Various Authors underline that HIV positive women have a higher CIN incidence and a higher prevalence of HPV. Genital warts are common and are a sexually transmitted disease affecting all socioeconomic groups and life-styles. They are caused by the human papillomavirus (HPV) and are highly contagious. Genital warts are often symptomless, but occasionally cause itching, bleeding after intercourse burning and pain. Referral to a Genitourinary Medicine Clinic is recommended, where specialist treatment and counselling are available. Special care is needed with children or if the patient is pregnant. At the clinic, patients may be shown how to treat themselves at home with podophyllotoxin.

OBJECTIVE: To determine whether women who have a history of genital warts or whose sexual partners have such a history were more likely to have borderline or dyskaryotic cervical smears than other women.
DESIGN: Prospective study conducted over a five month period.
SETTING: A genitourinary medicine clinic in Cambridge, UK.
PATIENTS: One hundred and eighty five women who attended the clinic during the study period, on whom cervical cytology was performed. Ninety-seven had a history of genital warts and twenty had partners with genital warts.

After a brief introduction, the Authors report the results of a clinical-"peniscopic" study of the genital regions in 56 male patients who were partners of females affected with genital condylomatoses. The results obtained show the importance of examining the male genital region under high magnification (peniscopy), especially if we consider recent research which indicates the close relationship between HPV, VIN, CIN and carcinoma of the cervix.

Twenty male patients with penile warts (HPV induced papular and flat condylomata) attending the Venereal Disease Clinic at the Department of Dermatology, University di Pavia were treated with podophyllotoxin 0.5% in ethanol, self applied twice daily for 3 consecutive days, followed by 4 days without treatment. All the patients were cured after a maximum of 6 treatments; 10 patients (50%) were cured after 1 or 2 treatments. Benign cervical alterations causing complaints in 44 pregnant women were cured by cryosurgery in all three trimesters of their pregnancy. Recovery resulted in 41 cases after a single and in three extensive chondylomatous changes, after two treatments. The painless out-patient treatment did not influence pregnancy and the course of delivery. Cryosurgery has advantages over traditional surgical interventions, or using chemical caustic agents and it can be applied throughout pregnancy.

Plantar and common warts can be cured with great efficacy by the intralesional injection of bleomycin using the jet injector. Traditional keratolytic treatment and electro-desiccation are also still in use. Condyloma acuminata respond to podophyllin combined with cryotherapy. It is advisable to use the CO2 laser in more extensive infections. Interferon injections also may be helpful in these cases. Plane warts can be treated by tretinoin and 5-fluorouracil. In case of diffuse verruca sensitization with diphenylcyclopropenone can be considered.

Condyloma acuminatum of the vulva was diagnosed in 165 cases, of which 117 were identified during mass screening and 48 discovered in the outpatient clinic. The gross appearance and histologic characteristics were described. The lesions were removed surgically in 5 cases, and treated with topical 5-FU ointment in 4. The remaining cases were treated by cryosurgery. A permanent cure was achieved in all cases. The authors emphasize that the importance of condyloma acuminata should not be overlooked on account of its close relationship to vulvar cancer.

The effectiveness of interferon beta in the treatment of condylomata of the cervico-vaginal tract associated with CINs was evaluated in 25 patients. Human fibroblastic interferon (IFN beta) was administered by intra and perilesional infiltration at a dosage of 3 x 10(6) IU/day for 5 consecutive days the first week and every other day the second. The treatment showed: complete regression in 61.76% of CIN I and 58.69% of CIN II; partial regression in 26.47% of CIN I and 21.73% of CIN II. The picture remained unvaried in 11.76% of CIN I and 19.56% of CIN II.
