["Central islands"--an early postoperative complication after photorefractive keratectomy].

BACKGROUND: After myopic photorefractive keratectomy undercorrected zones in the center of the treated cornea (central islands (CI)) occur early postoperatively occasionally the reason for double-vision, halos and reduction of best corrected visual acuity. PATIENTS AND METHODS: Sixty-two eyes of 33 patients were treated with an 193 nm excimer laser for correction of myopia (range: -1.0 to -12.0 D) and an optical zone of 6.0 mm. The patients were selected consecutively regarding the follow-up examination which ranged from 1 month to 21 months after surgery. RESULTS: At 1 month after surgery t

Automated keratoconus screening with corneal topography analysis.

PURPOSE: Although visual inspection of corneal topography maps by trained experts can be powerful, this method is inherently subjective. Quantitative classification methods that can detect and classify abnormal topographic patterns would be useful. An automated system was developed to differentiate keratoconus patterns from other conditions using computer-assisted videokeratoscopy. METHODS: This system combined a classification tree with a linear discriminant function derived from discriminant analysis of eight indices obtained from TMS-1 videokeratoscope data. One hundred corneas with a vari

Assessment of Human Corneal Biomechanical Properties After Refractive Surgery With Inflation Test Using Optical Coherence Tomography.

PURPOSE: To evaluate the biomechanical properties of the cornea after refractive surgery using an inflation test combined with optical coherence tomography (OCT). METHODS: Nine human donor corneas were divided into two groups: healthy (control) and treated (divided into three subgroups: photorefractive keratectomy [PRK], laser in situ keratomileusis [LASIK], and small incision lenticule extraction). Refractive treatments were applied to simulate -4.00 diopters of myopia correction. The corneas were mounted on an artificial anterior chamber, and an inflation test was performed using a custom s

Comparison of visual quality and optical zones after TransPRK, SMILE, and FS-LASIK myopia correction procedures.

BACKGROUND: TransPRK, SMILE, and FS-LASIK are widely used surgical procedures for myopia correction, but differences in postoperative visual quality and optical zone design remain a subject of clinical interest. This study aimed to compare visual outcomes, higher-order aberrations (HOAs), contrast sensitivity, optical zone diameter, and corneal biomechanics among these three techniques. METHODS: In this prospective cohort study, 82 patients (164 eyes) with myopia were randomly assigned to undergo TransPRK (n = 56 eyes), SMILE (n = 52 eyes), or FS-LASIK (n = 56 eyes). All procedures were perfo

Treatment of Corneal Ectasia by Implantation of an Allogenic Corneal Lenticule.

PURPOSE: To describe a case in which post-LASIK corneal ectasia was treated using an allogeneic lenticule implanted into the recipient eye. METHODS: A 29-year-old man presented with bilateral ectasia 10 years after LASIK. Manifest refraction was -1.50 -3.25 × 15° in the right eye and -13.50 -6.00 × 10° in the left eye. Because of contact lens intolerance in the left eye, the authors decided to implant an allogenic corneal lenticule, created by small incision lenticule extraction with the VisuMax laser (Carl Zeiss Meditec, Jena, Germany) in a donor eye with a refraction of +5.75 -0.50 × 15°.
