateral prefrontal cortex previously found to encode the reliability of both learning systems. The opposing neural intervent
