Six-Year Follow-Up of Immature Tooth Revascularization With Enamel Matrix Proteins.

Novel endodontic revascularization techniques have been considered as alternative options for treatment of immature teeth affected by apical periodontitis. The continuous development of the root canal has been recognized as a major advantage of these techniques over traditional apexification approaches. This article reports a case in which endodontic revascularization using extracellular matrix proteins (EMPs) as a scaffold was utilized to treat an immature maxillary lateral incisor with history of trauma and chronic apical periodontitis. The root canal was accessed and sequentially irrigated

The use of limited cone beam computed tomography in the diagnosis and management of a case of perforating internal root resorption.

AIM: To describe the use of cone beam computed tomography (CBCT) in the diagnosis and management of a maxillary lateral incisor with perforating internal root resorption (IRR). SUMMARY: Root resorption is the loss of dentine or cementum as a result of osteoclastic cell action. IRR occurs exclusively as a result of pulpal inflammation. Until very recently, the diagnosis of internal and external resorptive defects has been limited to the information obtained from conventional radiographic techniques. This case report describes the use of CBCT in the diagnosis and treatment planning of a case of

Endodontic treatment of maxillary first molars presenting with unusual asymmetric palatal root morphology using spiral computerized tomography: a case report.

Variations in root number and canal morphology are challenges for successful endodontic therapy. This report describes 2 endodontically managed maxillary first molars with asymmetric presentation of unusual palatal root morphology. These included the left maxillary first molar with a single palatal root and 2 palatal canals and the right maxillary first molar with 2 separate palatal roots with 1 canal in each root. An accurate diagnosis was confirmed with the help of spiral computerized tomography. This case report discusses this asymmetric variation in the palatal root morphology and highligh

Unusual presentation of active implant periapical lesions: a report of two cases.

Active implant periapical lesion (IPL) is a rare lesion which has been reported as one of the causes of dental implant failures. Usually, an affected implant shows radiolucency in the apical area, while remaining clinically stable. IPL is often accompanied by symptoms of pain, swelling, tenderness, and fistulation. In this paper, we describe two cases of IPL with very unusual findings which led to implant failure. A large IPL associated with an inflammatory cyst in the anterior maxilla, and a mandibular IPL resulting in an extra-oral fistula are presented. The etiology and treatment approaches

Root resorption and apical breakdown during orthodontic treatment of a maxillary lateral incisor with dens invaginatus.

A 13-year-old girl was referred for endodontic treatment of a maxillary right lateral incisor with root resorption and apical radiolucency after orthodontic treatment. Radiographically the tooth had an invagination canal (Oehlers' Type III) and responded positively to pulp testing. The invagination canal extending to the lateral resorption was opened. Vital bleeding tissue was found, and the canal was cleaned and obturated. The pulp in the main root canal remained vital. The radiolucent lesion gradually decreased during the 3-year follow-up. The bone resorption seen here, despite vital tissue

Effect of luxation injuries on permanent teeth.

Trauma to the supporting tissues of the teeth are among the most common dental injuries, leading to such complications as pulp canal obliteration, necrosis and root resorption. The aim of this investigation was to study the outcome for young permanent teeth subjected to luxation injuries. From 108 dental records 171 teeth with injuries to the supporting tissue were selected. The material comprised 130 subluxated, 15 extruded, 9 intruded, 15 exarticulated and 6 laterally luxated teeth in children aged 6-19 years. Apart from luxation, 59 teeth (35%) had additional crown and root fractures. 65% o

Automated Detection and Ordinal Severity Grading of Periapical Lesions on Panoramic Radiographs Using a Lesion-Preserving Tiling and Ordinal-Aware YOLO Framework.

Background/Objectives: Apical periodontitis is among the most prevalent dental pathologies, and its radiographic severity is graded on panoramic radiographs using the ordinal periapical index (PAI 3 < 4 < 5). Existing computer-aided systems typically treat these grades as unrelated classes and down-sample the radiograph to a fixed input, which erases the small periradicular radiolucencies that define early disease. This study evaluated a lesion-preserving, ordinal-aware YOLO framework for the simultaneous detection and severity grading of periapical lesions. Methods: A total of 3924 expert-ann

Inflammatory lesions of the tooth pulp induce changes in brainstem neurons of the rat trigeminal subnucleus oralis.

Neuroplastic changes are known to occur in the CNS in response to injury of peripheral nerves. Previous investigation has demonstrated neuroplasticity in second-order neurons of the subnucleus oralis (SO) of the trigeminal (V) nuclear complex in association with aseptic injury to the tooth pulp. A question arises, therefore, as to whether similar changes occur in response to injury associated with inflammation induced by tooth pulp infection. The effects of tooth pulp infection on the mechanoreceptive fields (RFs) of SO neurons were examined in rats. Infection was established by exposure and r

Genome-wide Association Study Identifies Novel Risk Loci for Apical Periodontitis.

INTRODUCTION: Apical periodontitis (AP) is a common consequence of root canal infection leading to periapical bone resorption. Microbial and host genetic factors and their interactions have been shown to play a role in AP development and progression. Variations in a few genes have been reported in association with AP; however, the lack of genome-wide studies has hindered progress in understanding the molecular mechanisms involved. Here, we report the first genome-wide association study of AP in a large and well-characterized population. METHODS: Male and female adults (n = 932) presenting wit

Berberine mediates root remodeling in an immature tooth with apical periodontitis by regulating stem cells from apical papilla differentiation.

Once pulp necrosis or apical periodontitis occurs on immature teeth, the weak root and open root apex are challenging to clinicians. Berberine (BBR) is a potential medicine for bone disorders, therefore, we proposed to apply BBR in root canals to enhance root repair in immature teeth. An in vivo model of immature teeth with apical periodontitis was established in rats, and root canals were filled with BBR, calcium hydroxide or sterilized saline for 3 weeks. The shape of the roots was analyzed by micro-computed tomography and histological staining. In vitro, BBR was introduced into stem cells f
