Incidental Median Nerve Injury in the Hand by a High-Speed Drill.

Traumatic injuries of the peripheral nerves can be easily overlooked in the emergency department. In these cases, extensive knowledge of anatomy, a high index of suspicion and correct evaluation of neurological status could maintain the proper diagnosis. Median nerve injury in the region of the hand is commonly due to glass material. However, extremely rare, other causes of injury, such as pieces of wood, acupuncture needle-fracture bodies, metallic foreign bodies, and migration of K-wires, have been reported. In the current report, a case of median nerve injury in a 35-year-old man treated wi

Contraceptive implant migration to the lung.

A 27-year-old female presented with a 'missing' contraceptive implant. Chest imaging demonstrated a 4-cm linear opacity in a subsegmental branch of the pulmonary artery to the left lower lobe consistent with a migrated contraceptive implant. A mini-thoracotomy and arteriotomy was performed. The artery was opened distally to its third division. However, it was not possible to retrieve the implant, and the decision was made to proceed to segmentectomy. After resection, it was noted that the foreign body had significantly endothelialised within the wall of the artery and required sharp dissection

Endonasal endoscopic retrieval of lodged silicone punctal plugs: a case report.

We present the case of a 59-year-old woman whose silicone punctal plugs migrated distally causing lacrimal obstruction. The plugs were removed from the lacrimal sac during endoscopic endonasal dacrocystorhinostomy and is illustrated by endoscopic photographs and videos taken at the time of surgery. This case highlights the potential complications of punctal plugs and the difficulties posed in retrieving them.

Morphology of parenchymally implanted foreign bodies indicates copulatory wounding in a planarian.

Traumatic mating, that is, copulation that involves wounding the partner's body, is a widespread phenomenon that is particularly prevalent in hermaphroditic animals. Traumatic mating is generally a collateral side effect of diverse strategies (from physical anchorage to injection of substances to manipulate the partner), but the trauma could also be adaptive by itself if it delays remating by the injured partner. In the Tricladida (the clade of planarian flatworms), reciprocal sperm transfer is often assumed to occur by means of a 'regular' nontraumatic copulation, that is, insertion of the pe

Fungal keratitis caused by Paecilomyces lilacinus associated with a retained intracorneal hair.

OBJECTIVE: To report a case of fungal keratitis caused by Paecilomyces lilacinus (P. lilacinus) associated with a retained intracorneal hair. METHODS: A 61-year-old man developed pain, decreased vision, hyperemia, and corneal infiltrates in his right eye without any predisposing factor. An intracorneal hair had migrated superiorly in the corneal stroma, giving rise to 3 separate stromal infiltrates. The patient demonstrated a waxing and waning course over several months despite antimicrobial and steroid therapy. RESULTS: Histopathologic examination of a corneal biopsy specimen disclosed the

Reoperation for complications of the Angelchik antireflux prosthesis.

The Angelchik antireflux prosthesis is associated with complications that require reoperation in 5% to 15% of patients. To determine the morbidity and success of reoperation for these complications, we conducted a retrospective study of 15 patients. Time to reoperation ranged from 3 weeks to 113 months with a mean of 31 months. Indications for reoperation included dysphagia (8 patients), recurrent reflux (6 patients), and prosthesis migration (1 patient). Ten patients underwent prosthesis removal and fundoplication, 4 had prosthesis removal without fundoplication, and 1 patient had the prosthe

Tympanostomy tubes: experience with removal.

Whereas the clinical indications for tympanostomy tube placement are well-established, the indications for operative tympanostomy tube removal remain unspecified. A 1-year retrospective review done at the Massachusetts Eye and Ear Infirmary revealed 131 tympanostomy tubes to have been removed under general anesthesia. Chronic otorrhea, granuloma formation, tube nonfunction due to blockage, and migration of the tube into the middle ear constituted the surgical indications in 75 cases. The remaining 56 tubes were removed on the physicians' judgment that artificial ventilation was no longer requi

Infection, temporality and inequality: Sanitizing foreign bodies and protecting public health in Taiwan.

This paper probes how temporality is integral to the health examination regime that aims to protect citizens from infectious diseases in Taiwan. The paper finds that migrant workers in less-skilled occupations are examined more frequently than foreign professionals. Analyzing such differentiation, this paper argues that a hierarchy of sanitization is built on and increases the inequality between them and perpetuates instability in migrant workers' circumstances. Applying a temporal approach to the study of health examination opens new inroads into our understanding of how a "migration state" a

Diabetes and prediabetes in patients with hepatitis B residing in North America.

UNLABELLED: Diabetes is associated with liver disease progression and increased hepatocellular carcinoma risk, but factors associated with diabetes in patients with chronic hepatitis B virus (HBV) infection in North America are unknown. We aimed to determine factors predictive of diabetes and impaired fasting glucose (IFG) in a large HBV-infected multiethnic cohort. Adults with chronic HBV not receiving antiviral therapy were enrolled from 21 centers in North America. Diabetes was defined by history/medication use or fasting glucose≥126 mg/dL and IFG as fasting glucose 100-125 mg/dL. Of 882 pa

The Mysterious Whistling Breath: Foreign Body Aspiration.

Foreign body aspiration is commonly seen in the pediatric age group and can be a life-threatening condition. Typical presentations include coughing, wheezing, and choking and can often masquerade as asthma causing misdiagnosis and treatment delay. Most of the time, the actual aspiration event is unnoticed, and patients may remain asymptomatic until they present with recurrent infections with or without positive radiological findings. Aspirated objects tend to migrate distally, and organic objects may induce edema and inflammation. Diagnosis is crucial as near-total or total obstruction of the

Challenging Removal of Embedded Fishbone in Tongue: Multimodal Technique, Surgical Approach and Consideration of Migration.

Fishbone ingestion is a common occurrence and patients present with various symptoms, posing challenges to the attending physicians. Here, we present two unique cases of patients with an unexpected rapidly migrating fishbone in the tongue. The first patient was operated transorally because of a foreign body embedded in the genioglossus muscle. In the second patient, CT scan located a fishbone embedded in the left hyoglossus muscle; however, the fishbone had to be relocated intraoperatively using bedside ultrasound guidance and was eventually found embedded within the mylohyoid muscle. The fish

Surgical removal of an embolized intravenous catheter fragment from the axillobrachial vein of a dog.

An 8-month-old neutered male miniature Australian shepherd was examined for evaluation and treatment of suspected embolization of a peripheral intravenous catheter fragment. Embolism had occurred after the catheter cannula was accidentally cut during removal from the right cephalic vein. Radiographs showed migration of the fragment to the proximolateral right brachium. Surgical exploration, assisted by fluoroscopy, localized the fragment to the axillobrachial vein, where the tip was visualized within the vessel lodged against the wall. A venotomy was performed, allowing the fragment to be extr

The wandering mimic: Meckel's diverticulum with enterolith simulating an intraperitoneal hydatid cyst.

Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract and is often asymptomatic. Unusual complications such as enterolith formation or foreign body impaction are rare, particularly in pediatric patients, and may produce atypical imaging findings that mimic other intra-abdominal pathologies. We report a case of a previously healthy 4-year-old boy who presented with intermittent colicky abdominal pain and non-bilious vomiting. Contrast-enhanced CT demonstrated a cystic lesion in the left paracolic gutter containing multiple internal rounded low-attenuatio

Anti-CD3 microporous annealed particle hydrogel protects stem cell derived beta cells from autoreactive T cells.

Type 1 diabetes (T1D) results from autoimmune destruction of pancreatic beta cells, leaving patients dependent on exogenous insulin and at risk of severe hypoglycemic episodes. Stem cell-derived beta-like cells (sBCs) offer a promising approach for beta cell replacement therapy, but clinical translation is limited by immune-mediated rejection, recurrent autoimmunity, and inhospitable transplantation sites. Biomaterials have been investigated to provide localized immune-isolation and immunomodulation, but foreign body responses and rapid depletion of therapeutic agents remain as obstacles to cl

Beyond Hemostasis: Understanding the Risks of Bone Wax in Orthopedic Surgery - A Case Report.

INTRODUCTION: Osseous hemorrhage following a surgical incision or traumatic fracture can be a challenging issue to manage. During orthopedic surgeries, bone wax is frequently utilized as a surgical technique to enhance hemostasis and reduce bleeding from bone. The hemostatic effect of bone wax mostly stems from its physical characteristics. The advantages of using bone wax are that it is easy to handle, low cost, inert, malleable, and has good sealing capacity and bone adherence. A review of the literature also shows that the most notable side effects of using bone wax are allergic foreign bod

Analysis of a polytef granuloma mimicking a cold thyroid nodule 17 months after laryngeal injection.

A 65-year-old female who received laryngeal injections with polytef (Teflon) paste for treatment of right vocal cord paralysis first noticed a small, right anterior neck mass 17 months later. Clinically this mass slowly progressed in size and was felt to be a recurrent carotid body tumor, but at surgery, 22 months after injection, a circumscribed nodule on the anterior right lobe of the thyroid was found and histologically diagnosed as a foreign body granuloma. Scanning electron microscopy (SEM) and energy-dispersive x-ray analysis (EDXA) of material in this lesion was identical to that of pol

Intracardiac Porcupine Quill Migration in a Dog: Multimodality Imaging Findings and Surgical Management.

The porcupine is a nocturnal quill-bearing rodent common throughout Italy. When threatened, it defends itself by erecting its quills, rattling its tail, and running sideways or backyard into predators. The quills are released upon contact with assailants and tend to migrate into several body tissues or cavities due to the unique inverted cuticles (crenate) directed downwards to the quill base (a sort of "harpoon effect"). Intracardiac migration of porcupine quills has been reported in a few dogs with severe clinical signs. This case report describes a single porcupine quill [Hystrix cristata,
