[Encephalopathies in infection by human immunodeficiency virus].

Encephalitis occupies a large part in the neurological complications of HIV infection. It is frequent and in most cases of poor prognosis. Some cases of encephalitis are directly related to HIV while others are caused by an opportunistic infection. Among the former is the acute encephalitis coincident with seroconversion, which is exceptional and spontaneously regressive, and the subacute encephalitis better known as HIV encephalopathy which has a constantly pernicious course ending in subcortical dementia lethal within a few months. Some cases of opportunistic encephalitis are associated with

Multifocal leukoencephalopathy in adult leukemia: histologic features and etiologic considerations.

A number of structural alteration in brains of leukemic children have been attributed to intrathecal administration of methotrexate and radiation therapy. We report an instance of asymptomatic encephalopathy in an adult who received thio-TEPA (by subarachnoid injection) as treatment for leukemic meningitis. The lesions were microscopic, confined to cerebral white matter and included: vacuolation, myelin destruction, axonal swelling, gliosis, deposition of mineral salts and aggregates of large foamy cells. The etiology of this condition remains undetermined.

Concomitant diagnosis of multiple sclerosis and human immunodeficiency virus (HIV) infection: case report and the review of literature.

BACKGROUND: To date, few cases of multiple sclerosis (MS) patients with concomitant Human Immunodeficiency Virus (HIV) infection have been described. However, none of the previously described cases has been treated with Natalizumab, probably due to the increasing risk of progressive multifocal leukoencephalopathy (PML). CASE: We report the case of a patient concomitantly diagnosed for HIV infection and MS treated with combined antiretroviral therapy (cART) and Natalizumab for 19 months, without clinical or radiological MS activity. CONCLUSIONS: Our case might suggest considering Natalizumab

Treatment of Viral Encephalitis.

Viral encephalitis is difficult to treat. Herpes simplex encephalitis has been successfully treated with acyclovir, but is still a cause for significant morbidity even with that treatment. A rare form of autoimmune encephalitis related to NMDA receptor antibody after infection by herpes simplex can be treated with corticosteroid therapy. Arthropod-borne encephalitides, such as West Nile virus encephalitis and Eastern equine encephalitis, are primarily treated with supportive measures. Attempts have been made to use immunoglobulin therapy with limited effects. Progressive multifocal leukoenceph

Clinical Course of Relapsing Remitting Multiple Sclerosis Post-Natalizumab.

Natalizumab is an efficacious disease modifying therapy (DMT) for relapsing remitting multiple sclerosis (RRMS), often limited by risk of progressive multifocal leukoencephalopathy. We describe the clinical course of RRMS patients switched from natalizumab to another DMT. We identified all RRMS patients treated with natalizumab ≥3 months with JC virus antibody positivity who switched to another DMT. Overall, 84 individuals switched DMT with 57 (68%) beginning fingolimod. On fingolimod, survival without a relapse was 74% (55.8-85.6%) at 36 months and survival without disease progression was 78%

Brief Report: Role of Thymic Reconstitution in the Outcome of AIDS-Related PML.

Implications of thymopoiesis in AIDS-related opportunistic infections remain unexplored. We used progressive multifocal leukoencephalopathy (PML), caused by JC virus (JCV), as an opportunistic infection model, and we simultaneously investigated thymic output and T-cell responses against JCV in 22 patients with PML treated with combined antiretroviral therapy. Thymic output was significantly associated with JCV-specific CD4⁺ and CD8⁺ T-cell responses and improved survival. Our data suggest that patients with AIDS-related PML and impaired thymopoiesis are less likely to develop a robust JCV-spec

Immunosuppression associated with novel chemotherapy agents and monoclonal antibodies.

The introduction of novel agents to the therapeutic armamentarium for oncologic, rheumatologic, and neurologic disorders has resulted in major clinical advances. These agents impact immune function, resulting in a discrete spectrum of infectious complications. Purine analogues and alemtuzumab alter cell-mediated immunity, resulting in opportunistic viral/fungal infections. Herpes zoster incidence increases with bortezomib. Hepatitis B reactivation may occur with rituximab. Cases of progressive multifocal leukoencephalopathy have occurred following monoclonal antibody therapy. Tumor necrosis fa

Clinically silent PML and prolonged immune reconstitution inflammatory syndrome in a patient with multiple sclerosis treated with natalizumab.

We report the case of a woman with natalizumab-treated multiple sclerosis (MS) and clinically silent progressive multifocal leukoencephalopathy (PML) with an unusually long preclinical phase, followed by acute symptoms due to development of immune reconstitution inflammatory syndrome (IRIS). Furthermore, the course of the IRIS was prolonged and continued to progress even five months after natalizumab treatment was ceased. This case shows that PML and IRIS can have a considerably variable course in natalizumab-treated MS patients and underlines the need for PML screening in JC virus antibody-po

Evolution of purely infratentorial PML under HAART--negative outcome under rapid immune reconstitution.

Progressive multifocal leukoencephalopathy (PML) caused by the polyomavirus JC is a well-recognised complication of AIDS. Purely infratentorial manifestations are rare. Introduction of highly active antiretroviral therapy (HAART) has been associated with a reduction in morbidity and an improvement in overall survival among HIV-infected individuals. Recently, several reports have described adverse events in patients with PML who begin HAART and show evidence for immune reconstitution. We describe the clinical course of two patients with PML with purely infratentorial manifestation, whose clinic

Molecular detection of JC virus in embalmed, formalin-fixed, paraffin-embedded brain tissue.

Embalmed tissues are adequate for the detection of JC virus in lesions of progressive multifocal leukoencephalopathy (PML) by immunohistologic and molecular methods. JC virus was readily detected in embalmed brain tissue using immunohistochemistry (IHC), in situ hybridization (ISH), and the polymerase chain reaction (PCR). Two brains were removed from bodies that had been embalmed at least 24 h prior to autopsy. They were subsequently post fixed in 10% buffered formalin for 10-14 days before dissection and molecular studies were performed. Though these techniques are not novel, their use in em

Stereotactic biopsy in cerebral lesions of AIDS.

The experiences with stereotactic biopsies in 121 patients with AIDS compared to 142 non-AIDS patients are presented. In the AIDS group most of the tumors (38) were lymphomas (34). Other frequent diagnoses have been progressive multifocal leukoencephalopathy (20) and toxoplasmosis (16)--although most AIDS patients already had been treated for toxoplasmosis, and those who responded to it did not undergo biopsy. Initially among the AIDS patients there was a tendency of delayed intracranial bleeding (4 cases, 3 of them fatal). After initiation of a prophylactic coagulopathy protocol no other such

A rapid method for in situ hybridization for viral DNA in brain biopsies from patients with AIDS.

Brain biopsy is often necessary in the diagnosis of neurological complications found in AIDS patients. We describe here a rapid method of tissue preparation and in situ DNA hybridization for detecting JC virus DNA in frozen brain biopsy sections which allows the diagnosis of progressive multifocal leukoencephalopathy to be established on the day of surgery. Once the diagnosis is established, therapeutic and management decisions can be made more easily. The commercial availability of biotinylated probes for several of the DNA viruses most frequently encountered in brain infections of AIDS patie

Evidence for polyomaviruses in human neurological diseases.

Polyomaviruses have been implicated in several chronic human neurological diseases. In progressive multifocal leukoencephalopathy there is an opportunistic and lytic infection of oligodendrocytes of the central nervous system leading to subacute demyelinating disease. Although aspects of the pathogenesis remain undefined, the causal role of the viruses seems firm. Nonpermissive infection of neural cells with resultant cerebral tumors has been suggested by epidemiological studies, virus isolations, and detection of viral antigens and nucleic acids. Nevertheless, it is uncertain whether papovavi

POLYOMA-LIKE VIRIONS IN HUMAN DEMYELINATING BRAIN DISEASE.

Specimens of brain tissue obtained at autopsy from three patients suffering from progressive multifocal leukoencephalopathy (PML) were examined by electron microscopy. In specimens from all three cases particles similar to those of the papova virus group were present, confirming previous observations. By the negative staining method it was possible to define the morphological characteristics of the particles more precisely and it was shown that they are structurally similar to virions of the polyoma-SV40-K type. The need is emphasized for obtaining fresh unfixed diseased tissue from persons su

"One a penny, two a penny", I saw the hot cross bun sign".

The hot cross bun sign is a radiological sign seen on MRI due to pontocerebellar demyelination and loss of neurons along with preservation of the pontine tegmentum and corticospinal tracts which is classically seen in Multiple System Atrophy (MSA). Hot cross buns have been in existence since as early as the 14th century up until the point when Schrag et al. (1998) coupled the appearance of this age-old bread with the T2 imaging characteristics of MSA. Over time the radiological sign has expanded with a differential diagnosis of spinocerebellar ataxia, progressive multifocal leukoencephalopathy

A Perplexing Case of Bladder Mass Biopsy-Proven Neurosarcoidosis.

Sarcoidosis is a multi-organ systemic disease that presents with several clinical manifestations, and patients can develop neurologic complications. Neurosarcoidosis may be life-threatening; therefore, early recognition and treatment are key. Here, we present a case of a 55-year-old African American male who presented with a complaint of dizziness and left-sided weakness; he ultimately received a diagnosis of neurosarcoidosis after elaborate radiographic investigations and bladder mass biopsy. Neurosarcoidosis remains a diagnostic dilemma as it can clinically and radiographically mimic multipl

Cerebrospinal fluid and serum JC virus antibody detection in multiple sclerosis patients treated with natalizumab.

Progressive multifocal leukoencephalopathy (PML) is a complication of natalizumab treatment. In order to identify natalizumab-treated patients at risk of developing PML, we assayed for anti-JC virus (JCV) antibody levels in cerebrospinal fluid (CSF). Serial CSF antibody levels were obtained, with 4 patients showing increases in anti-JCV levels indicating possibly viral activation. In patients with both CSF and serum antibody levels, a comparison showed only a moderate Spearman Rank Correlation Coefficient of 0.38. Our data suggests that serum anti-JCV antibody testing alone may not suffice in

A mechanism-based classification of dermatologic reactions to biologic agents used in the treatment of cutaneous disease: Part 1.

Biologic therapies are an efficacious new method of controlling a number of chronic conditions. Data regarding these medications continues to emerge, giving clinicians a greater understanding of their side effects profiles. The biologic agents used in dermatology, particularly the tumor necrosis factor-alpha inhibitors, have a number of varied dermatologic side effects. In this two-part article, we perform a review of literature regarding the cutaneous side effects of infliximab, etanercept, adalimumab, rituximab, efalizumab, and alefacept. In Part 1, we will discuss cutaneous infections, mali
