Ruptured Sinus of Valsalva Aneurysm: Use of Multimodality Imaging in Delineating Structure and Function.

Sinus of Valsalva aneurysm is a rare defect that can present with fatal complications in case of rupture. Heart failure symptoms are common due to high fraction of the resultant shunt. Multimodality imaging and invasive hemodynamic assessment is essential for comprehensive evaluation of the defect and guiding surgical planning. We describe the case of a 40-year-old woman who presented with heart failure symptoms and was found to have ruptured sinus of Valsalva aneurysm on transthoracic echocardiogram. Cardiac computed tomography angiography further characterized the defect and the associated a

Ischemic myocardial necrosis and papillary muscle dysfunction in infants and children.

Ischemic myocardial and papillary muscle dysfunction has considerable implication in newborn infants and children with normal or malformed hearts. Papillary muscle dysfunction in adults primarily involves coronary artery occlusion and ischemic necrosis in the left ventricle and papillary muscles. Infants and children rarely develop coronary artery occlusion. Their myocardial dysfunction and injury occurs with nearly equal frequency in both ventricles as a result of underperfusion from a wide range of causes, including severe birth asphyxia, congenital heart disease, and complications of premat

Reoperation after initial correction of tetralogy of Fallot.

Ten of 357 patients (2.8%) who had total correction of tetralogy of Fallot in our institute between 1955 and 1983 required intracardiac reoperation. The indication for reoperation included residual lesions alone or in combination with other lesions. The reoperation consisted of a new patch closure of a residual ventricular septal defect in nine, reconstruction of a residual right ventricular outflow tract obstruction in five, repair of a left ventricle-right atrium communication in two, repair of tricuspid insufficiency in two and closure of an atrial septal defect in one. None of these ten pa

Midterm Outcomes in a Pooled Cohort of Harmony Transcatheter Pulmonary Valve Recipients.

BACKGROUND: The Harmony transcatheter pulmonary valve (TPV) is designed to treat severe pulmonary regurgitation in the native or surgically repaired right ventricular (RV) outflow tract. Early outcomes after TPV replacement with the Harmony valve have been positive, but longer-term data are limited. METHODS: The study included patients who received a commercially available TPV22 or TPV25 device as part of the nonrandomized, prospective Native Outflow Tract Early Feasibility Study, Harmony Pivotal Trial, and Continued Access Study. Patients completed at least 3 years of follow-up, and outcomes

Stepwise approach for diagnosis and management of Takotsubo syndrome with cardiac imaging tools.

Takotsubo syndrome is featured by transient left ventricle dysfunction in the absence of significant coronary artery disease, mainly triggered by emotional or physical stress. Its clinical presentation is similar to acute coronary syndrome; therefore, cardiac imaging tools have a crucial role. Coronary angiography is mandatory for exclusion of pathological stenosis. On the other side, transthoracic echocardiography is the first non-invasive imaging modality for an early evaluation of left ventricle systolic and diastolic function. Left ventricle morphologic patterns could be identified accordi

Evaluation of an incidental cardiac finding in a patient with bronchitis.

UNLABELLED: CLINICAL INTRODUCTION: A 63-year-old man with HIV presented with 1 month of dyspnoea and productive cough without orthopnoea. He was afebrile with normal blood pressure, borderline tachycardia and mild pulmonary wheezing. He had exertional hypoxia requiring 4 L per minute of oxygen. No murmurs, jugular venous distention or lower extremity oedema was noted. Clinical evaluation included transthoracic echocardiography (TTE), which showed mild left ventricular hypertrophy with normal size and systolic function. The right ventricle was normal. There was mild aortic insufficiency; other

First Successful Cardiac Myosin Inhibitor Use for Donor-Derived HCM After Heart Transplant.

BACKGROUND: Donor-derived hypertrophic cardiomyopathy (HCM) is an exceptionally rare posttransplant complication, traditionally diagnosed years after transplant. CASE SUMMARY: A 55-year-old woman received a heart from a 30-year-old man donor who suffered sudden cardiac death. On postoperative day 1, she developed severe dynamic left ventricular outflow tract obstruction (peak gradient, 110 mm Hg). Despite calcium channel blocker therapy and later beta-blockade, she remained in NYHA functional class III with persistent gradients. Cardiac magnetic resonance imaging 5 months posttransplant confi

Systolic Anterior Motion After Mitral Valve Repair: Echocardiographic Prediction, Surgical Prevention and Perioperative Management.

Systolic anterior motion (SAM) of the mitral valve remains a clinically relevant complication after mitral valve repair and may result in dynamic left ventricular outflow tract (LVOT) obstruction, SAM-associated mitral regurgitation, and hemodynamic instability. Despite advances in surgical techniques and perioperative imaging, SAM remains an important cause of difficult separation from cardiopulmonary bypass and postoperative circulatory compromise. The development of SAM is multifactorial and results from the interaction between mitral valve anatomy, ventricular geometry, surgical repair cha

Altered intercellular communication and extracellular matrix signaling as a potential disease mechanism in human hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is considered a primary disorder of the sarcomere resulting in unexplained left ventricular hypertrophy but the paradoxical association of nonmyocyte phenotypes such as fibrosis, mitral valve anomalies and microvascular occlusion is unexplained. To understand the interplay between cardiomyocyte and nonmyocyte cell types in human HCM, single nuclei RNA-sequencing was performed on myectomy specimens from HCM patients with left ventricular outflow tract obstruction and control samples from donor hearts free of cardiovascular disease. Clustering analysis based on

Obstructive Hypertrophic Cardiomyopathy and Interarterial Anomalous Aortic Origin of the Right Coronary Artery (AAORCA) Presenting With Syncope: Multimodal Diagnostic Assessment and Individualized Management.

The coexistence of obstructive hypertrophic cardiomyopathy (oHCM) and anomalous aortic origin of the right coronary artery (AAORCA) with an interarterial course is exceptionally rare and poses a significant diagnostic and therapeutic challenge, particularly in patients presenting with syncope. We report the case of a 63-year-old man referred for evaluation of a sudden syncopal episode that occurred at rest while he was seated on an airplane, followed by a fall on standing. Transthoracic echocardiography revealed asymmetric septal hypertrophy, systolic anterior motion of the mitral valve. and a

Association of left ventricular outflow tract size with arch morphology in interrupted aortic arch.

OBJECTIVES: Left ventricular outflow tract obstruction (LVOTO) is a major cause of morbidity and mortality in infants with interrupted aortic arch (IAA). Left Ventricular Outflow Tract (LVOT) development may be flow-mediated, thus IAA morphology may influence LVOT diameter and subsequent reintervention. We investigated the association of IAA morphology [type and presence of aortic arch aberrancy (AAb)] with LVOT diameter and reintervention. METHODS: All surgical patients with IAA (2001-2022) were reviewed at a single institution. We compared IAA-A versus IAA-B; IAA with aortic AAb versus none

Aortic Stenosis: New Insights in Diagnosis, Treatment, and Prevention.

Aortic stenosis (AS) is one of the most common valvular heart diseases and the number of patients with AS is expected to increase globally as the older population is growing fast. Since the majority of patients are elderly, AS is no longer a simple valvular heart disease of left ventricular outflow obstruction but is accompanied by other cardiac and comorbid conditions. Because of the significant variations of the disease, identifying patients at high risk and even earlier detection of patients with AS before developing symptomatic severe AS is becoming increasingly important. With the proven

BATMAN-Assisted Valve-in-Valve TMVR With Planned Hemodynamic Support in Severe Pulmonary Hypertension.

BACKGROUND: Left ventricular outflow tract (LVOT) obstruction is a feared complication of transcatheter mitral valve replacement (TMVR), and balloon-assisted translocation of the mitral anterior leaflet (BATMAN) has emerged as a mitigation strategy. Patients with severe pulmonary hypertension are at risk, as abrupt elimination of mitral regurgitation (MR) can destabilize hemodynamics. CASE SUMMARY: A 78-year-old woman with a degenerated 25-mm surgical bioprosthesis, severe pulmonary hypertension (mean pulmonary artery pressure 65 mm Hg), and reduced cardiac index (1.9 L/min/m2) underwent tran

Effects of sex on clinical outcomes of hypertrophic cardiomyopathy in Singapore.

INTRODUCTION: Despite the growing recognition that sex can affect the presentation and outcomes in hypertrophic cardiomyopathy (HCM), this relationship is understudied in Asians. Therefore, we aimed to explore sex differences in Asian patients with HCM. METHOD: A total of 295 consecutive patients diagnosed with HCM were recruited from a tertiary cardiology centre from 2010 to 2017 over a mean of 3.9±2.7 years. We evaluated the effects of sex on the outcomes of HCM in Asian patients. RESULTS: HCM patients were more commonly men (72%). Women were older and had more comorbidities, including hyp

Prognostic Value of CTA-Derived Left Ventricular Mass in Neonates with Congenital Heart Disease.

For therapeutic decisions regarding uni- or biventricular surgical repair in congenital heart disease (CHD), left ventricular mass (LVM) is an important factor. The aim of this retrospective study was to determine the LVM of infants with CHD in thoracic computed tomography angiographies (CTAs) and to evaluate its usefulness as a prognostic parameter, with special attention paid to hypoplastic left heart (HLH) patients. Manual segmentation of the left ventricular endo- and epicardial volumes was performed in CTAs of 132 infants. LVMs were determined from these volumes and normalized to body sur

Interventional Procedures for Left Ventricular Assist Device-Associated Complications.

As patients on long-term left ventricular assist device (LVAD) face a substantial risk for open cardiac reoperation, interventional treatment approaches are becoming increasingly important in this population. We evaluated data of 871 patients who were on LVAD support between January 1, 2016 and December 1, 2020. Interventional treatments for LVAD-associated complications were performed in 76 patients. Seventeen patients underwent transcatheter aortic valve replacements (TAVR) and 61 patients underwent outflow graft interventions (OGI). TAVR improved symptoms in patients with severe symptomatic

Conal anatomy in 119 patients with d-loop transposition of the great arteries and ventricular septal defect: an echocardiographic and pathologic study.

OBJECTIVES: We sought to study the range of conal morphology in transposition of the great arteries with ventricular septal defect and their embryologic and surgical implications. BACKGROUND: Conal anatomy in transposition of the great arteries and ventricular septal defect is variable and might affect surgical repair. METHODS: Conal anatomy was explored using two-dimensional echocardiography in 119 patients with transposition of the great arteries and a large ventricular septal defect who presented between 1984 and 1991. The influence of conal anatomy on surgical technique was determined by

Adalimumab: in Crohn's disease.

PURPOSE: Sex differences in long-term outcomes of patients with repaired tetralogy of Fallot (TOF) are increasingly recognised. However, the sex-specific timing of re-intervention is still unclear. METHODS: A systematic review and meta-analysis was performed across six electronic databases from inception to February 2025. Inclusion criteria were studies that reported outcomes of sex differences in right ventricular (RV) volume, function, and right ventricular outflow tract obstruction (RVOTO) in patients after repaired TOF. Studies were identified and data were extracted by two independent re

Congenital diaphragmatic hernia and associated cardiovascular malformations: type, frequency, and impact on management.

The co-occurrence of congenital diaphragmatic hernia (CDH) and cardiovascular malformations (CVMs) has important clinical, genetic, and developmental implications. Previous examinations of this topic often included patients with genetic syndromes. To correct this potential bias, we undertook an extensive review of the literature and obtained new data. The frequency of CVMs associated with isolated CDH was 11-15%. A careful analysis of CVMs indicates that atrial and ventricular septal defects, conotruncal defects, and left ventricular outflow tract obstructive defects were the most common type
