This manuscript aims to address the biologic and mechanical principles underlying the treatment of displaced distal radial fractures. A literature review was performed using Ovid Medline 1950-2007 and all evidence-based medicine reviews. A recent Cochrane review and a meta-analysis were reference searched. All randomized trials comparing internal and external fixation and all articles related to volar locked plates were searched for. Bone transport can be performed with an external fixator alone or with the monorail technique which entails the combination of a fixator and an intramedullary nail. The purpose of this study was to compare the complication rates and long-term outcomes of these methods. Radiocarpal fracture translocations are uncommon injuries without well-defined treatment. This case report presents a patient with this injury that was treated with repair of the volar and dorsal structures and dynamic external fixation. Eight weeks after the procedure, the external fixation device was unlocked to allow wrist flexion and extension only. Twelve weeks after the procedure, the external fixation device was removed completely to allow full wrist range of motion. Six months after surgery, the patient had no reported pain or dysfunction, and no recurrence of radiocarpal translation.

BACKGROUND: The purpose of this study was an experimental assessment of changes in bone fragment position in patients with non-union of the tibia treated with a hexapod fixator.
HYPOTHESIS: We hypothesized that the use of hexapod fixators leads to differences between the planned and actual position of bone fragments.
METHODS: The study was conducted in physical models of the hexapod fixator-bone fragment system. Bone fragment displacement was measured using the Optotrak Certus Motion Capture System. We assessed differences between the planned and actual position of bone fragments.
