Pathophysiological and metabolic manifestations of pulmonary vascular disease in children.

In children with congenital heart disease pulmonary vascular disease can be fatal for a variety of reasons. Even before the classical changes of advanced pulmonary vascular obstructive disease have developed, a marked increase in pulmonary vascular smooth muscle can be fatal due to pulmonary hypertensive crises. After the Fontan procedure, a modest increase in muscularity can jeopardise the outcome since there is no subpulmonary ventricle to support the pulmonary circulation. Following heart transplantation, a slight increase in muscularity can cause failure of the donor right ventricle unless…

Postnatal Pulmonary Artery Development from Transcript to Tissue.

Many congenital conditions and surgical interventions perturb the hemodynamics experienced by proximal pulmonary arteries during early postnatal development, thus leading to differential gene expression and associated changes in vascular structure and function. Among these, pathologic conditions include patent ductus arteriosus, pulmonary atresia and stenosis, and hypoxemia-induced pulmonary hypertension while surgical interventions include the placement of a Blalock-Taussig shunt as well as Glenn, Fontan, and Norwood procedures. Despite the significant morbidity associated with these diverse

Waldmann's Disease (Primary Intestinal Lymphangiectasia) with Atrial Septal Defect.

Waldmann's disease or Primary Intestinal Lymphangiectasia (PIL) is a rare disorder of gastrointestinal tract characterized by dilated lymphatics and widened villi causing leakage of lymph into intestinal lumen. Loss of lymph leads to hypoalbuminemia, hyogammaglobulinemia and lymphopenia. Secondary lymphangiectasia occurs secondary to an elevated lymphatic pressure as in lymphoma, systemic lupus erythematosus, constrictive pericarditis, cardiac surgeries (Fontan's procedure), inflammatory bowel disease and malignancies. We, hereby present a five-year-old male child who presented with abdominal

Fontan completion after Norwood with bidirectional Glenn shunt: their haemodynamics and pulmonary artery configuration.

OBJECTIVES: We ranked the haemodynamics and pulmonary artery (PA) configurations of Norwood with bidirectional Glenn shunt (NW-G) patients among the other staged Fontan completion (conventional) patients in this study. METHODS: Between August 2001 and April 2010, 91 consecutive patients completed staged Fontan operations. Among them, NW-G was performed in 11. RESULTS: There was no operative or late death except in one case which resulted from a reoperation after Fontan completion. Although the age at bidirectional Glenn (BDG) and Fontan completion was younger in NW-G (4.2 vs. 10.9 months, P

Feasibility and Safety of Using a Fused Echocardiography/Fluoroscopy Imaging System in Patients with Congenital Heart Disease.

BACKGROUND: Fused real-time three-dimensional transesophageal echocardiography and fluoroscopy has been used in adult patients during percutaneous mitral valve and aortic valve procedures. The use of fused echocardiographic/x-ray fluoroscopic imaging (FEX) in pediatric patients undergoing congenital heart disease catheterization has not been evaluated for feasibility and safety. The aims of this study were to assess the feasibility and safety of FEX for interventional guidance and to perform a comparison of atrial septal defect (ASD) device closure using this technology with traditional guidan

Adults with Fontan circulation: prognostic value of exercise pulmonary vascular resistance index.

BACKGROUND AND AIMS: Decrease in pulmonary vascular resistance (PVR) during exercise confers favourable prognosis in biventricular physiology, but its role in Fontan circulation remains unknown. This study aimed to evaluate the prognostic value of PVR index (PVRI) response to exercise in adults post-Fontan palliation. METHODS: Retrospective cohort of 88 adults (age ≥18 years) post-Fontan undergoing exercise (supine cycling) cardiac catheterization. Patients were categorized into absence (n = 23) or presence (n = 65) of exercise-induced reduction in PVRI. The clinical endpoint was a composite

Association Between Hemodynamic Parameters and Gut Microbiota in Fontan Circulation: A Cross-Sectional Study.

BACKGROUND: The Fontan operation results in chronic hemodynamic alterations, including central venous hypertension, which may contribute to gut dysbiosis. This study aimed to compare gut microbiota profiles between Fontan patients and age- and sex-matched healthy controls and to examine how catheter-measured hemodynamic and liver-related parameters relate to microbial alterations. METHODS: This cross-sectional study enrolled 23 Fontan patients and 23 matched controls. Fontan patients underwent cardiovascular assessments, and stool samples were collected from all participants. Early donor-specific antibodies in pediatric patients with Fontan circulation after heart transplant.

BACKGROUND: De novo donor specific antibodies (DSA) are associated with increased morbidity and mortality in adults after heart transplant (HT). We describe our experience with pediatric patients undergoing HT for Fontan circulatory failure (FCF) who developed early DSA. METHODS: This is a retrospective case series of patients who received HT for FCF at a single center between April 2017 and May 2024. The inclusion criteria were age < 18 years, receiving HT for FCF, and positive DSA within 35 days of HT. Data collected included demographics, pre-transplant anatomy and FCF phenotype, pre- and

Single-cell spatial transcriptomics reveals hepatocyte reprogramming in Fontan-associated liver disease.

Fontan-associated liver disease (FALD) is a frequent complication in single-ventricle patients palliated with the Fontan operation. FALD severity can impact clinical decisions; however, the pathophysiology of FALD progression is unknown. Single-cell spatial transcriptomics (ST) was performed on liver explant tissue sections from FALD patients with early (n = 1) and advanced fibrosis (n = 1) using CosMx Spatial Molecular Imaging with in situ hybridization of 6000 genes. Immunofluorescence for liver zonation and cellular stress markers was performed to confirm protein expression based on ST anal

The Fontan Adult Brompton clinical score and mortality risk.

BACKGROUND AND AIMS: Adults with congenital heart disease who underwent a Fontan palliation are at increased risk of premature death. This study aimed to identify a risk score based on widely available clinical variables at the first adult visit and facilitate personalized care. METHODS: A single tertiary centre retrospective study included adults with a Fontan circulation during their first adults with congenital heart disease outpatient visit. Anatomic and periprocedural data, clinical, laboratory, and echocardiographic parameters at the first visit, and mortality or heart transplantation d

Influence of Heart Rate on Hemodynamics in Fontan Circulation.

The standard value of heart rate (HR) and the effect of HR on Fontan circulation remain unclear. HRs and hemodynamic parameters were evaluated in 59 adults with Fontan circulation without pacemaker (PM) and then compared with those of 15 patients with PM. The minimum HR in Holter monitoring, HR in resting electrogram, resting HR in cardiopulmonary exercise (CPX), maximum HR in Holter, and peak HR in CPX were observed during sleep, supine, sitting, maximum daily activity, and exercise, respectively. HRs in 15 patients with excellent status of Fontan circulation were 50 ± 6, 75 ± 15, 81 ± 7, 86

Role of strain imaging for risk stratification in Fontan patients with normal ventricular ejection fraction.

AIMS: Systemic ventricular (SV) strain imaging improves early detection of systolic dysfunction in patients with congenital heart disease and biventricular circulation. However, its role for risk stratification in the Fontan population has not been systematically studied. We hypothesized that among Fontan patients with normal SV ejection fraction (Echo_EF) at baseline, those with reduced SV longitudinal strain (Echo_LS) would have a higher risk of progressive SV systolic dysfunction, greater neurohormonal activation, and cardiovascular events (heart failure hospitalization/death/transplant) du

Semi-Invasive Pressure-Flow Plots Obtained Using Exercise Echocardiography Relate to Clinical Status and Exercise Capacity in Patients With a Fontan Circulation.

AIMS: Exercise echocardiography with peripheral venous pressure measurement (CPETecho-PVP) may provide superior insights into the pathophysiology of Fontan failure compared to standard cardiopulmonary exercise testing. Accordingly, we assessed (1) the clinical and hemodynamic correlates of pressure-flow plots obtained from CPETecho-PVP in Fontan patients and (2) the relationship between pressure-flow plots and exercise capacity. METHODS: Forty-one consecutive Fontan patients underwent CPETecho-PVP. Peripheral venous pressure was measured in the distal upper extremity using an 18- to 20-gauge

Spleen shear wave elastography measurements do not correlate with histological grading of liver fibrosis in Fontan physiology: a preliminary investigation.

BACKGROUND: Elevated spleen stiffness may be seen in patients with portal hypertension due to cirrhosis. In patients with Fontan physiology, elevated liver stiffness has been shown to correlate poorly with liver fibrosis. It is unknown whether spleen stiffness may instead serve as a surrogate marker of liver fibrosis in these patients. OBJECTIVE: To compare spleen stiffness determined by shear wave elastography (SWE) with histological findings of an ultrasound-guided liver biopsy in patients who had undergone Fontan palliation as a potential surrogate for Fontan-associated liver disease. Aerobic Capacity of Adults With Fontan Palliation: Disease-Specific Reference Values and Relationship to Outcomes.

BACKGROUND: Patients with Fontan palliation have reduced aerobic capacity because of impaired cardiac, pulmonary, and skeletal muscle function. However, the assessment of aerobic capacity in this population still relies on comparisons with people without cardiovascular disease rather than comparison with the expected aerobic capacity of other Fontan patients. The purpose of this study was to determine the expected aerobic capacity of adults with Fontan palliation. METHODS: Adults with Fontan palliation who underwent a cardiopulmonary exercise test at Mayo Clinic (2003-2023) were stratified in
