Two female siblings with primary idiopathic hypomagnesemia, born to consanguineous parents, are described. Both presented at 6 weeks of age, with convulsions and persistent hypocalcemia (calcium 1.5 adn 1.6 mmol/l; normal range (NR) 2.2-2.6 mmol/l), which could not be controlled with anticonvulsants and/or calcium gluconate. On further investigation they were also found to have hypomagnesemia (magnesium 0.17 mmol/l and 0.22 mmol/l; NR 0.65-1.05 mmol/l). Convulsions and the low serum calcium and magnesium levels were first managed by im and then by oral administration of magnesium supplements.

The most important compartments of Mg metabolism are described: Bone Mg stores correlate closely with serum Mg, which latter depends mainly on intestinal absorption and excretion, predominantly via the kidneys. Since intracellular Mg is mainly bound, the overall intracellular Mg concentration depends largely on the amount of available ligands such as ATP. Serum Mg reflects the Mg content of bone stores. The frequency at which hyper- or hypomagnesemia occurs depends largely on the precision with which the normal range is established. Recommendations for the sampling of serum are made.

This study was designed to assess the effectiveness of identifying serum magnesium abnormalities by comparing physician-initiated requests for this analyte with routine magnesium determinations. Because magnesium abnormalities frequently accompany other electrolyte abnormalities, we measured magnesium in 1033 serum specimens submitted for electrolyte analyses. Physician-initiated requests for magnesium measurements were received for 81 (7.4%) of these specimens. Serum magnesium abnormalities were identified in 546 of the 1033 specimens (hypomagnesemia [less than 0.74 mmol/L], 487; hypermagnese

During the last decades the kidney has emerged as a key organ in the homeostasis of magnesium and disturbances of magnesium metabolism have been described in several renal diseases. Restoring body stores to normal levels should be one of the goals in the conservative treatment of end stage renal failure, since magnesium retention is thought to participate in uraemic toxicity. The kidney is also the target organ of many drugs and increased magnesium excretion can be observed either as an obligatory side effect or as a sign of nephrotoxicity. Magnesium depletion could participate in the vascular
