For years the bladder epithelium has been the focus for diagnosing and treating the frequency, urgency, and pelvic pain of interstitial cystitis. However, many patients have not found symptom relief with these therapies. Pelvic floor dysfunction is often present in these patients, yet it is often untreated. Pelvic floor dysfunction may be related to abuse, past surgeries, or other causes. It is important to look outside of the bladder and explore other causes and treatments for this condition. Bladder interstitial cell (IC) is a cell, which lacks thick filaments and dense bodies but with incomplete basement membrane, rough endoplasmic reticulum and golgi apparatus. Patients with hypertonic pelvic floor disorders can present with pelvic pain or dysfunction. Each of the various syndromes will be discussed including elimination disorders, bladder pain syndrome/interstitial cystitis (BPS/IC), vulvodynia, vaginismus, and chronic pelvic pain. The symptoms and objective findings on physical examination and various diagnostic studies will be reviewed. Therapeutic options including physical therapy, pharmacologic management, and trigger point injections, as well as botulinum toxin injections will be reviewed in detail.

Women who have symptoms related to the genitourinary system present daily in a typical family medicine practice. The most common complaints are dysuria, increased urinary frequency, and incontinence. In the vast majority of cases the underlying problem is either urinary tract infection, urinary incontinence without infection, or bladder pain without infection that may be termed painful bladder syndrome/interstitial cystitis. This article discusses epidemiology, pathophysiology, risk factors, diagnosis, and treatment of these conditions.

Ten cases of sigmoid colocystoplasty carried out over a 19-year period have been reviewed. The indications were bladder gangrene (1), interstitial cystitis (2), bladder fibrosis (1), failed vesico-vaginal fistula (1), tuberculous contracture (2), atonic bladder (1) and invasive bladder carcinoma (2). All patients void with control every 2 to 4 h throughout the day and twice at night. There has been no long-term deterioration of the upper tract and no delayed complications.

Chronic pains typically evaluated by a urologist are discussed from the perspective of a non-urologist pain clinician. The pathophysiology of some pains is understood and so we believe the patient's symptoms: examples are cancer-related pain and recurrent urolithiasis. We treat these pains with traditional analgesics. Other pains, such as those of interstitial cystitis, chronic prostatodynia, and chronic orchialgia are less understood and so are treated in a more conservative and often empiric fashion. Proposed therapies for these disorders are discussed.

It is currently undisputed that cystoscopy is required after each hysterectomy to confirm the integrity of the urinary tract before leaving the operating room. Sometimes, secondary findings are detected on cystoscopy like interstitial cystitis and exophytic tumors. We are reporting a case of asymptomatic bladder cancer discovered incidentally during a posthysterectomy cystoscopy. The chance of a woman having this cancer is about 1 in 85. This demonstrates the additional merit of performing cystoscopy after hysterectomy as a screening test for bladder cancer.
