Cutaneous reactions to vaccinations.

Vaccinations are important for infectious disease prevention; however, there are adverse effects of vaccines, many of which are cutaneous. Some of these reactions are due to nonspecific inflammation and irritation at the injection site, whereas other reactions are directly related to the live attenuated virus. Rarely, vaccinations have been associated with generalized hypersensitivity reactions, such as erythema multiforme, Stevens-Johnson syndrome, urticaria, acute generalized exanthematous pustulosis, and drug hypersensitivity syndrome. The onset of certain inflammatory dermatologic conditio…

[Hand, foot and mouth disease. Report of 10 cases].

Ten children with hand-foot-and-mouth disease are described and their clinical manifestations reported in detail. It is a benign, minor viral disease of childhood, but in our country its real incidence is unknown. Diagnosis in the clinical practice is based on the recognition of typical and atypical localizations of the vesicular eruption, as well as on the identification of extracutaneous and extrabuccal manifestations. The differential diagnosis includes herpangina, aphthous stomatitis, dyshidrosis and erythema multiforme.

Inflammatory epidermolysis bullosa acquisita mimicking toxic epidermal necrolysis and dermatitis herpetiformis.

We report a patient with a spectrum of clinical features simulating toxic epidermal necrolysis, bullous erythema multiforme and later, dermatitis herpetiformis (DH). The histological features were suggestive of DH, bullous pemphigoid (BP) and epidermolysis bullosa acquisita (EBA). Direct immunofluorescence results suggested BP or EBA. Indirect immunofluorescence on salt-split skin and immunoblotting analysis on normal human dermal extracts gave results that were diagnostic for EBA.

Stevens-Johnson syndrome induced by doxycycline.

Stevens-Johnson syndrome (SJS) is an acute mucocutaneous eruption nosologically related to erythema multiforme (EM) and toxic epidermal necrolysis (TEN). Medications are the most common triggering factors for SJS, with anticonvulsants, sulfonamides, penicillins, allopurinol, and nonsteroidal anti-inflammatory drugs (NSAIDs) most commonly implicated. SJS is very rarely associated with tetracyclines. We report a case of doxycycline-induced SJS in a 46-year-old man.

Herpes simplex virus type 1 as a cause of widespread intracorneal blistering of the lower limbs.

Herpes simplex virus (HSV) infections often manifest as acute self-limiting eruptions of grouped vesicles, which have a tendency to recur. Common manifestations include that of gingivostomatitis, vulvovaginitis, recurrent erythema multiforme, herpetic whitlow, and eczema herpeticum. We report a case of HSV-1 presenting as a symmetrical intracorneal blistering eruption involving the lower limbs in a patient with no previous history of herpes simplex infection.

Herpes. Vaccines for HSV.

Herpes simplex viruses type 1 (HSV-1) and 2 (HSV-2) are ubiquitous human pathogens capable of producing primary, latent, and recurrent infections. These viruses cause a variety of clinical illnesses including genital herpes, oral-facial infections, cutaneous infections, ocular infections, neonatal herpes, herpes encephalitis, disseminated infection, and erythema multiforme. This article summarizes the past and current efforts to develop both prophylactic and therapeutic HSV vaccines.

Delayed hypersensitivity reaction to vancomycin.

The use of vancomycin has increased dramatically in recent years with the emergence of methicillin-resistant Staphylococcus aureus and coagulase-negative staphylococci as important hospital pathogens. A patient receiving vancomycin 1.75 g intravenously as a single daily dose developed a syndrome characterized by high fever, erythema multiforme, eosinophilia, and presumed interstitial nephritis. This delayed hypersensitivity reaction resolved with discontinuation of the drug and treatment with methylprednisolone.

Orofacial herpes simplex virus infections: current concepts in the epidemiology, pathogenesis, and treatment, and disorders in which the virus may be implicated.

Herpes simplex virus (HSV) is the causal agent of herpetic stomatitis, recurrent herpes labialis, and the recurrent intraoral infections seen particularly in the immunocompromised patient. HSV has also been implicated in some erythema multiforme, cranial neuropathies, Behçet's syndrome, and oral squamous carcinoma. Recent advances in understanding of HSV, epidemiology, pathogenesis, and treatment are reviewed because there have been numerous advances over the past 5 years.

Bullous systemic lupus erythematosus: an unusual clinical course and detectable circulating autoantibodies to the epidermolysis bullosa acquisita antigen.

Bullous systemic lupus erythematosus is a newly recognized form of systemic lupus erythematosus characterized by a skin eruption clinically and histologically resembling dermatitis herpetiformis and responsive to dapsone. We report on a patient with bullous systemic lupus erythematosus who initially presented with lesions clinically resembling erythema multiforme, experienced exacerbation of her disease with dapsone, and had detectable circulating autoantibodies to the epidermolysis bullosa acquisita antigen.

Fixed drug eruption in a cribriform pattern: an atypical presentation.

Fixed drug eruption (FDE) is a distinct adverse drug reaction characterized by a well-defined, dusky, violaceous to erythematous patch that recurs at the same site upon re-exposure to causative drugs and resolves with hyperpigmentation. This unique reaction is a type IV hypersensitivity reaction mediated by memory CD8+ T cells that reside in the basal layer of the epidermis of the resting FDE lesion. Variants of FDE described in the literature include bullous, generalized bullous, nonpigmenting, linear, papular, erythema multiforme-like, transitory giant, annular, psoriasiform, erythema dyschr…

Acute generalized exanthematous pustular dermatitis and viral infection.

Three patients with acute generalized exanthematous pustular dermatitis (AGEPD) are presented. This new clinical entity comprises generalized erythema, erythema multiforme, amicrobial pustules, fever and spontaneous healing over a 10-day period. Histologically, there are a mixed neutrophilic and eosinophilic vasculitis and an eosinophilic subcorneal and spongiform pustule. All the patients experienced pharyngitis 1 week before the onset of the rash. Two of them had high-titered IgM-neutralizing antibodies to enteroviruses and 1 had an enterovirus in stool and urine. AGEPD may be related to a v…

Necrolytic acral erythema: an expanding spectrum.

Hepatitis C virus (HCV) infection is the most common chronic blood-borne viral infection in the United States. Well-described cutaneous manifestations of HCV infection include polyarteritis nodosa, porphyria cutanea tarda, type II cryoglobulinemia-associated vasculitis, pruritus, erythema nodosum, urticaria and urticarial vasculitis, lichen planus, and erythema multiforme. First described in 1996, necrolytic acral erythema (NAE) is now recognized as a cutaneous acral eruption uniquely associated with HCV infection. Most patients present with chronic, acral, erythematous, and psoriasiform lesio…

Kissing bugs (Triatoma) and the skin.

Kissing bugs (Family Reduviidae) can be the source of nocturnal dermatologic wounds in the mid to southern latitudes in the United States. The insects are obligate blood feeders and though the bites may be asymptomatic, a variety of dermatologic eruptions or death from anaphylaxis can result. The various dermatologic forms of the bite can be mistaken for herpes zoster, erythema multiforme and the ubiquitous catch-all diagnoses of "spider-bite."

Outpatient management of Stevens-Johnson syndrome: a report of four cases and management strategy.

Stevens-Johnson syndrome is a severe form of erythema multiforme with or without bullous lesions. This is a potentially life-threatening condition that has traditionally required hospitalization for optimal management. We report the successful management of four patients who developed this syndrome following administration of antimicrobial agents. In our experience, for individuals with mild to moderate disease, Stevens-Johnson syndrome can be managed with corticosteroids as an outpatient.

Thalidomide: dermatological indications, mechanisms of action and side-effects.

Thalidomide was first introduced in the 1950s as a sedative but was quickly removed from the market after it was linked to cases of severe birth defects. However, it has since made a remarkable comeback for the U.S. Food and Drug Administration-approved use in the treatment of erythema nodosum leprosum. Further, it has shown its effectiveness in unresponsive dermatological conditions such as actinic prurigo, adult Langerhans cell histiocytosis, aphthous stomatitis, Behçet's syndrome, graft-versus-host disease, cutaneous sarcoidosis, erythema multiforme, Jessner-Kanof lymphocytic infiltration o…

Mycoplasma pneumoniae-Induced Rash and Mucositis: How Can We Help?

Mycoplasma pneumoniae-induced rash and mucositis (MIRM) is a rare clinical pathology distinct from erythema multiforme major and Stevens-Johnson syndrome. We present a case of a patient who developed severe oral mucositis following an M. pneumoniae respiratory tract infection. We aim to raise awareness of MIRM, its varying presentations, and its management, particularly in light of the limited scientific evidence and the variability in treatment strategies depending on disease severity.

Rowell Syndrome in a Middle-Aged Woman: A Case Report.

Rowell syndrome (RS) is characterized by the presentation of lupus erythematosus (LE) with erythema multiforme (EM)-like lesions. It is thought to display a characteristic serologic pattern consisting of a "speckled-type" antinuclear antibody (ANA), positive anti-Ro/SSA or anti-La/SSB, or positive rheumatoid factor (RF). We report the case of a patient with subacute cutaneous lupus erythematosus (SCLE) who presented with EM-like lesions responsive to oral corticosteroids.

Toxic epidermal necrolysis following allergic contact dermatitis caused by occupational exposure to ultraviolet-cured inks.

Erythema multiforme is a relatively common skin disorder; the most common cause is herpes simplex infection, but topical sensitivities reportedly also provoke this reaction. We report here a case that progressed to toxic epidermal necrolysis due to contact with ultraviolet (UV)-cured inks. The diagnosis was confirmed by patch tests to acrylates in the UV-cured inks, histopathological studies of the lesions, and positive patch test to 1,6-hexanediol diacrylate.
