Impact of alignment to gravito-inertial force on motion sickness and cardiopulmonary variables.

INTRODUCTION: In tilting trains partial alignment to the gravito-inertial force (GIF) in the curves seems to be the best tilt compensation to reduce the incidence of motion sickness. We investigated the effect of alignment to the GIF on the development of motion sickness during low-frequency horizontal motion. METHODS: There were 12 healthy subjects who participated. The design was a three-period, single-blind, crossover trial, counterbalanced for order. Cardiopulmonary measurements, Misery SCores (MISC), and questionnaire data (Motion Sickness Susceptibility Questionnaire, Nijmegen Questionn

Off vertical axis rotation motion sickness and field dependence.

BACKGROUND: Our study probed the relationship between field dependence and the development of nausea in light and dark during whole-body, off vertical axis rotation (OVAR). METHODS: There were 24 subjects who underwent OVAR at 0.2 Hz, 18 degree tilt. Exposures were undertaken in both light and darkness in sessions spaced 5 d apart in balanced order design. During rotation, nausea was rated at 1-min intervals to a cut off at 20 min or a level of 'moderate nausea' was attained, at which point motion stopped. Before and after OVAR sessions field dependence was rated with the rod and frame test (

Spontaneous pneumocephalus after commercial air travel complicated by meningitis.

BACKGROUND: Pneumocephalus usually results from trauma, infection, neoplasm, or iatrogenic causes. Barotrauma-induced spontaneous pneumocephalus is extremely rare, usually seen in divers or occassionally with air travel. CASE REPORT: We report a case of a 61-yr-old female presenting with confusion, fever, and respiratory failure one day after developing sudden nausea, vomiting, and headache during descent on a commercial airliner. Pneumocephalus and meningitis were present on admission. Sinus computed tomography (CT) showed pansinusitis and a tiny bone defect in the posterior wall of the righ

Exacerbation of pneumomediastinum after air travel in a patient with dermatomyositis.

BACKGROUND: Although the presence of pneumothorax is generally considered an absolute contraindication to air travel, reports on pneumomediastinum after air travel are extremely rare. Moreover, to the best of our knowledge, exacerbation of existing pneumomediastinum after commercial air travel has never been reported. CASE REPORT: We report on a case of a patient (the first case that we are aware of) who suffered exacerbation of pneumomediastinum after commercial air travel. This patient, with confirmed pneumomediastinum before air travel, flew to our city for medical care without being warne

Paranasal sinus mucosal hypertrophy in experienced divers.

INTRODUCTION: Due to the increasing popularity in recreational diving long-term health consequences of this sport have aroused interest in the diving community. Although not as important as dysbaric osteonecrosis, ischemic brain lesions, or neurophysiological symptoms, paranasal sinus mucosal hypertrophy (PSMH) has also been discussed as being more prevalent among divers. In this study, we used magnetic resonance imaging (MRI) to compare asymptomatic divers with control subjects for PSMH. METHODS: We enrolled 79 male divers (34.0 +/- 5.7 yr) and 79 nondiving age- and sex-matched controls (33.

Vestibular schwannoma and fitness to fly.

INTRODUCTION: When a pilot is referred for vestibular schwannoma (VS), his or her fitness to fly may be questioned. The objective of this retrospective study was to describe a series of VS cases in a pilot population and to discuss their fitness to fly options. METHODS: Between September 2002 and March 2010, the ENT/Head and Neck Surgery Department of the National Pilot Expertise Center conducted nearly 120,000 expert consultations for 40,000 pilots. We examined the files of 10 pilots who were referred to our 2 national experts for VS. RESULTS: At the time of the expert consultation, hypoacu

Decompression from saturation using oxygen: its effect on DCS and RNA in large swine.

INTRODUCTION: The use of hyperbaric oxygen (HBO) to expedite decompression from saturation has not been proven and may increase risk of toxicity to the pulmonary system. To evaluate any benefit of HBO during decompression, we used a 70-kg swine model of saturation and examined lung tissue by microarray analysis for evidence of RNA regulation. METHODS: Unrestrained, non-sedated swine were compressed to 132 fsw (5 ATA) for 22 h to achieve saturation. Animals then underwent decompression on air (AirD) or HBO (HBOD) starting at 45 fsw (2.36 ATA). Animals were evaluated for Type I and Type II deco

Suspected arterial gas embolism after glossopharyngeal insufflation in a breath-hold diver.

INTRODUCTION: Many competitive breath-hold divers employ the technique of glossopharyngeal insufflation in order to increase their lung gas volume for a dive. After a maximal inspiration, using the oral and pharyngeal muscles repeatedly, air in the mouth is compressed and forced into the lungs. Such overexpansion of the lungs is associated with a high transpulmonary pressure, which could possibly cause pulmonary barotrauma. CASE REPORT: We report a case of transient neurological signs and symptoms occurring within 1 min after glossopharyngeal insufflation in a breath-hold diver. He complained

Neurologic decompression sickness following cabin pressure fluctuations at high altitude.

Decompression sickness (DCS) occurs in diving, altitude chamber exposures, and unpressurized or depressurized high-altitude flights. Because DCS takes many forms, in-flight cases may be misinterpreted as hypoxia, hyperventilation, or viral illness, with resulting failure to respond appropriately. In this case, a 28-yr-old male pilot of a single-seat, tactical aircraft experienced 12 rapid pressure fluctuations while flying at 43,000 ft above sea level. He had no symptoms and decided to complete the flight, which required an additional 2 h in the air. Approximately 1 h later he began to experie

Acceleration tolerance after ingestion of a commercial energy drink.

BACKGROUND: Caffeine ingestion has been demonstrated to increase physical performance in some situations. This study examined the ability of a commercial energy drink containing caffeine to enhance acceleration tolerance and strength under G load. METHODS: Eight experienced centrifuge subjects completed three separate experimental acceleration exposures following ingestion of 11.5 ml x kg(-1) bodyweight of (1) a commercial energy drink, providing 5.0 mg caffeine/kg bodyweight; (2) a commercial energy drink without caffeine; or 3) a placebo. The acceleration exposures consisted of a relaxed gr

Motion sickness with combined lateral and roll oscillation: effect of percentage compensation.

BACKGROUND: Both lateral acceleration and roll through the force of gravity produce lateral forces. On a tilting-train the tilt offsets lateral acceleration so as to improve the physical comfort of passengers, but motion sickness is believed to increase as the lateral force is reduced by increased roll (i.e., as the percentage roll compensation is increased). OBJECTIVES: We investigated how motion sickness caused by combined lateral acceleration and roll displacement depended on the percentage compensation. METHOD: There were 8 groups of 20 subjects who were exposed for up to 30 min to vario

Vestibular evoked myogenic potentials and saccular plasticity in divers.

INTRODUCTION: Otolith function, which is dependent on linear velocity and acceleration, may be expected to change in underwater divers, who are submerged in a medium that is denser than air. The purpose of the present study was to examine possible changes in the sacculocollic reflex of professional divers and to investigate whether there might be diving-induced adaptation of the saccular response. METHODS: We used the vestibular evoked myogenic potential (VEMP) response to evaluate saccular function in 12 professional divers shortly after a dive and after an interval of at least 24 h. The con

Head movements and simulator sickness generated by a virtual environment.

BACKGROUND: For virtual environments (VEs) to be useful in training it is important to understand the negative effects of VE exposure. The purpose of this study was to observe head movements in virtual and real environments and the relation between those movements and simulator sickness. METHOD: There were 48 men who were trained in 4-person teams in either a head-mounted display (HMD)-based VE or in a real world environment. Head position data were collected in both systems and simulator sickness scores were collected in the VE. The relationship of head movements and sickness scores in theVE

Sacral meningocele in a military pilot, presenting during flight.

This study describes an unusual case of a military pilot with sacral meningocele that contained cerebrospinal fluid and presented as episodes of gluteal neuralgia during flight. The patient, a 38-yr-old male pilot, had complained of a dull and mild low lumbar pain over the previous 10 mo. These pains were exacerbated and radiated to the left gluteal region during flight. The patient's history, clinical examination, imaging findings, and treatment are reported. CT and MRI imaging revealed an unusual case of sacral meningocele (2.2 cm x 3.6 cm X 5.8 cm). These lesions can progress steadily in si

Cardiovascular and thermal responses to repeated head-up tilts following exercise-induced heat stress.

BACKGROUND: We evaluated the acute cardiovascular and thermoregulatory responses to repeated 70 degrees head-up tilts (HUT) performed following exercise-induced hyperthermia. METHODS: Eight male subjects underwent intermittent episodes of 70 degrees HUT in either a non-exercise/ non-heat stress state (NH) or an exercise-induced hyperthermic state (EIH). Subjects remained supine for 30 min in a thermoneutral environment (22 degrees C) and were subsequently exposed to three successive 6-min 70 degrees head-up tilts (HUT1, HUT2, HUT3), each separated by 10 min of supine resting. During EIH, subj

Contact urticaria from an oxygen mask in a military pilot.

Contact urticaria is a transient, wheal-and-flare response at the site of contact with a substance. We report a case of a military fighter pilot, with a background history of seborrheic dermatitis and steroid-induced facial skin atrophy, who subsequently developed contact urticaria from an oxygen facemask. Skin protection products such as barrier creams were used to try and minimize the provocation of contact urticaria from the oxygen mask whenever he flew, but without success. He was eventually permanently restricted from flying fighter aircraft.

Ultrasound evaluation of sinus fluid levels in swine during microgravity conditions.

BACKGROUND: Acute rhinosinusitis is a common problem that could occur in space secondary to absence of gravity-dependent drainage or odontogenic or external sources of infection. The purpose of this study was to determine the efficacy of ultrasound to determine sinus fluid distribution levels in swine and to assess the accuracy of ultrasound in the animal during normal and microgravity conditions. METHODS: Anesthetized swine had a catheter placed through a frontal bone window to allow aliquots of a viscous solution to be injected at 1 G (N = 4) or during brief microgravity parabolic flights (
