Same disease, different outcomes: a retrospective cohort study of COVID-19-associated AKI across Brazil's dual-tiered healthcare system.

INTRODUCTION: Acute kidney injury (AKI) is a critical complication of COVID-19, yet disparities in outcomes between public and private healthcare systems remain underexplored. Brazil's two-tiered healthcare system offers a unique setting to evaluate how resource allocation impacts AKI outcomes. This study compares AKI epidemiology in COVID-19 patients treated in two Brazilian hospitals, one publicly governed and the other of privately governed. METHODS: This retrospective cohort study analyzed 2,333 ICU patients with RT-PCR-confirmed COVID-19 (public 1,041; private 1,292, March 2020-April 202

Retrospective empirical analysis of the success rate of inguinal hernia operations in outpatient and inpatient sectors: a cohort study.

INTRODUCTION: In Germany, the proportion of outpatient surgeries was low during the study period (2014-2019) and low by international standards. Less than 20% of inguinal hernias are treated on an outpatient basis. Hybrid DRGs are intended to promote outpatient treatment, but their impact on the quality of care and referral criteria has not been sufficiently investigated empirically. METHODS: A retrospective analysis of routine data included 90,512 cases from 41 company health insurance funds, spanning 2014 to 2019. These were analysed descriptively in terms of care sector, age, and surgical

Agricultural Commodity Price Volatility and Adolescent Reproductive Health in Developing Economies: Pathways, Controversies, and Policy Priorities.

Agricultural commodity markets remain central to household survival across many developing economies, yet their volatility is rarely framed as an adolescent sexual and reproductive health problem. This mini review uses a structured narrative approach anchored in a screened evidence map of 1065 records, from which 50 papers were retained and 16 studies were prioritized for full synthesis. We define adolescent reproductive health holistically, including sexual agency, contraceptive information and use, pregnancy intention, antenatal and obstetric care, protection from coercion, and maternal and

An index-based system for early alerts of potential zoonotic disease outbreaks.

The Information System on Wildlife Health (SISS-Geo) is a free platform designed for the real-time collection of georeferenced data on wildlife health and environmental conditions via mobile devices. It serves as a collaborative tool, enabling health professionals, researchers, environmental managers, and the general public to report information about wildlife health occurrences directly within the system. Since its launch in 2014, SISS-Geo has been successfully applied in supporting decision-making during significant wildlife health events in Brazil. In this paper, we introduce a dynamic aler

Evaluating Rituximab Failure Rates in Neuromyelitis Optica Spectrum Disorder: A Nationwide Real-World Study From South Korea.

BACKGROUND AND PURPOSE: Treatments for neuromyelitis optica spectrum disorder (NMOSD) such as eculizumab, ravulizumab, satralizumab, and inebilizumab have significantly advanced relapse prevention, but they remain expensive. Rituximab is an off-label yet popular alternative that offers a cost-effective solution, but its real-world efficacy needs better quantification for guiding the application of newer approved NMOSD treatments (ANTs). This study aimed to determine real-world rituximab failure rates to anticipate the demand for ANTs and aid in resource allocation. METHODS: We conducted a nat

BMR-YOLO: A deep learning approach for fall detection in complex environments.

Traditional fall detection methods face significant limitations in complex environments, particularly under occlusion and poor lighting conditions. To address these challenges and enhance the detection accuracy in intelligent real-time monitoring systems, this study proposes an optimized BMR-YOLO framework based on the YOLOv8n architecture. Our approach introduces four strategic improvements to effectively overcome environmental constraints. First, we enhance the backbone network by incorporating a BiFormer vision transformer with dual-layer routing attention, enabling dynamic allocation of co

A population-based cohort study defined estimated glomerular filtration rate decline and kidney failure among Canadian immigrants.

The link between immigrant status, a key social determinant of health, and kidney disease remains uncertain. To evaluate this, we compared incident adverse kidney outcomes between immigrants and non-immigrants using Canadian provincial health administrative data. We conducted a population-based observational cohort study of all adult Ontario residents (immigrants and non-immigrants) with normal baseline kidney function (estimated glomerular filtration rate (eGFR) 70 mL/min/1.73m2 or more). Multivariable Cox proportional hazard regression modeling was used to evaluate the relationship between i

Modeling and control of Chikungunya with chronic infection.

Recognized globally as a major public health concern in the tropics and subtropics, Chikungunya fever also poses a potential epidemic risk in areas of China such as Guangdong Province, where suitable mosquito vector habitats exist. Based on a Chikungunya fever outbreak in Shunde District, Foshan City, this study develops a dynamical model incorporating a chronic infection stage. We derive R 0 and perform a thorough stability analysis of all equilibria. Using daily reported case data from Shunde District, model fitting yields estimates for three key transmission parameters (β, ρ 1, ρ 2), the to

The Tremendous Importance and Impact of Guidelines and Standardized Protocols in Dermatologic Ultrasound.

The emergence of dermatologic ultrasound as a new imaging subspecialty has created the need for evidence-based guidelines and standardized examination protocols. However, to date, it is common for institutions and codification systems to consider all soft tissue ultrasound examinations as essentially the same type of examination, including dermatologic exams. Unlike conventional soft tissue ultrasound examinations, dermatologic ultrasound usually requires a much longer time, and the mandatory use of color and/or power Doppler and pulsed Doppler. It includes an integrated clinical approach that

Food Insecurity and Sexually Transmitted Infections in the United States.

BACKGROUND: Individual food insecurity, defined as having limited or uncertain access to food of sufficient quality and quantity, is associated with increased sexual risk behavior and diagnosis of a sexually transmitted infection (STI), even after accounting for measures of socioeconomic status. Little is known about how food stress at the community level may exacerbate sexual and social vulnerabilities, or its potential association to a higher rate of STIs at the county level. METHODS: We conducted an ecological analysis of the association between county-level reported rates of chlamydia, go

Barriers to Mental Health Care in US Military Veterans.

BACKGROUND: Military veterans often encounter multiple obstacles to mental health care, such as stigma, practical barriers (e.g., high cost), and negative beliefs about mental health care. To date, however, nationally representative data on the prevalence and key correlates of these barriers to care are lacking. Such data are critical to informing population-based efforts to reduce barriers and promote engagement in mental health treatment in this population. METHODS: Data were analyzed from the National Health and Resilience in Veterans Study, which surveyed 4,069 US veterans, 531 (weighted

Advances in Addressing Patient Heterogeneity in Economic Evaluation: A Review of the Methods Literature.

Cost-effectiveness analyses commonly use population or sample averages, which can mask key differences across subgroups and may lead to suboptimal resource allocation. Despite there being several new methods developed over the last decade, there is no recent summary of what methods are available to researchers. This review sought to identify advances in methods for addressing patient heterogeneity in economic evaluations and to provide an overview of these methods. A literature search was conducted using the Econlit, Embase and MEDLINE databases to identify studies published after 2011 (date o
